In addition to providing a wide range of patient services, we also contribute to training the next generation of nurses, doctors and therapists and conduct research that will help to advance clinical practice and treatments in the future.
The NHS Foundation Trust support children and young people’s mental health through their schools projects across Somerset, and their child and adolescent mental health service (CAMHS). For adults, we offer a range of community-based mental health services. For those with more serious health needs, our community mental health teams offer specialist care to support recovery. Patients who require intensive, daily support and who may otherwise be likely to be admitted to in-patient care are seen in our home treatment service.
WHAT IS THE DEPARTMENT & TEAM LIKE?
The Service is divided into four Teams, and there is also a Central County Wide Memory Assessment service (which undertakes much of the MAS work) and also a County Wide Intensive Dementia Support Service IDSS (which acts as a Crisis/Home Treatment Team for those with Dementia). The community mental health teams and memory services provide psychiatric community care to patients over the age of 65. There is also a much smaller group of patients under 65 years of age, with primary dementias who are also cared for by the older adults’ community mental health team.
The multi disciplinary team includes medical, nursing, Psychology, OT with clinical support workers and admin support staff. The overall team provides CMHT, MAS and Specialised Day Hospital services. The two services area covers GP practices in four towns and equates to approximately 11,000 patients over the age of 65 (CCG figures December 2017). The community mental health teams are part of wider integrated teams. Each team is co located with staff from district nursing and the integrated rehabilitation team (community OT and Physiotherapists).
The post holder will hold regular clinics, as well as undertaking domiciliary visits when required. The post holder will provide assessment, diagnosis and treatment of mental health needs of community patients in their community area, and work closely with the community teams to provide effective multidisciplinary collaborative care: this will include availability for consultation, regularly attending team meetings and providing case discussion and advice at the request of team members and / or team managers.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The Service, which is currently based within the Adult and LD Mental Health Division, is one of the highly performing mental health services at the NHS Trust, and has two wards, both with 15 beds. The service admits adults of working age from west of the county including 4 towns. Between November 2020 and October 2021, there were 299 admissions to all adults mental health units, that gives a rate of 101 admissions per 100,000 people in the town. Of the 599 patients admitted to all adult mental health units, 295 were informal and 302 were detained under the Mental Health Act.
This post does not include any community responsibilities. There are 3.5 WTE consultants in the Community Mental Health Service and 1.0 WTE in Home Treatment team (HTT). Community teams including the Home Treatment Team are located on site. Also on site can be found the local Section 136 suite, and another Ward, which provides inpatient services to older adults.
The post holder will deliver direct interventions including assessment, diagnosis and treatment of mental and physical health needs of the inpatients in the ward, be a responsible clinician for Inpatients detained under the Mental Health Act, undertake MHA assessments as the Trust Section 12 doctor for patients in their care, and regularly attend Inpatient ward multidisciplinary team meetings and ward handovers.

The country has a GP registered population of approximately 525,000. There’s so much to explore from stunning beaches, green spaces, woodlands, lakes, a wide range of landscapes that includes a beautiful National Park. Whilst tourism and agriculture are important, the county has a number of industrial concerns and a thriving local economy. The area provides access to the M5 motorway and a mainline intercity rail link. Travel connections are convenient with the main line station offering trains to London Paddington in less than 2 hours.
