WHAT IS THE DEPARTMENT & TEAM LIKE?

The Department & Team is looking for a a Specialty Doctor with the Newcastle and Gateshead Older Persons Crisis Resolution and Home Treatment Team (CRHTT). The Team has been set up in April 2020 to provide Crisis services to Older Persons in Newcastle and Gateshead and includes this post. It is based with but not part of the well- established Newcastle Gateshead Adult Crisis Team. The OP Crisis Team consists of One Full Time Consultant Psychiatrist, a Clinical Lead, 5 Band 6 Nurses and 2 Band 3 workers; these staff have been recruited and are currently working as an extended hours service, 8am – 9pm and share a Team Manager with the Adult Crisis Team. The post will be normal working hours i.e. Monday – Friday 9am - 5pm.

The multidisciplinary team will provide older adults with both functional and organic mental illnesses with emergency assessment, home based treatment and early discharge support. The team will see all emergency referrals requiring assessment within its extended hours and provide home based treatment as an alternative to admission as well as gate-keeping OP’s acute inpatient beds. It works on a crisis resolution framework, remaining involved until the crisis has settled. It interfaces with inpatient services, acute day services, specialist community mental health teams (SCMHTs), OP Mental Health Day centres, Memory Services, OP Liaison Services, General Practitioners and Nursing/Residential Homes.

The post holder is expected to provide medical assessment, diagnosis and formulation to the OPCRHT patients. On completion of assessment, to provide and instigate treatment plans for patients receiving home-based treatment, to provide regular review of patients receiving home-based treatment, to provide (or ensure referral to the appropriate service for the provision of) up to date, evidence-based interventions for the pts of the OPCRHT, and to collaborate with GP’s, hospital doctors and primary care clinicians regarding the physical health care of the OPCRHT patients.

WHAT IS THE DEPARTMENT & TEAM LIKE? 

The Team is a group of collaborative consultants who always makes sure that their patients get the best service. The department is well established with good support from senior clinical and managerial colleagues. They accept referrals from the SPA, Primary Care Liaison Team, Crisis Resolution Team, Inpatient wards, Child and Adolescent Mental Health Team as well as national Early Intervention services for service users with a first episode of psychosis. It normally works with service users for 3 years from age of 14 upwards. It functions between 9am and 5pm between Monday and Friday.

The department works in conjunction with other local mental health teams, and has developed close links with a range of community and youth services. This specialist multidisciplinary, community based team, provides intensive input to young people between the ages of 14 and 35 plus, who have experienced a first episode of psychosis. In the future it is anticipated that the service will extend the hours of operation from 8am to 8pm seven days a week. The team has a Key Performance target to review all new referrals within 2 weeks and meet the level 3 standard for Early Intervention services.   

The post holder is expected to be responsible for carrying out detailed psychiatric assessments and risk assessments and to contribute to planning and implementing service users care plans, under CPA procedures and guidelines. The post holder is expected to carry out the above in a variety of settings, including home visits, and other non-stigmatising settings. The successful candidate is expected to work flexibly to ensure that clients and their carers are able to access the service effectively.

WHAT IS THE DEPARTMENT & TEAM LIKE? 

The service (generally for people aged 16 to 64) has been running since 2008, according to a model whereby their team of consultants are deployed as follows: three consultants work exclusively in the community and outpatient areas, one consultant works with the Early Intervention Service, two consultants cover the inpatient area and one works with our Home Treatment and A&E Liaison Team. Within this arrangement, the consultant appointed to this post will work with other sector consultants (East, West and South sector community outpatients).

The Service is divided into four Teams, and there is also a Central County Wide Memory Assessment service (which undertakes much of the MAS work) and also a County Wide Intensive Dementia Support Service IDSS (which acts as a Crisis/Home Treatment Team for those with Dementia). The community mental health teams and memory services provide psychiatric community care to patients over the age of 65. There is also a much smaller group of patients under 65 years of age, with primary dementias who are also cared for by the older adults’ community mental health team.

The multi disciplinary team includes medical, nursing, Psychology, OT with clinical support workers and admin support staff. The overall team provides CMHT, MAS and Specialised Day Hospital services. The two services area covers GP practices in four towns and equates to approximately 11,000 patients over the age of 65 (CCG figures December 2017). The community mental health teams are part of wider integrated teams. Each team is co located with staff from district nursing and the integrated rehabilitation team (community OT and Physiotherapists).

The post holder will hold regular clinics, as well as undertaking domiciliary visits when required. The post holder will provide assessment, diagnosis and treatment of mental health needs of community patients in their community area, and work closely with the community teams to provide effective multidisciplinary collaborative care: this will include availability for consultation, regularly attending team meetings and providing case discussion and advice at the request of team members and / or team managers.

WHAT IS THE DEPARTMENT & TEAM LIKE? 

The Department is a 14 bedded ward which admits adults of working age. The district covers a largely rural area of 285 square miles with a population of approximately 114,000. There are 275 admissions per 100,000 adult population in Somerset. Over the last three years there was an average of 223 admissions per year with bed occupancy being lower than the national average. The majority of the patients admitted were given a diagnosis of a psychotic illness, followed by a mood disorder and then personality disorders.

The post holder will be part of the team which consists of a ward manager 0.5WTE, a deputy ward manager 1WTE, a trainee doctor 1.WTE, a medical secretary 0.4WTE, a ward administrator 1.0WTE, a ward psychologist 0.5WTE, three occupational therapists, 11 staff nurses, 17 health care assistants, and 6 service assistants. 

The post holder will also be responsible for conducting Mental Health Act assessments for adults of working age admitted to the ward. Other Mental Health Act assessments for adults of working age in Mendip will fall to the community adult consultant psychiatrists. The post holder may be invited to contribute.

There is an emphasis on members of the CMHT seeing the majority of initial assessments with a day time duty rota dealing with urgent referrals. This helps to minimise consultant emergency work and keeps their individual community caseload at a manageable size. There are approximately 200 CMHT referrals each year, and approx. 150 Memory Assessment Service referrals in Exmoor and Minehead. With the increase in primary care diagnosis of dementia the number of memory assessment service referrals is expected to reduce. 

The post holder will provide assessment, diagnosis and treatment of mental health needs of community patients in their community area, and work closely with the community teams to provide effective multidisciplinary collaborative care: this will include availability for consultation, regularly attending team meetings and providing case discussion and advice at the request of team members and / or team managers.

There is real opportunity to progress through CESR at pace at this trust, with a strong desire for newly-qualified consultants to develop their specialty areas of interest and undertake in various cancer trials and research projects where possible. Research is encouraged here, particularly when this is appropriate to professional development and to the service development of the department. Opportunities are available with applications for funding co-ordinated by the Trusts Medical Directors and clinical trial recruitment to International, National and Regional trials is active in all tumour sites. This trust was also rated as an area of Outstanding Practice by CQC (2016), and with the highest GMC score in the South West for training.
