The service will have an overall caseload of approximately 110 patients. The referral rate is approximately 3 cases per week and the post holder will be involved with the diagnosis, ongoing management and crisis support of the patients.
 
The post holder will take a leading role in developing the service, supervising junior colleagues and providing lead medical input to a multi- disciplinary team and be required to enter information on the Trust\'s electronic patient record system according to Trust policies. The post holder is expected to work within the multidisciplinary team promoting respect for all professional groups and focusing on coherent and integrated team work.

WHAT IS THE DEPARTMENT & TEAM LIKE? 

The Department provides specialist healthcare to adults who have a diagnosed learning disability. The team is committed to providing evidence-based, safe, and effective care that is centred on each person\'s individual needs and delivers clear outcomes. The service they provide include specialist assessment and support for people with complex and challenging needs, enabling people to access a range of services to keep them healthy, and a range of initiatives to reduce and overcome the health inequalities that can be experienced by people with learning disabilities.

The team works closely with a range of partners, including social care, GPs, voluntary sector, and user-led organisation. They also work with other health professional across the health board to ensure services are accessible for people with a learning disability and that we provide whole person care. The department consists of learning disability nurses, occupational therapist, physiotherapist, speech and language therapist, psychologist, psychiatrist, behaviour practitioners and social workers.
