The Ward is a 14 Bed, mixed gender assessment ward based in Worcester. The ward takes all admissions from the community for patients in acute crisis. The ethos of the ward is dynamic assessment, allowing staff to work on identifying, and safely managing patient’s needs. The Department is a team of kind, fair & determined doctors. They support people with mild, moderate or severe learning disabilities who are registered with a GP, to help them live a valued life in their community. The team is made up of learning disability nurses, occupational therapists, speech and language therapists and psychologists. They deliver a specialist assessment and intervention plan, which focuses on health inequalities, behaviour, risk, sensory, communication and psychological needs. They work in partnership with families and agencies to achieve positive outcomes for the patients in their home, community and other relevant settings.

The Team accepts referrals from service users, carers, social workers, GP\'s and other health professionals. All referrals will go through a screening process which checks whether you are eligible to access the service. In previous years, their colleagues have picked them as winners of the Principles of Care Awards, for their work with Down\'s Syndrome and dementia patients. The population is around 191,830 (ONS Data). At present, around 4144 people, within the borough, have a known learning disability, with the five year forecast showing this to increase to 4236 by mid-2025. Currently, the outpatient case load is around 150.

The post-holder will work under the supervision of a consultant psychiatrist and and Multi Disciplinary team, and will provide clinical support for the assessment and management of mental health problems on a routine and emergency basis. The post-holder will have the opportunity to rotate through different posts on a yearly basis according to service needs, offering opportunities for developing clinical expertise across a range of specialities. The Service, which is currently based within the Adult and LD Mental Health Division, is one of the highly performing mental health services at the NHS Trust, and has two wards, both with 15 beds. The service admits adults of working age from west of the county including 4 towns. Between November 2020 and October 2021, there were 299 admissions to all adults mental health units, that gives a rate of 101 admissions per 100,000 people in the town. Of the 599 patients admitted to all adult mental health units, 295 were informal and 302 were detained under the Mental Health Act.

This post does not include any community responsibilities. There are 3.5 WTE consultants in the Community Mental Health Service and 1.0 WTE in Home Treatment team (HTT). Community teams including the Home Treatment Team are located on site. Also on site can be found the local Section 136 suite, and another Ward, which provides inpatient services to older adults.

The post holder will deliver direct interventions including assessment, diagnosis and treatment of mental and physical health needs of the inpatients in the ward, be a responsible clinician for Inpatients detained under the Mental Health Act, undertake MHA assessments as the Trust Section 12 doctor for patients in their care, and regularly attend Inpatient ward multidisciplinary team meetings and ward handovers.

The CMHT psychiatrists provide medical input to the well-staffed Assertive Outreach Team (AOT). The Outreach service is expanding and recently merged with the Dual Diagnosis service. This Team has input from an experienced manger and a psychologist. The Dual Diagnosis service offers an intensive service to complex patients with significant substance use and associated risks and a designated lead who assesses, manages or offers a consultation for this patient group. The Dual Diagnosis lead is a Non-Medical Prescriber (NMP).

The community mental health services have expanded across the Trust with concomitant expansion in staff as we have built up a new service called Open Mental Health, in the last 2 years. This service works closely with primary care, the wider community and the voluntary sector. This service includes psychologists, assistant psychologists, liaison staff, eating disorder leads and support workers. 

The Consultant will deliver direct interventions including assessment, diagnosis and treatment of mental and physical health needs through outpatient clinics at another department and home visits as required, attend the weekly CMHT meeting, and work collaboratively with the inpatient consultant and team based at another Ward to ensure smooth transition for patients admitted and discharged from the ward.

The Service is divided into four Teams, and there is also a Central County Wide Memory Assessment service (which undertakes much of the MAS work) and also a County Wide Intensive Dementia Support Service IDSS (which acts as a Crisis/Home Treatment Team for those with Dementia). The community mental health teams and memory services provide psychiatric community care to patients over the age of 65. There is also a much smaller group of patients under 65 years of age, with primary dementias who are also cared for by the older adults’ community mental health team.

The multi disciplinary team includes medical, nursing, Psychology, OT with clinical support workers and admin support staff. The overall team provides CMHT, MAS and Specialised Day Hospital services. The two services area covers GP practices in four towns and equates to approximately 11,000 patients over the age of 65 (CCG figures December 2017). The community mental health teams are part of wider integrated teams. Each team is co located with staff from district nursing and the integrated rehabilitation team (community OT and Physiotherapists).

The post holder will hold regular clinics, as well as undertaking domiciliary visits when required. The post holder will provide assessment, diagnosis and treatment of mental health needs of community patients in their community area, and work closely with the community teams to provide effective multidisciplinary collaborative care: this will include availability for consultation, regularly attending team meetings and providing case discussion and advice at the request of team members and / or team managers.

In this sense this trust is a very modern part of the NHS, something the new Long Term Plan wants to see more of, with better integration and coordination of care, delivered as close to home as possible . WHAT DOES THE INPATIENT DEPARTMENT LOOK LIKE? 

The wards admit a mixture of Informal and detained patients. Detained patients account for 60% of patients on the wards. Delayed transfer of care patients account for 10% of the wards population. The wards admit adults with various psychiatric disorder and majority suffer with severe enduring mental illness. Patients with personality disorder tend to account for 40% of the wards population and they tend to have the below average length of stay. The post holder is expected to support patient flow team in their effort to facilitate discharges from the ward. The current length of stay (LOS) on our acute ward is 50 days but we are working reducing this to 25 days.
