The CMHT psychiatrists provide medical input to the well-staffed Assertive Outreach Team (AOT). The Assertive Outreach service is expanding and recently merged with the Dual Diagnosis service. This Team has input from an experienced manger and a psychologist. The Dual Diagnosis service offers an intensive service to complex patients with significant substance use and associated risks has a designated lead who assesses, manages or offers a consultation for this patient group. The Dual Diagnosis lead is a Non-Medical Prescriber (NMP).
The community mental health services have expanded across the Trust with concomitant expansion in staff as we have built up a new service called Open Mental Health, in the last 2 years. This service works closely with primary care, the wider community and the voluntary sector. This service includes psychologists, assistant psychologists, liaison staff, eating disorder leads and support workers.
The Consultant will deliver direct interventions including assessment, diagnosis and treatment of mental and physical health needs through outpatient clinics at another department and home visits as required, attend the weekly CMHT meeting, and work collaboratively with the inpatient consultant and team based at another Ward to ensure smooth transition for patients admitted and discharged from the ward.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The Team is a group of determined doctors that is based at one of the NHS Trust\'s hospital. The inpatient teams are managed by the ward managers and deputies. The department is very much focused in providing efficient and high-quality inpatient care and in developing and embedding service improvement. The successful candidate will be joining a team that works closely together in a supportive and collaborative manner, and have close links with our community teams to ensure that they provide high quality, safe and effective care. The Consultants have their own office in the administrative area of the hospital with IT access.
The Mental Health Unit has 4 acute admissions wards, each with 17 beds, except for one of the wards that has 16 beds. Admissions are from the local CMHTs, HTTs or Psychiatric liaison services; all admissions are gatekept by HTT. Current bed occupation is at approximately 90-95% but the Trust has a bed occupancy project to reduce this to 85% over the next 12 months. On average 75% of admissions are under the Mental Health Act, with an average of 4 admissions per week. This results in Tribunals and Managers Hearing on average fortnightly. The team therefore works closely with bed managers and HTT to ensure that there is appropriate throughput through the service.
The post holder will have principal duties to provide clinical care to the residents in their covered areas admitted to the ward. The post holder will provide joint leadership with the ward manager, for the performance culture of the ward, ensuring best practice is employed across the ward, and that the Trust’s quality and productivity targets are met. The post holder will be expected to participate in Quality Improvement and service evaluation projects and will be encouraged to undertake any research which may be relevant to either short or long-term service improvement.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The Department is an established high-quality multidisciplinary community & liaison Perinatal Mental Health Service across five Boroughs. The service specification is defined in line with the standards recommended by the RCPsych CR197, the Perinatal Quality Network and the Pan London Perinatal Mental Health Network. In the area served by the NHS Foundation Trust, there are four Maternity Units with a projected delivery rate of more than 6,000/year. In addition, there are approximately 4,000 deliveries in the private sector. The team have the capacity of enabling the delivery of evidence-based perinatal mental health care through a coordinated multidisciplinary specialist approach across maternity, primary and secondary care, tertiary specialist services, social care and voluntary sector.
The service operates at the interface between one of the Trust\'s hospital maternity unit and psychiatry with an outreach arm in the community in the form of home visits, satellite clinics, joint case management with caseload midwifery and a perinatal mental health specialist Health Visitor and NGOs. The service works closely with the liaison psychiatric team with clinical cross cover arrangements for annual and study leave. The Team Consists of: 0.5 WTE team manager, 0.5 WTE consultant psychiatrist in perinatal, 1 WTE specialty doctor (post advertised), 1 WTE Medical secretary, Band 4, 1 WTE Band 8a Psychologist, 0.8 WTE Band 8a Psychotherapist 7, 3 WTE band 7 specialist psychiatric Nurses, 1 WTE Band 6 OT, 0.6 WTE band 7 social worker, 0.6 WTE nursery nurse, and 0.6 WE peer support worker.
The post-holder will work with a medical team consisting of a Consultant Psychiatrist. The post-holder will contribute to the supervision of the trainees/medical students, clinical discussion and support for other non-medical MDT members and is expected to contribute and write perinatal care plans, safeguarding reports for social care and attend pre-birth planning meetings, discharge from maternity or MBU meetings and children and families safeguarding meetings.

WHAT IS THE GENERAL ADULT PSYCHIATRY - INPATIENT DEPARTMENT & TEAM LIKE?
The Ward is a 14 Bed, mixed gender assessment ward based in Worcester. The ward takes all admissions from the community for patients in acute crisis. The ethos of the ward is dynamic assessment, allowing staff to work on identifying, and safely managing patient’s needs. The ward\'s role is to formulate care plans, assess risk and plan each patient’s progression over a 14 day period.

The team is made up of learning disability nurses, occupational therapists, speech and language therapists and psychologists. They deliver a specialist assessment and intervention plan, which focuses on health inequalities, behaviour, risk, sensory, communication and psychological needs. They work in partnership with families and agencies to achieve positive outcomes for the patients in their home, community and other relevant settings.
The Team accepts referrals from service users, carers, social workers, GP\'s and other health professionals. All referrals will go through a screening process which checks whether you are eligible to access the service. In previous years, their colleagues have picked them as winners of the Principles of Care Awards, for their work with Down\'s Syndrome and dementia patients. The population is around 191,830 (ONS Data). At present, around 4144 people, within the borough, have a known learning disability, with the five year forecast showing this to increase to 4236 by mid-2025. Currently, the outpatient case load is around 150.
The post-holder will work under the supervision of a consultant psychiatrist and and Multi Disciplinary team, and will provide clinical support for the assessment and management of mental health problems on a routine and emergency basis. The post-holder will have the opportunity to rotate through different posts on a yearly basis according to service needs, offering opportunities for developing clinical expertise across a range of specialities. The Old Age / Geriatric Psychiatry Mental Health Service provides a range of mental health services for adults across the age range within the community and in hospital settings. These are integrated health and social care services. Many services across the boroughs are now integrated because of the synergies of the two patches, with members of the senior management often having overarching responsibilities. There is a middle/second on call tier through higher trainees and first on call provision is provided by 3 resident junior doctors.

WHAT IS THE GENERAL ADULT PSYCHIATRY DEPARTMENT & TEAM LIKE?
The Adult Mental Health Service provides a range of mental health services for adults across the age range within the community and in hospital settings. These are integrated health and social care services. Many services across the boroughs are now integrated because of the synergies of the two patches, with members of the senior management often having overarching responsibilities. There is a middle/second on call tier through higher trainees and first on call provision is provided by 3 resident junior doctors.
The post holder will provide senior clinical advice and support to the daily triage, MDT and other regular clinical team meetings, ensure that all referrals are triaged daily and responded to in a timely way, and do assessment of new patients and clinical supervision of assessments carried out by other team members. The post holder review complex needs patients to ensure appropriate care packages are put in place, have Mental Health Act related-work, and do domiciliary visits to provide assessment and treatment services as a member of the CMH.

The Team prioritises continuity of care and accepts referrals for patients with both functional and organic presentations. Patients can choose to be seen in a number of settings including their own home, a modern accessible clinic or in their GP surgery. The service offers a comprehensive programme of group and individual therapeutic interventions to cater for different levels of need. There is a separate and specialist care home pathway with HCPs to assess and manage behaviour that challenges; including joint GP/OAP wards rounds & detailed formulations using the Newcastle model. There is additional support from separate and specialist YOD Occupational Therapists for patients with Young Onset Dementia.
The post holder will be responsible for delivering consultant psychiatric input to patients referred (excluding care homes) to and accepted for treatment by the CMHTOP in line with the agreed criteria and commissioned care pathways.

What is the Pathology Group like?
The group comprises four pathology laboratories providing pathology services for acute hospitals and also local GPs. Some of their laboratories also offer specialist services to the wider NHS and work on research studies and trials. As part of a major initiative in the NHS, Pathology in England is being reorganised into much larger local network models and this sees the formation of 29 pathology units across the country.

With almost 200 Consultants based over the four main hospital sites, the trust currently provide anaesthetic and ICU cover in excess of 400 sessions/week across both the Trust and those hospitals with which we have service level agreements.

The Trust is committed to supporting CME / CPD for consultants. There is a Training and Education Task Force at which this specific item is discussed in depth. It is expected that consultants will attend meetings and courses in their area of special interest and that the appointee will register for the CME / CPD programme within the specialty.
There is also a strong clinical governance system in place in the Directorate. The appointee will contribute to Quality, Risk & Clinical Governance within the department in line with national and Trust strategy/guidance and CQC requirements and based on evidence and best practice. The appointee will be encouraged to develop clinical audit and to have regular departmental audit meetings.
Research interests are also encouraged. The hospital has academic links two nearby Universities. The CMHT psychiatrists provide medical input to the well-staffed Assertive Outreach Team (AOT). The Outreach service is expanding and recently merged with the Dual Diagnosis service. This Team has input from an experienced manger and a psychologist. The Dual Diagnosis service offers an intensive service to complex patients with significant substance use and associated risks and a designated lead who assesses, manages or offers a consultation for this patient group. The Dual Diagnosis lead is a Non-Medical Prescriber (NMP).
The community mental health services have expanded across the Trust with concomitant expansion in staff as we have built up a new service called Open Mental Health, in the last 2 years. This service works closely with primary care, the wider community and the voluntary sector. This service includes psychologists, assistant psychologists, liaison staff, eating disorder leads and support workers.
The Consultant will deliver direct interventions including assessment, diagnosis and treatment of mental and physical health needs through outpatient clinics at another department and home visits as required, attend the weekly CMHT meeting, and work collaboratively with the inpatient consultant and team based at another Ward to ensure smooth transition for patients admitted and discharged from the ward.
