THE ONCOLOGY DEPARTMENT AND TEAM:
The department is responsible for SACT services for adults with solid tumours in the region of Wales. The majority (60%) of day-case services are delivered on site, while there is a long-established network of outreach sites in LHB acute and community hospitals, and based with 3rd-sector providers.
Based on its professional and public reputation for delivering excellence in terms of patient and staff experience, the Trust has has been selected to lead the ‘Transforming Cancer Services’ programme and to build a new hospital. The new site will be a £200million pound development and will open in the coming years. It will be nearly twice the size of the current hospital and will be equipped with brand new, state-of-the-art diagnostic and treatment facilities
The new site will have 10 new linear accelerators, 2 CT and 2 MRI scanners to support precision image-guided and adaptive radiotherapy. The Team is a group of excellent & hardworking doctors that provides service in a town with a population of 100,000 with the 16-65 population of 61,000. There are two full time Consultants covering the Community Mental Health Team (this post being one of them). The Community Mental Health team comprises: Operational team managers, Service manager, CPNs, STR Workers, Non-medical prescriber, psychological therapists, and admin staff.
The community mental health team accept referrals from varies agencies including General practitioners, IAPT services, secondary and tertiary agencies. The current caseload is approx. 650 with a referral rate of approximately 4-6 patients per day. Inpatient services are provided in a new purpose-built 40-bedded unit four miles from the team base. A dedicated inpatient consultant psychiatrist and related team provide care for inpatients. The team is also supported by a CRHT service, which deals with all crisis referrals from 9am to 9pm with an on-call service thereafter.
The post holder will have access to dedicated admin with a ratio of 3:1, with consultants also having full access to a secretarial pool for all preparation of letters and reports etc. The post holder is expected to manage, appraise and give professional supervision to junior medical staff as agreed between consultant colleagues and the medical director and in accordance with the Trust’s personnel policies and procedures. The post holder needs to ensure that junior medical staff working with the post holder operate within the parameters of the New Deal and are Working Time Directive compliant.

It is a truly exciting time to be joining the Child & Adolescent Mental Health Service as they are moving towards a county-wide service with clinics at localities. The department provides an assessment and treatment service for young people from the age of 0 – 18 years. The service has an expanded primary mental health staff and has recently been funded to start the Trailblazers project within schools and to support existing clinical services. The Department is an established high-quality multidisciplinary community & liaison Perinatal Mental Health Service across five Boroughs. In the area served by the NHS Foundation Trust, there are four Maternity Units with a projected delivery rate of more than 6,000/year. In addition, there are approximately 4,000 deliveries in the private sector. The team have the capacity of enabling the delivery of evidence-based perinatal mental health care through a coordinated multidisciplinary specialist approach across maternity, primary and secondary care, tertiary specialist services, social care and voluntary sector.
The service operates at the interface between one of the Trust\'s hospital maternity unit and psychiatry with an outreach arm in the community in the form of home visits, satellite clinics, joint case management with caseload midwifery and a perinatal mental health specialist Health Visitor and NGOs. The Team Consists of: 0.5 WTE team manager, 0.5 WTE consultant psychiatrist in perinatal, 1 WTE specialty doctor (post advertised), 1 WTE Medical secretary, Band 4, 1 WTE Band 8a Psychologist, 0.8 WTE Band 8a Psychotherapist 7, 3 WTE band 7 specialist psychiatric Nurses, 1 WTE Band 6 OT, 0.6 WTE band 7 social worker, 0.6 WTE nursery nurse, and 0.6 WE peer support worker.
The post-holder will work with a medical team consisting of a Consultant Psychiatrist. The post-holder will contribute to the supervision of the trainees/medical students, clinical discussion and support for other non-medical MDT members and is expected to contribute and write perinatal care plans, safeguarding reports for social care and attend pre-birth planning meetings, discharge from maternity or MBU meetings and children and families safeguarding meetings.

WHAT IS THE DEPARTMENT & TEAM LIKE?
This established CAMHS MDT is a long established specialist CAMHS service with an excellent reputation for innovation as well as established therapies. It follows a treatment model organised in multiple Care Pathways. All new patients are seen within 9 weeks of accepted referral by clinicians in multidisciplinary teams. The CAMHS service also offers embedded clinical sessions to partner agencies in London Borough. Through this we are able to offer consultation and direct work to mainstream schools, Special Schools and Pupil Referral Units. This service also extends to Children’s social care with clinicians embedded in Assessment and Intervention Teams, Children’s Centres, Youth Offending teams and Looked After Children’s Teams. The CAMHS service is also complemented by a CAMHS specialist eating disorder ‘hub and spoke’ service - the Community Eating Disorders Service for Children & Young People (CEDS-CYP) and an extended crisis service (11 am – 11 pm) across three London Boroughs.

WHAT IS THE GENERAL ADULT PSYCHIATRY DEPARTMENT & TEAM LIKE?
The Department is a modern 17-bedded en-suite unit which comprises doctors, qualified psychiatric nurses, occupational therapist, psychologist and health care assistants, providing assessment and treatment predominately for older people with complex or acute mental health needs, enabling a return to independent living wherever possible. The team accommodates all Older Adult services, including CMHT, Home Treatment Service and Memory Service. On average, there are approximately four new admissions and approximately four discharges per month. The majority of patients are admitted from their usual place of residence with a small percentage coming from the general hospital.

WHAT IS THE NHS TRUST LIKE?The Old Age Psychiatry Team (Memory Assessment and Management Service) provides assessment and initial diagnosis to patients of all ages where there is a suspected dementia. It operates across Newcastle and North-Tyneside. The team works closely with the older adults Community Treatment Teams (CTT), which covers Newcastle North and East; Newcastle West; and North Tyneside. The department includes a specialist Dementia. The team receives approximately 100 referrals per month for patients who are low-complex, and where there is a high likelihood of a dementia diagnosis. Most new referrals are seen by nurses, discussed at the daily MDT meeting, and then offered a diagnostic appointment with a doctor, senior nurse or psychologist.
The Young Onset Dementia Team provides specialist follow up for people living with dementia aged under 65. It is a well-established multi-disciplinary team within Older Adults Community Services, and the sessions allocated provide opportunities for specialist assessment and support for this group of patients, and the chance to develop greater integration with the Old Age Psychiatry Team. Most appointments will be conducted in the out-patient setting, but home visits are also offered for patients based on their clinical need. Referrals are reviewed at a daily triage meeting and new assessments are allocated to the most appropriate member of staff.
The post holder will be expected to see more complex new patients (approximately 1 per week), conduct diagnostic appointments (approximately 10 per week) and to lead the MDT meeting (2 per week). The post-holder is expected to be committed to delivering excellence in clinical care, provide medical leadership to and work effectively with the multidisciplinary MAMS service and Young Onset Dementia teams, promote the safety and well-being of the patients we serve, and to respect the rights and the dignity of patients.
