An excellent opportunity for an experienced General Adult Psychiatrist to join a community mental health team, supportive of CESR. This NHS Foundation Trust is the leading provider of specialist mental health, learning disability, and drug & alcohol services for people of all ages. WHAT IS THE DEPARTMENT & TEAM LIKE?
The Team provides a service to their respective town and it’s surrounding villages. Total population served by the Team is 130,00 with the 19-64 population being 75,120. The current case load varies between 500 and 600 cases with a referral of approx. 5 cases per day. The consultant will work according to the principles highlighted in the New Ways of Working so all CPA cases will be carried by a care coordinator with consultant being consulted in case management.
The Team is consists of (1) Locality Manager, (2) Operational Team Managers, (1) Whole Time Equivalent Senior Practitioner (nusring), (6) Full Time equivalent CPN's, (2) Whole Time equivalent Occupational Therapist, (6) Full time STR workers, and (1) Non Medical Prescriber. Each Consultant may hold a small caseload of standard care patients who only require medical input prior to transfer to primary care (maximum number of 20).
The post holder will be expected to take an active leadership role in screening of referrals as well as assessment of new referrals jointly with other team members. The post holder will attend Trust meetings including Strategic Development Meetings, Clinical Governance Meetings within the Service Line and other meetings as requested.

The Mental Health Team provides a service to a city that has for towns, including their surrounding villages. The post holder will provide clinical leadership within the Community Mental Health Team alongside 3.5 Consultant colleagues. The CMHT staff have enhanced roles, the Team comprises of (4.5) WTE Consultant psychiatrists (including this post), (1) x band 8 Service Manager, (2) WTE Band 7 Operational Team Lead, (7.6) WTE Band 6 care coordinators and (1) WTE Band 6 OT, (1) WTE Band 5 nurse, (1) WTE Band 5 OT, (1.5) WTE Band 3 HCA, (1) WTE Band 3 STR, (4.5) psychological practitioners with skills in a range of modalities, (1) WTE Non Medical Prescriber & GP trainees on rotation.
It will be expected of the post holder to be actively involved in strengthening relationships with external agencies including the local CCG. The post holder will also provide medical advice and supervision to the Community Mental Health Team looking after patients who have severe and enduring mental illness with more complex needs who are managed at enhanced level of CPA and their care is managed and co- ordinated by the team.

There are currently no trainee doctors within the team – however this can be developed with the appropriate supervision qualifications.
The post holder will attend Trust meetings including Strategic Development Meetings, Clinical Governance Meetings within the Service Line and other meetings as requested. The post holder will build constructive relationships with the team manager and with other senior professionals.

WHAT IS THE DEPARTMENT & TEAM LIKE?
???The total population of the area is 100,000 with the 16-65 population of 61,000. There are two full time Consultants covering the Community Mental Health Team (this post being one of them). The Community Mental Health team comprises: (1) WTE Operational team managers, (1) WTE Service manager, (6) WTE CPNs, (6) WTE STR Workers, (1) WTE Non-medical prescriber, (4) WTE psychological therapists, and (5) WTE admin staff. There are currently no junior doctors within the team – however this can be developed with the appropriate supervision qualifications - there is a band (7) non medical prescriber.
The community mental health team accept referrals from varies agencies including General practitioners, IAPT services, secondary and tertiary agencies. The current caseload is approx. 650 with a referral rate of approximately 4-6 patients per day. Within the caseload there are currently 3 patients on a CTO and 6 patients who are on a conditionally discharged MoJ Section. Every patient has a lead healthcare professional and the post holder will provide advice/input as requested and attend the majority of CPA meetings. You will see between 15 and 20 patients a week.
The post holder will have access to dedicated admin with a ratio of 3:1, with consultants also having full access to a secretarial pool for all preparation of letters and reports etc.

WHAT IS THE GENERAL ADULT PSYCHIATRY - COMMUNITY DEPARTMENT & TEAM LIKE?
The Psychotherapy Department has been successful in rolling out the IAPT programme and providing brief interventions to the GP practices. This has made a significant reduction in the number of GP referrals to secondary care and has facilitated progress in the New Ways of working. The outpatient clinics are increasingly CTP reviews/ consultation. It Is anticipated that the further introduction of IAPT services will potentially reduce the total number of GP referrals coming to the team as this component of a 'stepped care' approach is implemented.
The introduction of the Mental Health Measure has provided for a Primary Care Mental Health service to be established. In Carmarthenshire there is a system of Link working with GP Surgeries. The link workers are all CMHT members and provide weekly sessions in GP surgeries. The Speciality doctors from the community teams may have sessional time to support the Acute care service during working hours and to broaden their experience of liaison and acute care.
The post holder will not hold an outpatient caseload however outpatient clinics are held In the 4 CMHT's. The post holder will be responsible for the assessment and management of adult community patients or inpatients and is expected to remain closely involved with the care of those admitted to the associated wards. All these clinical activities will be supervised by the Consultant.

WHAT IS THE STROKE MEDICINE DEPARTMENT & TEAM LIKE?
The Stroke Centre provides Stroke & TIA services for the NHS Trust. The geographical area served is wide and the population diverse. We currently treat around 1400 stroke patients per year. There are currently 6 stroke specialist consultants and one associate specialist within the Centre, supported by a team of junior doctors in training grades, Clinical Nurse Specialists, and a well-established multidisciplinary team of stroke specific allied health professionals.
Inpatient services are centred on the Acute Stroke Unit (with dedicated HASU beds). There are currently 59 inpatient beds within the Centre; there are plans to increase this total number contingent upon appropriate staffing across medical and nursing specialties. As an increasingly important regional provider of stroke services, The department is seeking to enhance their service quality and scope through the recruitment of dynamic, forward thinking, flexible and innovative consultant colleagues who share the team\'s commitment to providing the very best in care for the populations they serve.
The post holder will be responsible for the continuing care & treatment of patients in post holder\'s charge & personal performance of clinical duties as agreed in the post holder\'s job plan, allowing for all proper delegation to & training the post holder\'s staff. The post holder is expected to participate in the clinical governance arrangements of the Trust & specifically within the Division including clinical audit, & research.

WHAT IS THE GENERAL ADULT PSYCHIATRY TEAM LIKE?
The General Adult Psychiatry Service is consist of dynamic and forward-looking team members that provides acute inpatient psychiatry care, liaison service & Crisis Resolution Home Treatment. The Community Team "gate keep" admissions to the Acute inpatient unit and the consultant working with the CRHT also covers the general psychiatric admission ward which facilitates a coherent approach to service user needs when they are in relapse/crisis and require intensive support or admission.
The Department is a 10 bedded unit, where the team regularly work on the ward and ward staff had been seconded to the CRHT fostering a close working relationship. The Team provides a service consistent with models recommended in the National Service Framework.
MHA referrals during daytime are the responsibility of the community teams and CRHT attend where possible. The Specialty Doctors from the community teams may have sessional time to support the Acute Care service during work hours. The postholder will be responsible for the treatment and management of patients under care of Acute Inpatient team and support the development & delivery of a sub specialty service in hospital (and in the community where possible).

WHAT IS THE DEPARTMENT & TEAM LIKE?
The pre-existing team is commissioned by hospital and provides a co-located integrated liaison support for the Paediatric and Neonatal Units at the hospital. The team provide clinical input for in-patients and out-patients under the care of these departments who have comorbid or combined psychological and medical needs. The team are not commissioned for crisis input.
The Team anticipates about 600 referrals to the MHLS per site per year across two hospitals under the trust. Roughly 60% of these are in ED and need one assessment which is carried out initially by the local CAMHS teams and the OOH nurse team. Approximately 30 CYP a year present on a Section136 to A&E in both sites requiring psychiatric evaluation. Of the 600 referrals up to 60% will remain on the pediatric wards and will require Liaison Psychiatry.
The successful post-holder will have their own desk and office space within the Paediatric Consultant office area and immediate access to private rooms for confidential work. They will have a mobile phone, a desktop and also a laptop for remote working, with access to IT support. The post holder will have access to full time administrator support.

WHAT IS THE INPATIENT WARD & TEAM LIKE?
The ward is a mixed gender acute psychiatric ward with 18 beds. The ward provides assessment and treatment for patients who cannot be safely managed in the community by the Crisis Resolution & Home Treatment Teams. This includes a significant number of patients detained on Sections 2 and 3 of the Mental Health Act.
The Team is a close-knit group of 30 colleagues, mostly holding substantive posts, which meet regularly in formal and informal meetings. Whilst several CPD peer groups have formed within the trust, the consultants are also encouraged to seek peer groups outside the organization as an opportunity to network and share experience with colleagues from other services. A monthly Balint-Group for consultants is facilitated by the Consultant Medical Psychotherapist and gives opportunity to conduct in-depth case based discussions (CbD) required for annual appraisals and revalidation.
The post-holder is expected to be actively involved in all aspects of service development of the department and the wider organization. The postholder is expected to formulate treatment plans and oversee implementation and evaluation of these treatment plans and contribute to management of patient flow with a view to manage bed capacity effectively and avoid delayed transfers of care.

WHAT IS THE NHS TRUST LIKE?

WHAT IS THE DEPARTMENT & TEAM LIKE?
The department consist of a strong highly qualified staff which includes a professional lead, psychologists, psychotherapists of different backgrounds, clinical nurse specialists and nurse practitioners, dieticians, occupational therapists and support workers. It has a vibrant multidisciplinary community team, a day unit, a newly appointed FREED champion, is actively involved in the East of England New Care Models collaborative and is hoping to secure permanent funding for nursing posts to manage medical monitoring, an expansion to the existing team complement to manage the increase in demand and for an intensive community provision.
The Service receives 50 –60 referrals a month, with about 10-12 service users in Specialist Eating Disorder Units and 5-6 on mental health wards. The service also often have one or two service users in general medical wards either for medical stabilisation or while they are awaiting a SEDU. The active caseload varies from 50-60% of total referrals. Of these, about 55% have Anorexia or atypical anorexia, 24% Bulimia or atypical and 19% EDNOS-Eating Disorder Not Otherwise Specified or ARFID-Avoidant/Restrictive Food Intake Disorder and BED-Binge Eating Disorder.
The role of the post holder is complimentary to all these professionals with the intention of providing a comprehensive management of eating disorder. The post holder’s focus will be on clinical leadership, and medical management of the eating disorder with a strong respect for and understanding of the psychological drivers and maintainers of the eating disorder.

The Trust covers a mixed rural and urban area, including large and smaller towns. The area has excellent connections to London and the rest of the country. There are three major roads going North-South (the M1, the A1 and the A10). In addition the M25 passes through.
