What should I know about Psychiatry in the UK?
NHS psychiatrists may see in-patients in a hospital setting, or work within the community. Psychiatrists work across a range of services, such as specialist hospital units, schools, residential care homes, or a combination of settings. However, most work in community mental health teams, out-patient departments and hospital wards.
There are six psychiatry specialties: adult psychiatry, psychiatry of old age, child and adolescent psychiatry, forensic psychiatry, psychiatry of intellectual disability, and psychotherapy. It is possible to sub-specialise in adult psychiatry - in liaison, perinatal, substance misuse or addictions, neuro-psychiatry, eating disorders, rehabilitation and social psychiatry.
What can you expect to do as part of a typical working week in the NHS?
Psychiatrists in the UK often follow a regular Monday-Friday daytime pattern. On call work, especially at associate specialist or consultant level, is generally less onerous than other medical and surgical specialties. The regular work pattern enables an excellent work-life balance.
Hospital work can include ward rounds for acute or long-term patients as well as outpatient clinics. New patients may have appointments lasting an hour, considerably longer than appointments given in other medical specialties. Follow-up appointments may last around 20 or 30 minutes.
Most consultant psychiatrists have around ten hours allocated each week for non-clinical activities including research and audit.
Psychiatrists sometimes see patients on their own in an out-patient clinic. More often, they work as part of a team with colleagues from other professions such as nursing, social work, psychology and occupational therapy.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The service (generally for people aged 16 to 64) has been running since 2008, according to a model whereby their team of consultants are deployed as follows: three consultants work exclusively in the community and outpatient areas, one consultant works with the Early Intervention Service, two consultants cover the inpatient area and one works with our Home Treatment and A&E Liaison Team. Within this arrangement, the consultant appointed to this post will work with other sector consultants (East, West and South sector community outpatients).

The community mental health teams and memory services provide psychiatric community care to patients over the age of 65. There is also a much smaller group of patients under 65 years of age, with primary dementias who are also cared for by the older adults’ community mental health team.
The multi disciplinary team includes medical, nursing, Psychology, OT with clinical support workers and admin support staff. The overall team provides CMHT, MAS and Specialised Day Hospital services. The two services area covers GP practices in four towns and equates to approximately 11,000 patients over the age of 65 (CCG figures December 2017). The community mental health teams are part of wider integrated teams. Each team is co located with staff from district nursing and the integrated rehabilitation team (community OT and Physiotherapists).
The post holder will hold regular clinics, as well as undertaking domiciliary visits when required. The post holder will provide assessment, diagnosis and treatment of mental health needs of community patients in their community area, and work closely with the community teams to provide effective multidisciplinary collaborative care: this will include availability for consultation, regularly attending team meetings and providing case discussion and advice at the request of team members and / or team managers.

The Department is a 14 bedded ward which admits adults of working age. The district covers a largely rural area of 285 square miles with a population of approximately 114,000. There are 275 admissions per 100,000 adult population in Somerset. Over the last three years there was an average of 223 admissions per year with bed occupancy being lower than the national average. The majority of the patients admitted were given a diagnosis of a psychotic illness, followed by a mood disorder and then personality disorders.
The post holder will be part of the team which consists of a ward manager 0.5WTE, a deputy ward manager 1WTE, a trainee doctor 1.WTE, a medical secretary 0.4WTE, a ward administrator 1.0WTE, a ward psychologist 0.5WTE, three occupational therapists, 11 staff nurses, 17 health care assistants, and 6 service assistants.
The post holder will also be responsible for conducting Mental Health Act assessments for adults of working age admitted to the ward. Other Mental Health Act assessments for adults of working age in Mendip will fall to the community adult consultant psychiatrists. The post holder may be invited to contribute.

With the increase in primary care diagnosis of dementia the number of memory assessment service referrals is expected to reduce.

For adults, we offer a range of community-based mental health services. For those with more serious health needs, our community mental health teams offer specialist care to support recovery. Patients who require intensive, daily support and who may otherwise be likely to be admitted to in-patient care are seen in our home treatment service.
WHAT IS THE DEPARTMENT & TEAM LIKE?
The CMHT psychiatrists provide medical input to the well-staffed Assertive Outreach Team (AOT). The Assertive Outreach service is expanding and recently merged with the Dual Diagnosis service. This Team has input from an experienced manger and a psychologist. The Dual Diagnosis service offers an intensive service to complex patients with significant substance use and associated risks has a designated lead who assesses, manages or offers a consultation for this patient group. The Dual Diagnosis lead is a Non-Medical Prescriber (NMP).
The community mental health services have expanded across the Trust with concomitant expansion in staff as we have built up a new service called Open Mental Health, in the last 2 years. This service works closely with primary care, the wider community and the voluntary sector. This service includes psychologists, assistant psychologists, liaison staff, eating disorder leads and support workers.
The Consultant will deliver direct interventions including assessment, diagnosis and treatment of mental and physical health needs through outpatient clinics at another department and home visits as required, attend the weekly CMHT meeting, and work collaboratively with the inpatient consultant and team based at another Ward to ensure smooth transition for patients admitted and discharged from the ward.

Taunton offers close proximity to both the beaches of North Devon and Cornwall, while also being less than two hours from the city of London.

WHAT OPPORTUNITIES ARE AVAILABLE TO YOU?
There is real opportunity to progress through CESR at pace at this trust, with a strong desire for newly-qualified consultants to develop their specialty areas of interest and undertake in various cancer trials and research projects where possible. Research is encouraged here, particularly when this is appropriate to professional development and to the service development of the department. Opportunities are available with applications for funding co-ordinated by the Trusts Medical Directors and clinical trial recruitment to International, National and Regional trials is active in all tumour sites. This trust was also rated as an area of Outstanding Practice by CQC (2016), and with the highest GMC score in the South West for training.
