WHAT IS THE DEPARTMENT LIKE?
The Community Mental Health Team provides an assessment and treatment service for people who are experiencing difficulty with their mental health based on episodic model of care. The CMHT works closely with local Crisis Team, Liaison service and Inpatient services to ensure safe and timely transitions when required. The Community Mental Health Team provides a service for men and women over the age of 18 years who require a period of assessment and treatment.

Patients are generally care coordinated by the wider team but the post holder can expect to be the Lead Professional in caring for some of these patients. Work allocation is divided between the consultants according to the patient’s GP practice, however fairness and equity of caseload is always ensured. The team receives around 25 referrals a week. Each Specialist would be expected to attend one SPA (single point of access) meeting and one PAM (post assessment meeting) on one day per week.

WHAT IS THE GENERAL ADULT DEPARTMENT LIKE?
The Community Mental Health Assessment and Recovery Team provides a service for adults with severe and enduring mental health conditions. For example, this could include:

• depression which is difficult to treat, reoccurs or has a significant impact upon your ability to get on with daily tasks
• personality disorders
• specific disorders such as moderately severe and severe eating disorder, severe obsessive compulsive disorder, phobias and severe anxiety
• people with a dual diagnosis such as learning disability, or a substance misuse problem together with a mental health problem

The team is made up of a number of people including mental health nurses, social workers, doctors, occupational therapists and psychologists.

WHAT IS THE OLDER ADULTS DEPARTMENT & TEAM LIKE? 
Based out of a London Old Age Community Mental Health Team, you will provide leadership and supervision for a multidisciplinary community mental health team (CMHT) for older people and will work closely with the Memory Service (MS), providing clinical input to that service. You will have responsibility for the diagnostic assessment and treatment of older adult psychiatric patients from the designated catchment area referred to and accepted by the service. Outpatient clinics and domiciliary visits in the community will constitute the majority of the direct clinical care provided by the Consultant. There will be a one day a week commitment to the Memory Service.
The personnel of the CMHT include: medical staff, Community Psychiatric Nurses, Occupational Therapist, and Psychiatric Social Workers. There is a Psychological Therapy Department which provides therapy for older adults and which works across CMHTs in the Borough as does the Occupational Therapy Service. A secretarial team provides administrative support.

WHAT IS THE THE LIAISON PSYCHIATRY TEAM LIKE?
Based in Preston, this service provides:

• A rapid mental health assessment service for individuals aged 16 and over who present to the Accident & Emergency Department and Medical Assessment Unit of the Acute Trusts.

• Mental health assessments and advice for clients who are in-patients on medical wards within the Acute Trusts

• Comprehensive risk and mental health assessments to a standardised level of best practice

• Advice and support to colleagues within the Acute Trusts

• Signposting/referral onto relevant statutory and non-statutory agencies as identified, including Single Point of Access (SPOA), Perinatal Community Mental Health Teams (PCMHT), Home Treatment Teams (HTT), Substance Misuse Services and Housing and Emergency Social Services Team in response to client need

• Improved communication between the Accident and Emergency Department, Mental health services and other departments within the Acute Trust Hospital setting in relation to patient care and management
