WHAT IS THE GENERAL ADULT PSYCHIATRY DEPARTMENT & TEAM LIKE?
The post is based with the Community Mental Health Service. This involves providing clinical leadership, offering diagnostic assessments and treatment to service users with severe, complex, and high risk psychiatric conditions.
Adult Community Mental Health Teams operate from two clinical team bases. Both teams provide a range of community based services, including outpatient clinics based on recovery care pathways. The teams provide assessment and treatment to people with complex or severe mental health conditions in HONOS clusters of care 5-17. This includes service users formerly supported by the Assertive Outreach Team and the Community Personality Disorder Services, and those with a diagnosis of ADHD. The Wellbeing team provides assessment and psychological treatment to people with common mental health conditions in HONOS clusters of care 1-4. The wellbeing team is based in Primary Care and requires psychiatric input from the post holder according to the catchment area of the GP practice.
Adult community teams currently support approximately 1500 patients, approximately 400 of whom are subject to CPA. The service receives approximately 16 referrals every week. The post-holder carries in the caseload 4 patients on CTO and 2 on conditional discharge currently. The overall caseload for the post is about 330 service users as a responsible clinician considering clients with a care coordinator; patients under the care of trainees or the specialty doctor and under his/her direct care. The service users followed up in the outpatient clinic are on standard level of care. They attend a weekly multidisciplinary meeting where they can communicate about clinical and administrative matters.
The post holder will be responsible for the assessment and treatment of people of working age from his/her catchment area who are referred or otherwise present to secondary care mental health services. Initial triage by the Single Point of Access (SPA) determines whether the service user meets the criteria for a secondary care assessment.

An opportunity to join a dynamic, enthusiastic and forward thinking respiratory department. This is a full-time post in Respiratory / General Medicine supportive of CESR. The trust is based in South Central England and the department provides the full range of Respiratory services within an established team. The Trust is looking to expand the outpatient services to reduce waiting time and develop services and the new appointee is expected to contribute to its development. He/she will be required to attend the weekly Lung Cancer and COPD MDT and participate in the acute on call rota. There is continuous responsibility for teaching, training and supervision of junior medical staff and Cambridge medical students.
WHAT IS THE NHS TRUST?The Hospital currently provides services for a population of around 270,000, of which 119,600 live in the main county town. The region is primarily agricultural, with numerous pleasant villages within easy commuting distance of the hospital. In recent years most of the expansion in population has been in the M1 / A1 corridor and significant further growth is planned in the surrounding areas. It is anticipated that by 2021 there will be a further 50-55,000 people living in the town area.
The town is a thriving market town, which provides easy access to London (55 miles), the East Midlands, the North of England and beyond through its rail and road links.
WHAT IS THE RESPIRATORY DEPARTMENT & TEAM LIKE?The Lung cancer service is supported by 2 specialist nurses and an admin staff. There is a fully equipped lung function lab, supported by a lung function technician. There is also a screening service for patients with sleep apnoea syndrome. A weekly Radiology and lung cancer MDT meeting is held at Bedford. There are very close links with the regional cardiothoracic centre at UCLH London and Papworth Hospital, Cambridgeshire and two thoracic surgeons attend the weekly MDT. The department has been commissioned to deliver the Integrated COPD service and runs an ARAS (acute respiratory assessment service), ESD (early supported discharge), nurse led outpatient and oxygen clinics, physiotherapy and pulmonary Rehab clinics and a clinical psychology clinic.
Currently they are performing about 280 bronchoscopies a year which is carried out in the Endoscopy Unit as a day case/ward inpatient. There is currently one dedicated EBUS list in the Endoscopy dept. and this is likely to increase to twice weekly. They perform approximately 130 EBUS’s a year. There is a Day Assessment Unit for doing pleural procedures and CT guided lung biopsies. Pleural procedures including indwelling pleural catheters are done under ultrasound guidance. Non-invasive ventilation is provided on the respiratory ward. The Respiratory ward also offers a step down service for patients from ICU.
The Department is also working towards developing new services including treatment for Obstructive Sleep Apnoea Syndrome and Medical Thoracoscopy. Being an associate teaching Hospital of the University of Cambridge Clinical School, the department has attachments of both stage 1 and 3 medical students. The department also has close links with University College Hospital, London and 2 Respiratory consultants have honorary contracts with UCLH.

The town has a varied selection of accommodation for rent or purchase, both in rural and urban surroundings. Education is a prominent feature in the area. Local Trust schools provide first-class independent education for some 4,000 girls and boys supported by preparatory schools and other independent schools that cater for all ranges of ability. The University has a campus in the town; and the College also offers a comprehensive range of courses in Applied Sciences, Health, Education, Business and The Arts. The hospital is situated within walking distance of the town centre.
The town and several villages to the north are situated on a famous River , which is a very attractive feature of the area. The town has two good parks within walking distance of the town centre. A nearby Country Park is within walking distance, offering water sports, nature trails, a visitor centre and marina. WHAT IS THE CAMHS DEPARTMENT & TEAM LIKE?
The clinics accept referrals from a number of sources including General Practitioners, Social Workers, Health Visitors and Consultant Paediatricians. Referrals are processed through SPA (the Single Point of Access). The teams are multi-disciplinary including Child and Adolescent Psychiatrists, Psychologists, Mental Health Nurses, Child Psychotherapists, Systemic Therapists and Social Workers. Medical & Clinical Oncology consultants looking to start or continue their careers in the NHS are invited to join excellent, large specialised hospital Trust on the South Coast of England. The Specialist Oncology Centre is based onsite and provides Cancer services to the local population in addition to parts of Sussex (Chichester) and the Isle of Wight. WHAT IS THE RESPIRATORY DEPARTMENT & TEAM LIKE?
The service are looking for individuals able to run subspecialty respiratory medicine for a population of over 800,000. They wish to develop existing specialist sleep and NIV services and create specialty services in lung cancer and intervention, Bronchiectasis, lung infection, and asthma. Individuals with an interest in any of these sub-speciality areas would be considered.
Both of the NHS trust Hospitals have well established and progressive units providing a wide range of respiratory services to the local population. They pride themselves on their reputation for being a cohesive, supportive and friendly departments. The trust is developing a model of partnership bringing exciting new service development opportunities. To complement the established services they are looking to develop specialist respiratory services to serve the combined catchment areas of both trusts of over 800,000. Growing the department, the service are appointing x2 Consultants, one as a replacement post and one new post to expand our compliment to 6 full time Consultants. They aim to make further appointments in due course with the aim of working towards a 7 day service.
They have recently expanded their provision of undergraduate medical education on both sites and there are opportunities for appointees to be formally involved in their education programme. We would welcome applications from those with an interest in medical education. The Respiratory Consultants do not participate in the acute medical take rota, although any appointee may be able to join the rota if they wish.
You will have regular outpatient responsibilities (2 per week), a specialist service session or teaching, share of the 2 bronchoscopy/EBUS lists each week, partake in the department internal rotation covering respiratory inpatients and providing specialist ward referrals. Along with departmental colleagues, you will provide a specialist on call phone advice service for our ward based NIV unit. The trust has invested in a brand new high dependency respiratory unit with 8 negative pressure rooms. There is capacity to provide CPAP or NIV via 8 new V60 Philips ventilators. They have access to 4 Optiflow devices for high flow nasal oxygen. Each side room is equipment with a Fukuda DS monitor to ICU standard. A procedure room is being created on the new high dependency ward. The trust is currently building a new ambulatory care unit which our department will have access to. The trust is planning to increase ambulatory and interventional services, and would support service development in these areas. They currently have access at Ipswich to Bronchoscopy, Linear EBUS and a full pleural service. They are looking to develop physician led cancer diagnostics with ultrasound guided biopsy and radial EBUS.

Post overview:
In the community service you will contribute to assessment of patients referred to the service for diagnosis and treatment of mental disorders and support the Multi Disciplinary Team (MDT) members who carry out assessment of patients both within the team and in primary care.
The service is committed to a multidisciplinary approach to care and you will actively contribute to MDT meetings and formulation meetings and work in close collaboration with team members and other services.
The team describe themselves as a dynamic multidisciplinary team in community mental health services. You would join a wider group of supportive and motivated consultant psychiatrists across the region.

There is an excellent network of wider consultant support with weekly academic meetings and the opportunity to join a well-established consultant peer group for adult consultants in the area. The team has full time staff grade Dr support, and the team regularly hosts Specialty Trainees, GPVTS Trainees and higher trainees.
The post holder would be encouraged to be both a clinical and educational supervisor for trainees. If successful you will join a friendly team of 12 Consultant Geriatricians providing Acute Frailty & General Medicine expertise; as well as supporting a dedicated Frailty ward which prides itself on providing high quality care for patients, relatives and carers, front door ED assessment for frailty and AMU support for frailty.
There is a potential for joining a 1 in 12 Acute Frailty on call rota (selected take for Frailty only).

The post holder will join a Frailty team offering acute medical services to a catchment population of approximately 400,000.
The position is for 10 PAs with responsibility for:
- inpatient workload
- developing and contributing to sub-speciality clinics / pathways
- contributing to running and the development of Frailty Hubs in the community
- potential out of hours / weekend Acute Frailty On-Call cover (this can be discussed prior to or after joining)

This is an excellent opportunity for doctors who have UK experience, but have not yet completed their CESR, or those are ready to step up into a locum consutlant role after some years working as a senior middle grade in haematology. The Haematology Team The haematology team comprises five substantive consultants, 2 specialty doctors, 1 SpR, 3 junior doctors & 2 clinical nurse specialists who provide inpatient and outpatient services for the surrounding towns. There is an 8b Senior Manager for Blood Sciences, an 8a Transfusion Manager; there is one Band 7 Transfusion Practitioner, 4 band 7, 6 band 6 and 4 band 5 biomedical scientists. A new Band 8a haematology manager post has been approved. The Department & Service: There is also a dedicated haemato-oncology ward with 18 patient capacity as well as day unit for delivery of chemotherapy & a busy outpatient service. A Level 2 Haemato-Oncology service is delivered on dedicated McMillan supported Ward in addition to a specialist Day Chemotherapy Unit for treatment of Haemato-Oncology and Oncology patients. Inpatient capacity is 18 beds most of them are single rooms and there is close collaboration with King’s College Hospital for referral for autologous and allogeneic stem cell transplantation and well as use of specialist integrated haemato-pathology diagnostic services (SIMDS). Currently the Haematology laboratory performs an average 700 FBC tests, 475 coagulation tests and 100 blood bank tests daily. Adult medicine also has an 18 bedded specialist Haematology ward run by our 4 clinical Haematologists. There is also a dedicated outpatient haemato-oncology day unit run by specially trained nurses. The unit is used by Haematology and by the regional Oncology Service as a satellite treatment site from their base. An adjacent medical infusion suite also provides transfusion, blood product and other infusion support; this is also nurse led. The haematology service also has close relationships with ITU and HDU, which regularly provide support for our critically ill patients. Requirements for this role: To be eligible for this role it is advised that you should have the following:
- FRCPath (ideally in full and in haematology)
- MRCP UK
- Extensive expereince at consultant grade in haematology
- Ideally some NHS experience
- Empathetic, caring and team player type of personality
- Willingness to learn, adapt and strive for improvement
