Oncology in the NHS branches into two main sub-specialisms, medical oncology and clinical oncology. Clinical oncology is the branch of oncology more commonly referred to as radiation oncology outside the UK, though a clinical oncologist's duties encompass both systematic and radiotherapy treatments in the UK. Cancer waiting times are one of the most critical performance indicators an NHS trust is judged upon, and as such oncology is an area with significant demand, as despite large numbers of oncologists registered in the UK, the numbers of qualified oncologists is relatively low when compared to vacancy rates. This means there are many fantastic opportunities available both medical and clinical oncologists interested in joining the NHS, with many NHS trusts offering excellent support with CESR, specialty training applications and general career & sub-specialty development and registration support. Leadership roles with significant career development opportunities are also available to most specialists entering the UK system, and generally motivated clinicians looking to develop in any area of specialisation or leadership in the UK will be supported to do so. 

Requirements for an overseas doctor in these areas:

  • IELTS or OET
  • Medical Oncologists must obtain MRCP
  • Clinical Oncologists must obtain FRCR (Oncology)
  • EEA applicants only require IELTS or OET (plus their Recognised European Qualification) for GMC registration

What can you expect to do as part of a typical working week in the NHS?

Clinical oncologists often work in specialist cancer hospitals, large acute teaching hospitals, district general hospitals or private sector clients. Radiotherapy treatments sessions, multidisciplinary team meetings, outpatient clinic, consultations and follow-ups, inpatient ward rounds, reports writing for GPs, teaching sessions for juniors and trainees and very importantly, research, will all form part of your working week. Research is an integral part of clinical oncology.

Medical oncologists work to provide the best possible outcome for cancer patients, whether that is cure, or palliation and prolongation of good quality life. In addition, they provide counselling for patients and their families, whilst clinical research is an important feature of their role like clinical oncologists. Clinical care, clinical trials, laboratory and translational cancer research form an integral part of the training, and they are trained to work as part of a multidisciplinary team, being able to advise on all aspects of treatment including surgery and radiotherapy as well as having the skills to administer systemic therapies. A medical oncologists work also includes diagnosing cancers, discussing treatment options with patients, arranging and supervising drug treatment and therapies including the management of any complications that may arise and supporting patients and overseeing their care, including their lives after treatment.

Medical oncologists treat patients who have localised or metastatic malignancy in need of systemic therapy. They also treat patients whose cancer has potentially been cured by surgery but for whom further therapy improves their outlook.

Medical and clinical oncologists often work in partnership together, and clinical oncology covers both the therapeutic administration of ionising radiation (radiotherapy) and cytotoxic chemotherapy. However, only clinical oncologists administer radiotherapy.

THE TRUST:

The Trust consists of 10,000 employees, and provides services to approximately 700,000 people living in surrounding areas.  

These services form an integral part of South-Eastern Cancer Services. The Department of Oncology is managed within the Oncology, Haematology, Radiotherapy and Breast Services Clinical Delivery Group which is part of ESNEFT’s Cancer & Diagnostics Division.

THE ONCOLOGY DEPARTMENT AND TEAM:  
The Oncology Team is located across two sites. There is also an opportunity and an expectation that some cross site working will take place in the future.  The Department has a full range of equipment, which includes four matched Varian TrueBeam Linacs, undertaking IGRT and IMRT. Localisation is carried out on a Toshiba Wide Bore CT scanner and planning is carried out using Prosoma Virtual Simulation Software. All of these units are fully networked with the Eclipse planning systems via the ARIA network. There are full mouldroom facilities and an Orthovoltage and Superficial unit as well as HDR brachytherapy.

The Trust provide state-of-the-art facilities in which to deliver first class medical care and the most advanced Radiotherapy and Chemotherapy treatments, alongside a range of complementary and alternative therapies, in a purpose-built sympathetic environment, which enables the best possible holistic and multi-disciplinary approach to the care of all cancer patients and their loved ones. There are currently 28 beds on West Bergholt Ward for Oncology and Haematology patients. The Oncology Department has close links with St Helena Hospice and Farleigh Hospice, both of which provide in-patient and day-care facilities. The hospital Specialist Palliative Care team consists of a Palliative Care consultant and several Nurse Practitioners.

The department has three linear accelerators and a ‘decant’ bunker. Each machine has 2 or 3 photon energies, a range of electron energies, IMRT and VMAT, CBCT capabilities (Varian on board imager) and respiratory gating, the latest Truebeam also has 6DoF couch. In addition both TrueBeam have flattening filter free energies.

CAREER DEVELOPMENT OPPORTUNITIES:

The post-holder will be required to participate in programmes for teaching medical students, training junior doctors and in clinical examinations. THE ONCOLOGY DEPARTMENT AND TEAM:

The department is responsible for SACT services for adults with solid tumours in the region of Wales. The majority (60%) of day-case services are delivered on site, while there is a long-established network of outreach sites in LHB acute and community hospitals, and based with 3rd-sector providers.

Based on its professional and public reputation for delivering excellence in terms of patient and staff experience, the Trust has has been selected to lead the ‘Transforming Cancer Services’ programme and to build a new hospital. The new site will be a £200million pound development and will open in the coming years. It will be nearly twice the size of the current hospital and will be equipped with brand new, state-of-the-art diagnostic and treatment facilities

The new site will have 10 new linear accelerators, 2 CT and 2 MRI scanners to support precision image-guided and adaptive radiotherapy. There will be a dedicated clinical trials treatment unit including a 10-bedded early phase trials unit, and 2 isotope cubicles for molecular radiotherapy treatments.

CAREER DEVELOPMENT OPPORTUNITIES:

Doctors wishing to pursue specialty training in either Medical or Clinical Oncology will be served well by time working in such a developed cancer centre. That being said, the department have overseen the CESR progress of many overseas doctors and have good understanding and experience of it. For more senior doctors holding FRCR or MRCP qualifications this may be the best route to attaining Specialist Registration in your oncological specialty. 

The Trust provides fantastic career development opportunities and once middle grade doctors have attained their CESR in Clinical or Medical - there is real opportunity to begin specialising and leading on any major cancer site within the NHS cancer pathway. Due to recent funding the Trust has been able to purchase five new state of the art Varian Trubeam Linear Accelerators across its Oncology Centre. Over the 2 sites we have 9 Linear Accelerator Treatment Machines and 3 CT Planning Simulators.  

The research department is made up of more than 60 dedicated staff and recruits around 3000 patients a year to clinical studies across 24 different specialities. The trust benefits from being near academic partners such as Universities and local Medical Schools (KMMS). Additionally, the trust has studies open that are interventional and observational, as well as studies that investigate genetic profiling. Within radiotherapy, the Trust are also undertaking SABR trials. We have many clinicians that support oncology research trials within various specialities.

Chemotherapy is prepared in a satellite pharmacy within the Unit. Treatment is provided by experienced chemotherapy nurses who are well versed in cannulation and administering chemotherapy. There are plans to move the unwell assessment area to the inpatient ward very shortly. They have 70 dedicated oncology beds across the trust and provision of side rooms for neutropenic patients and special requirements. The Oncology service is well supported by a well-established Haematology Service, Palliative Medicine Service including inpatient unit, Clinical Psychologists, interpreters and bereavement support staff.

Radiotherapy, Brachytherapy and Radiotherapy Physics are housed together in a purpose-built centre, with the following equipment:
  • 4 Varian TrueBeam Linear Accelerators
  • Aria Oncology management system and Eclipse treatment planning.
  • HDR Brachytherapy Unit
  • Xstrahl Superficial Xray therapy – 80kV & 120kV
  • Toshiba wide-bore CT-simulator, with Prosoma Virtual simulation

The hospitals are located in the north east midlands with great access to the North Westerly cities, Birmingham, London and the rest of the UK with its excellent national transport connections. The local region is beautiful, not far from the world-famous Peak District National Park and home to some of the UK’s most beautiful and dramatic countryside with nature, fresh air and wildlife being the cornerstone of this stunning part of the UK. It is not just a rural area however, and the area is well developed with two cities and several towns offering a feasible location for individuals to set up home.

ARE YOU ELIGIBLE FOR THIS ROLE?
Requirements for this role include:
  • 2 years+ consultant experience (NHS experience is desirable but not essential)
  • MRCP UK 
  • SCE Medical Oncology
  • GMC eligibility 
  • IELTS or OET either passed or in process
