General Adult Psychiatry Department & Team

The inpatient service features 17 adult acute beds, including an extra-care area, for individuals aged 18 to 65 with severe mental illness requiring admission. The goal is to promote recovery and well-being, helping service users achieve their potential. The service caters to a diverse population with complex needs. The multidisciplinary inpatient team includes doctors (two part-time consultants, staff grade/clinical fellow, and F2), a clinical psychologist, nurses, a social worker, occupational therapists, an art therapist, healthcare assistants, and administrative staff.

The Home Treatment Team functions as a step-up, step-down service to prevent unnecessary hospital admissions and facilitate early discharge. They attend ward reviews, support patients on leave, and assist with discharge planning, also gatekeeping inpatient beds. The team includes two part-time consultants, a 0.5 FTE team manager, two FTE senior practitioners, 5.5 FTE nurse practitioners, one FTE social worker, three FTE senior HCAs, and one FTE occupational therapist. Covering all adult ages, there is an arrangement with an Older Adult Consultant for additional support if needed.

Continued Professional Development

This position offers further training and accreditation within psychiatry, including CESR Portfolio Pathway support, access to specialty training, and the opportunity to develop sub-specialist interests. The post holder will have ample opportunities to develop knowledge, skills, and experience through attendance at relevant courses, conferences, and locally organized CME events. The Mental Health Service is staffed by a dedicated multi-disciplinary team, supported by secretarial and administrative staff. Currently, CAMHS manages 1,627 open cases. In 2022, 926 new referrals were received, with 752 new referrals to date in 2023. The current locum postholder\'s caseload is approximately 90, with most cases having an allocated care coordinator. While most new assessments are conducted by other members of the multi-disciplinary team, some will require an initial medical assessment. On average, there are 1-2 new assessments and 15-20 follow-up appointments per week, depending on case complexity and other commitments.

This multidisciplinary team manages a range of complex cases, including patients with a BMI below 15, those with complex medical or psychiatric comorbidities, pregnant patients, and diabetic patients. Treatments provided encompass individual therapy based on psychodynamic, cognitive-behavioural, and systemic approaches. The service also offers specific pathways for patients transitioning from CAMHS, pregnant patients, and SEED patients, alongside a dedicated Clinic for Eating Disorders and Diabetes, with plans for an ARFID clinic.

Facilities include both an outpatient service and a 15-bed regional inpatient unit, all housed in a self-contained unit on a general hospital site. Annually, the service is expected to handle a caseload of approximately 230 patients, with around 360 new referrals. The team works closely with the Community Mental Health and Wellbeing Teams, the Crisis and Home Treatment Team, and the Gastroenterology Departments at the NHS Trust. This comprehensive service, supported by significant investment through the Community Mental Health Transformation Plan, aims to deliver high-quality, specialized care to patients with eating disorders across the town they service.

The CAMHS Community Eating Disorders Service is a newly expanded, multi-disciplinary team which provides specialist CAMHS input to children and young people with eating disorders. It provides home-based treatment, out-patient treatment and gate-keeps referrals to in-patient services. The team has a very positive reputation and the ED practitioners have won numerous Trust wide and national awards. The team has been successful in attracting Government funding to allow service expansion.

The post holder is Providing consultation, advice and appropriate support to professionals in the wider network of the young person and/or family. The inpatient service consists of one acute assessment and treatment medium secure male 4 4 ward, four treatment and intervention medium secure male wards, one female medium secure ward, one medium secure and one low secure male learning disability ward, one low secure male FICU ward and 4 low secure male wards. There are currently 25 Consultant Forensic Psychiatrists in post, working with 5 senior psychiatric trainees who rotate with posts at other forensic psychiatric services in the region. There are 5 junior psychiatric trainees, coming from the UCH training schemes. The service currently employs 10 specialty doctors, working in the prisons and in the rehabilitation parts of the service.

WHAT IS THE DEPARTMENT & TEAM LIKE? 
The post offered is that of Specialist Psychiatrist in Older Adult Psychiatry, working in both the community mental health team and the memory clinic. In addition to this post, three additional CMHTs make up the older adult community service for the borough. The personnel of the CMHT include: medical staff, Community Psychiatric Nurses, Occupational Therapist, and Psychiatric Social Workers. There is a Psychological Therapy Department which provides therapy for older adults and which works across CMHTs in the Borough as does the Occupational Therapy Service.

The post holder will have responsibility for the diagnostic assessment and treatment of older adult psychiatric patients from the designated catchment area referred to and accepted by the service. Outpatient clinics and domiciliary visits in the community will constitute the majority of the direct clinical care provided by the post holder.

WHAT IS THE DEPARTMENT & TEAM LIKE? 

The service is a team of excellent and forward thinking doctors consisting of: psychiatrists, psychologists, nurses, creative therapists and family therapists. There is currently funding for 4.8 WTE Community Consultant Child & Adolescent Psychiatrists. The service aims to keep children and adolescents at home with increased support if possible. The Trust provides a comprehensive Specialist Child & Adolescent Mental Health Service for young people that are 19 years and under. 

The department has a Crisis Team that operates 24 hours a day, 7 days a week. While the Home Treatment (Intensive Intervention Team) aims to work from 8AM to 6PM, 7 days a week. The services is provided at various locations across the town in order to be able to offer families an accessible and convenient service within a less stigmatising and less threatening environment. There is also a team that works with people aged between 14 and 65, who are experiencing their first episode of psychosis or might be at risk of developing psychosis.

The post holder expected to make assessment of referrals/admissions. The post holder will liaise and collaboratively work with other services/agencies. The successful candidate is expected to have a care plan and treatment formulation, guidance on evidence-based treatment and effectiveness.
