An excellent opportunity for an experienced Consultant in Care of the Elderly - Geriatric Medicine to join a Health Board in North Wales that promotes equality and diversity. The post if suitable for overseas consultants, including candidates who are pursuing CESR. Specialists in Care of the Elderly - Geriatric Medicine holding a specialist certification would be suitable for this post, based on experience entering at either Specialty Doctor or Consultant grade.

WHAT IS HEALTH BOARD LIKE? 
The Health Board is largest health organisation in Wales, that are developing an integrated health service which provides excellent care delivered in partnership with the public and other statutory and third sector organisations.

The Health Board have a budget of £1.3 billion and a workforce of over 17,000 staff. Providing primary, community, mental health and acute hospital services for the population of around 700,000 across of North Wales. They continuously develop their workforce so that it has the right skills and operates in a research-rich learning culture.

Besides their three main hospital sites, they are also responsible for community hospitals, health centres, clinics, mental health units, community teams, GP practices and other NHS services provided by dentists, opticians and pharmacists. They plan to improve the health of the population, with particular focus upon the most vulnerable in our society.

WHAT IS THE DEPARTMENT & TEAM LIKE? 
The department services in the hospital is based on two 24-bedded COTE wards (Wards 1 & 2). Combined acute and rehabilitation stroke beds are available on a specialist Stroke Unit that offers all elements of stroke care, including 24/7 out-of- hours stroke thrombolysis and a newly commenced thrombectomy service. has an average annual attendance of 58,000 patients of whom 22% are above 75 years of age. 70% of the attendees are discharged directly from the ED. 

The Department has a newly established Frailty Intervention Team (FIT), and it is at the hospital’s front door, expecting to provide the opportunity to bring Comprehensive Geriatric Assessment (CGA) in the Emergency Quadrant (EQ) to prevent hospital admission and facilitate patient flow and early discharge of a proportion of frail older people. The Frailty Team will have access to 4-6 beds in the Emergency Department Observation Unit (EDOU). The team will also work in liaison with the Assessment and Discharge Team (ADT) to provide Comprehensive Geriatric Assessment (CGA) and interventions on appropriately selected patients.

The post holder will be required to provide consultant input to the Frailty Intervention Team and advice to the Community Resource Teams to help admission avoidance where appropriate. The post holder is expected to contribute to discharge, D2RA (Discharge to Recover and Assess), flow and transfer planning in conjunction with the MDT, as well as participate in daily board rounds and MDT discussions.
