WHAT IS THE THE OLD AGE PSYCHIATRY TEAM LIKE?
The Harbour is a new 154 bedded mental health hospital, with state of the art facilities, designed with extensive input from service users and clinicians and is based on good practice guidance and 'safety by design' to ensure that it can provide high quality care. The ethos of the unit is to provide therapeutic care which is empowering, person centered and needs led, and focuses on promoting recovery and independence.
The ward team is fully multidisciplinary and includes a variety of mental health professionals and the hospital has single ensuite bedrooms of varying specialisms over 10 wards, of which one is the older adult ward relating to this vacancy.
The advanced care wards accommodate older patients who are experiencing an acute mental illness that are also physically frail or who are also experiencing physical disability or illness.
The dementia wards accommodate patients diagnosed with dementia who are requiring the relational and physical care and security that an inpatient ward can provide.

An opportunity for a Consultant Old Age Psychiatrist to join an established community older adults team, as part of the Memory and Later Life Services. This is a replacement post due to the retirement of a full-time Consultant. CESR, Section 12 and AC training and development are supported in this NHS post.
Based in one of the trust main hospitals, this full time old age psychiatry job would be part of the community services which are organised into two teams. Each of these teams delivers: Memory clinic function (all ages); Hospital liaison; and CMHT functions for patients with dementia (all ages) and people who have functional disorders with age related needs and/or frailty issues.

WHAT IS THE THE OLD AGE PSYCHIATRY TEAM LIKE?
As a consultant in the Community Mental Health Team for Older Adult Psychiatry, you would be joining an enthusiastic supportive team of Consultant Old Age Psychiatrists covering the 3 clusters that provide specialist assessment and care for Older Adults with moderate to severe mental illness. The total team case load at present is around 700 (none on CTOs). Consultants do not regularly carry their own case load as each patient has a care co-ordinator in the CMHT.
You will work with a large number of partners and stakeholders both in mental health services, universal services and specialist mental health practitioners and health visitors.
For community clinic work adequate time, at least one hour, is allocated for the assessment of a new patient, or typically half an hour for a follow-up appointment. Current medical staff will tend to see 3 or 4 patients in a typical clinic (1PA) as a mixture of new and followup.
In the past few years the older adult CMHTs have started to work through ‘virtual clinics’. This model is to manage the increasing demand of new referrals with suspected dementia. These referrals are seen initially by a nurse who conducts an enhanced assessment including a cognitive evaluation. Neuroimaging is then typically carried out prior to the ‘virtual clinic’. In these clinics a psychiatrist will meet with the nurses and discuss the assessments carried out in order to try to reach a diagnosis and management plan, which is followed through by the nurse. This way of working has improved doctor time efficiency by 300% and allowed nearly 75% of new dementia referrals to be diagnosed this way.
The Trust is responsible for community and mental health services. Having specialists such as neurologists, neurophysiologists, speech and language therapists, physicians and many others within the same Trust has made for a much more collaborative way of working, breaking down barriers that sometimes exist when these services are within differing NHS Trusts.

WHAT IS NHS TRUST LIKE?

WHAT IS THE ONCOLOGY & DEPARTMENT & TEAM LIKE?
The Cancer Business Unit offers high quality clinical care within flexible and sustainable job plan structures. Recently the Trust has seen the opening of a graduate entry Medical School which has greatly enhanced the teaching and research opportunities for medical staff.
The Chemotherapy facility includes a state of the art Day Unit with 24 outpatient chemotherapy chairs, a 5 chair assessment bay for unwell patients, two 2 bedded siderooms, and 4 counselling/clinic rooms. Chemotherapy is prepared in a satellite pharmacy within the Unit. Treatment is provided by experienced chemotherapy nurses who are well versed in cannulation and administering chemotherapy. There are plans to move the unwell assessment area to the inpatient ward very shortly. They have 70 dedicated oncology beds across the trust and provision of side rooms for neutropenic patients and special requirements. The Oncology service is well supported by a well-established Haematology Service, Palliative Medicine Service including inpatient unit, Clinical Psychologists, interpreters and bereavement support staff.
Radiotherapy, Brachytherapy and Radiotherapy Physics are housed together in a purpose-built centre, with the following equipment:
- 4 Varian TrueBeam Linear Accelerators
- Aria Oncology management system and Eclipse treatment planning.
- HDR Brachytherapy Unit
- Xstrahl Superficial Xray therapy – 80kV & 120kV
- Toshiba wide-bore CT-simulator, with Prosoma Virtual simulation
The hospitals are located in the north east midlands with great access to the North Westerly cities, Birmingham, London and the rest of the UK with its excellent national transport connections. THE GENERAL MEDICINE CONSULTANT POST:
The post offers broad medical experience across the field of medical specialities. The holder of this post will usually work within one speciality team, however may be asked to cover in other areas across general medicine at times in order to maintain a safe effective service.
There is a firm commitment to continuing professional development and the opportunity to develop further in a chosen sub-specialty.
THE HOSPITAL & GENERAL MEDICINE DEPARTMENTS:
Within General Medicine, there are currently 19 Consultants, of which 13 are substantively employed with special interests in Respiratory Medicine, Cardiology, Gastroenterology, Diabetes and Endocrinology, Acute Medicine, Stroke, Rheumatology and Care of the Elderly.
An integrated service in general medicine and care of the elderly has matured within the Hospital. The team of consultants are enthusiastic and forward thinking. We pride ourselves in being a small, friendly hospital, with real drive to improve both patient care and the working environment for our staff.
In addition to an acute in-patient and out-patient medical service, specialist diabetic and non-invasive cardiology services are provided.
Acute medical work is focused on 195 beds in eight wards. ITU/HDU facilities are based on five beds. The Trust is recognised as providing efficient and well-organised Cancer services. This has been supported by both Regional, and National Accreditation processes. As part of the Avon, Somerset and Wiltshire Cancer Network, the Trust provides a range of cancer services. Those services already accredited include, Breast Care and Urology.

North Somerset, the surrounding county, has long coastlines with superb cliff tops and English countryside. You\'ll find yourself visiting the Mendip Hills, the Quantocks and Cheddar Gorge sooner or later. Joining an established and settled team of 8 respiratory consultants across both sites in the trust, the department has well managed services in lung cancer, sleep, bronchiectasis and COPD.
Whilst any subspecialty is welcome those with asthma, ILD or cancer interests would be especially desirable.
This hospital in the trust is concentrated on the Respiratory outpatient activity and there are established and proactive day hospitals administering omalizumab and IVIG, flush portacaths in addition to well established pathways for ambulatory care of PE and cellulitis.
The hospital has 182 acute medical beds and Monday - Friday all admissions are assessed by the dedicated acute physicians team which allows for a steady and controlled follow of mostly respiratory patients with some general medical patients to the respiratory team.
There is some on call with this position.
Successful applicants will be encouraged to contribute to the teaching provision at the neighboring hospital in the trust.
