What should I know about Psychiatry in the UK?  

NHS psychiatrists may see in-patients in a hospital setting, or work within the community. Psychiatrists work across a range of services, such as specialist hospital units, schools, residential care homes, or a combination of settings. However, most work in community mental health teams, out-patient departments and hospital wards. 

There are six psychiatry specialties: adult psychiatry, psychiatry of old age, child and adolescent psychiatry, forensic psychiatry, psychiatry of intellectual disability, and psychotherapy. It is possible to sub-specialise in adult psychiatry - in liaison, perinatal, substance misuse or addictions, neuro-psychiatry, eating disorders, rehabilitation and social psychiatry.

What can you expect to do as part of a typical working week in the NHS?

Psychiatrists in the UK often follow a regular Monday-Friday daytime pattern. On call work, especially at associate specialist or consultant level, is generally less onerous than other medical and surgical specialties. The regular work pattern enables an excellent work-life balance. 

Hospital work can include ward rounds for acute or long-term patients as well as outpatient clinics. New patients may have appointments lasting an hour, considerably longer than appointments given in other medical specialties. Follow-up appointments may last around 20 or 30 minutes.

Most consultant psychiatrists have around ten hours allocated each week for non-clinical activities including research and audit. 

Psychiatrists sometimes see patients on their own in an out-patient clinic. More often, they work as part of a team with colleagues from other professions such as nursing, social work, psychology and occupational therapy.

WHAT IS THE NHS FOUNDATION TRUST LIKE? 

Formed on the 1st of April 2020, the Trust is on a mission to deliver outstanding care through a culture of listening, learning and continuous improvement. The NHS Trust provides mental health services to all age ranges and supports people with a range of mental health problems whether they are mild, moderate or severe.

The NHS Trust will be running community services, mental health and learning disability services with a workforce over 9,000 people who deliver support for the patient services. WHAT IS THE DEPARTMENT & TEAM LIKE? 

The Service is divided into four Teams, and there is also a Central County Wide Memory Assessment service (which undertakes much of the MAS work) and also a County Wide Intensive Dementia Support Service IDSS (which acts as a Crisis/Home Treatment Team for those with Dementia). The community mental health teams and memory services provide psychiatric community care to patients over the age of 65. There is also a much smaller group of patients under 65 years of age, with primary dementias who are also cared for by the older adults’ community mental health team.

The multi disciplinary team includes medical, nursing, Psychology, OT with clinical support workers and admin support staff. The overall team provides CMHT, MAS and Specialised Day Hospital services. The two services area covers GP practices in four towns and equates to approximately 11,000 patients over the age of 65 (CCG figures December 2017). The community mental health teams are part of wider integrated teams. Each team is co located with staff from district nursing and the integrated rehabilitation team (community OT and Physiotherapists).

The post holder will hold regular clinics, as well as undertaking domiciliary visits when required. The post holder will provide assessment, diagnosis and treatment of mental health needs of community patients in their community area, and work closely with the community teams to provide effective multidisciplinary collaborative care: this will include availability for consultation, regularly attending team meetings and providing case discussion and advice at the request of team members and / or team managers.

The county has a GP registered population of approximately 525,000. The Mental Health Crisis Assessment Service (MHCAS) was launched at the end of 2022 and is based in Central London. The service offers a calm and therapeutic mental health setting to treat the majority of emergency mental health presentations, seeing and asses anyone over 18 years old that would otherwise go to an emergency department for mental health reasons and does not have an urgent medical need (for example, an overdose of medication). It offers full emergency mental health assessments and onward care planning in the department with a diverse and highly skilled mental health workforce incorporating nurses, doctors, support workers and access to drug and alcohol workers and psychology. The service can support every aspect of emergency mental health care planning from GP referrals to Mental Health Act Assessments.

The majority of services are provided in the community, which means treating people in their homes or from clinics close to home. In this sense this trust is a very modern part of the NHS, something the new Long Term Plan wants to see more of, with better integration and coordination of care, delivered as close to home as possible . Where community care is not possible they offer a number of facilities to treat people in hospital or residential environments. The trust also provides healthcare in prisons in London and the surrounding areas.

The postholder may be invited to contribute to assessments of patients admitted to the Section 136. This Consultant post does not include any community responsibilities

WHAT DOES THE INPATIENT DEPARTMENT LOOK LIKE? 

The wards admit a mixture of Informal and detained patients. Detained patients account for 60% of patients on the wards. Delayed transfer of care patients account for 10% of the wards population. The wards admit adults with various psychiatric disorder and majority suffer with severe enduring mental illness. Patients with personality disorder tend to account for 40% of the wards population and they tend to have the below average length of stay. The post holder is expected to support patient flow team in their effort to facilitate discharges from the ward. The current length of stay (LOS) on our acute ward is 50 days but we are working reducing this to 25 days.

The Crisis Service, which provides for the assessment of emergencies during normal working hours and at the weekend during the day and by the HOPE team for more intensive outpatient intervention (see below for more details); Extended HOPE provides consultation and assessment to young people in crisis, out of normal working hours. Further, the eating disorders team and the Specialist CAMHS team for adopted and looked after children (established in early 2010) provide input across the county. Access CAMHS provides a single point of access for both CAMHS community team and Primary Mental health team referrals.

The post holder is expected to provide a diagnostic, assessment and treatment service for children and adolescents with a mental health problem. The General Adult Psychiatry department is a team of innovative & hardworking doctors. The Site Hospital is the new purpose built 60 bedded working age adult in-patient Hospital. The Working Age Adult psychiatric services are organised into a community service comprising a Community Mental Health Recovery Services. The Acute Service component comprises a Home Treatment team covering the boroughs. The home treatment team ‘gate keeps’ admissions to the inpatient units.

In patient services aims to provide timely assessments and promote recovery of people with a wide range of mental health needs. The goal is one of therapeutic engagement within a recovery framework. There is a multidisciplinary team who develop individual care plans working with each service user. The inpatient wards interfaces with home treatment teams, psychiatric liaison services, Section 136 places of safety, community mental health recovery services, and the wider criminal justice system.

It is expected that the post holder will provide Consultant Psychiatrist input and Responsible clinician cover for a 18-bedded ward mainly serving the population of aligned to the wards alongside another Consultant.
