INTRODUCTION TO THE POST
This is a 10 PA permanent (substantive) post to be based in medium sized district general hospital in the midlands of England. Working in a small, friendly team this is a good job for consultants looking for their first post in the UK in a well located, beautiful part of the UK.

WHAT IS THE HOSPITAL LIKE?
The hospital has approximately 400 bed beds, serving a population of approximately 270,000 across the county and surrounding areas (with a 900,000 catchment for vascular services) of which 24 are maternity, 16 are paediatric, ten are critical care and 28 are day-case beds. The hospital has over 2500 members of staff (making it the largest local employer in the area) and a current turnover of approximately £205m. The hospital enjoys strong community support with over 200 volunteers at the hospital and over £6 million being raised by it's independent Charity over the past 10 years. The support from the charity has enabled the hospital, for example, to build a dedicated cancer centre and has also recently funded a new MRI scanner, operational since early 2018.

WHAT IS THE PATHOLOGY DEPARTMENT & TEAM LIKE?

The department of cellular pathology is split across two sites and currently has seven full time consultants in post. The department would like to bring this up to 9 in total as the department expands its service. There is a team of biomedical scientists and laboratory assistants that support the service and each consultants is provided with secretarial support, office, an ergonomic microscope, and a designated computer with internet access.

The workload for 2019-20 for this particular site was:

• Histopathology - 18,924 cases
• Diagnostic cytology -1047 cases
• Andrology - 654 cases

Other important details regarding the department include:

• The surgical pathology work from the hospital (biopsies and resections) reflects the clinical speciality mix of the hospital whilst the case mix is typical of a district general hospital.
• The department provides a histopathology service for the NHS bowel cancer screening, breast screening and cervical screening service.
• A good repertoire of antibodies is available for immunohistochemistry at both sites, with less commonly used antibodies referred out to specialist centres for staining.
• There is a histology laboratory and “cut up” room. The laboratory contains standard equipment, which includes three tissue processors (a VIP and two Leica processors), two Embedding Centres (a Sakura and a Leica) and two Leica integrated auto-stainers and cover-slippers. The Roche Ventana Ultra staining platform is used for immunocytochemical stains
• The department has a multi-headed microscope which enables the Consultants to meet and discuss cases
• The Cytology laboratory includes a dedicated preparation room for the preparation of non-gynaecological specimens and contains one Cytospin, one centrifuge.
• The pathology laboratory is fully computerised with the CliniSys WinPath Enterprise system. The laboratory based system links directly with hospital and ward based systems, with both ICE requesting and reporting.
• The Hospital has many clinical links with the tertiary centre of Addenbrooke’s and Papworth Hospitals. It is part of the Anglia Cancer Network and the department participates in specialist MDTs within the Network. The perinatal and paediatric autopsy service is provided by Consultant Paediatric Pathologists at Addenbrooke’s Hospital.
• The Mortuary is also the Public Mortuary for the county. Coroner’s post-mortems are not part of the NHS contract and are performed at the behest of the Coroner.
• The Cellular Pathology Department has full accreditation against ISO15189.
• The Laboratory participates in NEQAS for:
  • General Histopathology (staining)
  • Immunocytochemistry
  • Andrology (sperm motility and morphology)

WHAT IS LIFE LIKE IN THE REGION?
The local town is a thriving market town, which provides easy access to London (55 miles), the East Midlands, the North of England and beyond through its rail links and M1 / A1 / A14 accessibility. First Capital Connect runs trains directly to London Luton and London Gatwick airport. Trains run to London St. Pancras, the international terminal for Eurostar.

The Hospital currently provides services for a population of around 270,000, of which 119,600 live in the main county town of. The area is primarily agricultural, with numerous pleasant villages within easy commuting distance of the hospital. In recent years most of the expansion in population has been in the M1 / A1 corridor and significant further growth is planned in the surrounding areas. It is anticipated that by 2021 there will be a further 50-55,000 people living in the town area.

There is a varied selection of accommodation for rent or purchase, both in rural and urban surroundings. Education is a prominent feature in the Town. The local schools provide first-class independent education for some 4,000 girls and boys supported by preparatory schools and other independent schools that cater for all ranges of ability. Cranfield University is located in the local area. The hospital is situated within walking distance of the town centre.

Several local villages to the north are situated on the large river which is a very attractive feature of the area. There are two good parks within walking distance of the town centre. Russell Park, with its children’s play area and formal gardens, takes its name from the Dukes. The main Park, laid out by the Victorians, has a circular carriage ride, pavilions, lodges and bandstand. The Country Park is within walking distance, offering water sports, nature trails, a visitor centre and marina. The river is an important feature and provides a focus for a very strong rowing community and River Festival held every two years, a large free outdoor event.

The area is well provided for sport, leisure and cultural activities including an International Athletic Stadium, five golf courses, three rowing clubs, a Rugby Club, a Town Football Club and several swimming pools. The world-famous local Orchestra has a residency nearby where it performs eight concerts each year, as well as involvement in educational activity in schools and the local community. John Bunyan was born in 1628 in Elstow, just south of the town.

Two biomedical scientists have completed the Diploma of Expert Practice in Histological dissection (cut-up), with ongoing training for another. Technical staff handle small biopsies, while Consultant Pathologists report all FNA and non-cervical cytology specimens. Although there is no sub-specialization in surgical histology or diagnostic cytology at present, lead pathologists for each cancer site act as contact points within the department, actively participating in multidisciplinary meetings. Importantly, there is no on-call commitment, and the post holder has the option to undertake coronial post-mortem examinations by direct agreement with local coroners services.

Currently, there are six consultant pathologists, with capacity for two more, including the current vacancy. The Histopathology and Blood Science services are fully accredited by UKAS, participating in both interpretative and technical EQA schemes. The hospital itself holds HTA approval as an institution.

WHAT IS THE PATHOLOGY DEPARTMENT LIKE? 

The department has full UKAS ISO 15189 accreditation. It\'s Histopathology and Cytopathology services are centralised into a custom-built laboratory in April 2003, which has accommodation for eight consultants plus specialist registrars. There is a well-equipped library and teaching suite within the department. Fully automated immunohistochemistry is provided with a comprehensive range of routine antibodies available at the Trust. The laboratories are served by the Telepath computer system with links across the Trust and NHS networks. PathoSys is used for reporting breast cores and resection specimens, colorectal cancer specimens, bowel cancer screening specimens, cervical punch and loop biopsies and bronchial biopsies and in line with recent advances in the field of histopathology, the department has introduced and have recently gone live with skin
reporting. The team consists of five consultant Histopathologists, two Specialty Doctors, plus up to two specialist registrars on rotation from a nearby infirmary who cover histopathology and cytology on a rota basis. The non-gynae cytology plus the surgical histology workload will be shared equally.

WHAT CAREER DEVELOPMENT AND OPPORTUNITIES ARE AVAILABLE? 

The Trust and Histopathology team are dedicated to supporting their doctors\' career development through devising a personal development plan, the post holder will be required to agree a programme of continuing professional development which meets his/her and service provision requirements. Regular MDT meetings are held for lung, colorectal, breast and upper GI. High definition videoconferencing and slide projection facilities are available in the MDT suites on the North Tyneside and Wansbeck sites and there will also be opportunity for the development of teaching experience. Undergraduate students from the affiliated University are regularly attached to clinical teams within the Trust and the new appointee would be expected to undertake teaching at an undergraduate level throughout the Trust, including the assessment of clinical medical students and examining from time to time.

They would also be expected to to participate in clinical audit activities (personal, departmental and inter-departmental) including clinical audit meetings. What is the department & team like?

The Histopathology Department is a highly specialized unit. The department is staffed with a dedicated team of five full-time equivalent (WTE) Consultant Histopathologists and one Specialty Doctor. They are supported by a team of Biomedical Scientists (BMS) at various levels and trainees/associate practitioners, with secretarial staff providing essential administrative support.

Annually, the department processes approximately 22,000 surgical requests at one of their sites alone, with around 17% originating from GP users, and similar volumes reported at their other hospital. The department handles a wide range of cases including Urology, Colorectal, Breast, Upper Gastrointestinal, Dermatology, and Lung biopsies. The department is notable for being a Breast and Bowel Cancer Screening Centre, dealing with Head and Neck, Endocrine, Medical Liver biopsies, and Gynaecology among others.

Facilities at the department include an advanced processing laboratory, immunohistochemistry services, and limited molecular genetics capabilities. The department is fully accredited with ISO 15189:2012, ensuring high standards in diagnostic accuracy and patient care. Operating Monday to Friday from 08:00 to 17:00, the department is committed to continuous improvement and the integration of new technologies such as Digital Pathology to enhance service delivery.

As part of a significant initiative within the NHS, pathology services in England are undergoing reorganization, transitioning into larger local network models. This restructuring has led to the establishment of 29 pathology units nationwide.

The Pathology Service was established following the Lord Carter review of Pathology services in England, aiming to reduce overtesting, enhance resilience, and lower costs. amalgamates staff, services, and IT platforms from four trusts to enhance pathology quality in the Black Country, serving 1.4 million individuals and conducting 60 million tests annually. It underwent a comprehensive IT system overhaul to facilitate integrated services, embrace new technologies, and expedite laboratory results.

The pathology service operates three laboratories offering nationally accredited consultant-led pathology services. With state-of-the-art facilities and IT systems, they garnered commendation from Lord Carter for its efficiency and innovation in pathology. Notably, innovations showcased during Lord Carter\'s visit in 2021 included pre-barcoded tubes for sample tracking and digital automation in microbiology labs.

Lord Carter praised the service as a model of service integration and technological advancement, emphasizing the importance of technology, including artificial intelligence, in expediting diagnostics. He commended the leadership and dedication of the staff, highlighting their achievements and commitment to patient care. The evidence is there to be seen, as latest figures show they scored 92 per cent compliance on a dashboard provided by NHS Improvement, ranking it one of the best performing services in the country

It is compact, with no boundary more than four miles from the centre and is approximately a 30-minute drive to Birmingham (15 miles), 1 hour 20 minutes to Manchester (60 miles). The city is an urban, multi-ethnic city with a population of around 250,000. The wider hospital catchment area has a population of in excess of 350,000. A Welsh Health Board are looking to appoint an experienced consultant histopathologist to work in a joint clinical and educational supervisor to four innovative CESR Fellows, all FRCPath Histopathology holders. 

This is a unique opportunity to work in a developing initiative that will bring immense social good to the local the people of Wales. 

Short / mid / long-term ambitions for this role.

Short-term, there is the opportunity to work a close group of four international doctors, with specialist qualifications in histopathology including FRCPath. You\'ll have the opportunity to shape the careers of hungry pathologists ready to absorb all the knowledge and experience from an experienced NHS colleague like yourself.

Mid-term you\'ll be jointly responsible for the successful portfolio submissions for these individuals, helping the laboratory at Glangwili General Hospital to establish a newly qualified consultant team to provide stability and excellent pathological services for the patients of Wales, UK.

Long-term, the potential is enormous. As part of ‘A Regional Collaborative for Health’ or ARCH a regional hub for South and West Wales is being built in Swansea, to house a 30-35 consultant lead service for histopathology alone. Fully digitalised regional services for half the Welsh landmass would be provided via one central and digitalised hub.
