A fantastic opportunity for specialist in geriatric or elderly care medicine to settle into their first consultant post and work towards their CESR application, if that is the intention.
The hospital will consider senior consultants with no UK experience who are not on the UK specialist register for geriatric medicine.
The Geriatrics department is a small and manageable size with a 28 bedded inpatient ward and various other outlier beds.
Currently no formal geriatric outpatient services are offered with the exception of a Parkinson's / Movement clinic. The department will be looking to develop these in conjunction with skill set of a successful applicant.
The Geriatrics department comprises 3 consultants in elderly care medicine supported by 15 specialty registrars, 9 core medical trainees and FY2's and 9 FY1's working full shifts for GIM on call.
Each consultant will be offered a budget and 30 days of study and professional leave over each 3 year period to pursue their career development activities. Furthermore there is an active Postgraduate Medical Centre which acts as a focus for contract between doctors at all levels an in all disciplines.
Consultants will be encouraged to actively participate in teaching and training of juniors throughout their tenure.

WHAT IS THE GERIATRIC DEPARTMENT & TEAM LIKE?
You will be working along with 4 consultant in the department along with specialists registrars, trust grade doctors, IMT’s and Foundation year students in the department.
The facilities in the department include a 24 bedded Frailty/Dementia along with a othro-geriatric unit admitting 15 new patients per week. The department also has an active Geriatric Emergency Medicine Service (GEMS) that run a 7/7 rota admitting 8-12 patients a day for a full MDT comprehensive assessment. Additionally, the team is responsible for a 26 - bedded community wards
The team in involved in 3 clinics - Fall clinics for Speciality MDT clinics seeing 4 patients per clinics, Hot Clinics (post GEMS review) according to the demand and Parkinson’s clinics, a 1 PA per clinics mixture of new and follow up’s.

What is the Trust look like?
The NHS trust situated in the North West England consists of 4 main hospitals providing services for acute hospitals and also local GPs. All our hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. The trust has won the Diverse Company Award in the National Diversity Awards (NDAs) in 2020 welcoming IMGs from different part of the world to join their team.
In July 2015 the trust was the first, and so far only, to get an increase in the NHS tariff for its services agreed by Monitor (NHS) because of its "increased costs associated with this trust running health services across multiple sites in rural locations" and this is expected to increase the trust's income by more than £20 million per year. The trust consists of Emergency Department, the Acute Medical Unit (AMU), the Acute Frailty Unit (AFU), Huggett Suite (Acute Stroke), Coronary Care Unit (CCU) and a Medical Same Day Emergency Care unit (SDEC). Currently, the trust sees approximately 350 Stroke admissions per year to one of the hospital
What does the geriatric department look like?
The department consists on 6 Substantive consultants and 2 Locum consultants in the senior medical team. The department also consists of junior teams (FY1 and 2, IMT and LED doctors) from general medicine along with speciality doctors in the department. The department is currently developing a multidisciplinary team that currently consists of a Consultant Geriatrician, Advanced Nurse Practitioner, a foundation year’s doctor / ST1 and REACT Community Assessor who provide a 7 day service assessing Frail & Older patients attending the emergency department. The Acute Frailty unit comprises of 15 acute beds that seems and manages around 12/15 patients per day.

The Stroke Centre is one of the highest performing centres in the UK achieving all the required targets and supports the requirements for CME as recommended by the Royal College of Physicians and is committed to providing time and financial support for these activities. The trust also provides an opportunity for an optional Tele-stroke on call when required with active, innovative and handles virtually 100% of secondary general medical referrals for its area with respect to all subspecialties.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The department aims to provide an effective 7 day service, within national and local standards. This would mean each consultant would be doing a 1:5 weekend shift in our Frailty Intervention Team service. The successful appointee will be expected to:

WHAT IS THE DEPARTMENT & TEAM LIKE?
The Geriatric Department is an established team of 8 consultants responsible for the provision of geriatric medicine services to the local area. The department has two 24-bedded COTE wards (Wards 1 & 2). Combined acute and rehabilitation stroke beds are available on a specialist Stroke Unit that offers all elements of stroke care, including 24/7 out-of- hours stroke thrombolysis and a newly commenced thrombectomy service. There are plans to commence a new Frailty Intervention Team based service at the front door incorporating a COTE consultant, therapists, nurse practitioner and a junior doctor.
The Geriatric Medicine department in Glan Clwyd Hospital participates in a robust educational programme for Cardiff University medical students, FY, IMT and STR training. Specialist Registrars in Geriatric Medicine for Wales Deanery have a training rotation based in North Wales rotating between the 3 hospitals under BCUHB. The department is pro-active with participation in National Audits, QIP and other projects and publications.
The post holder will provide consultant input to a COTE ward (Ward 1) along with other consultant colleagues based in that ward. The post holder will also attend the ward’s Board Rounds to discuss patient’s management and discharge plans along with the junior doctors and multi-disciplinary team

Catchment area
The hospital is a modern district general hospital in North East Wales serving a population of approximately 300,000. It is one of the largest towns in North Wales, with a thriving and diverse industrial centre and also being the market town for a large rural area.
The countryside surrounding is exceedingly attractive with a National Park less than an hour away. Additionally, major cities are a short commute away.
The Health Board
The Health Board comprises three acute District General Hospitals, mental health services and community hospital provision, and covers all health care providers Wales. There is a consultant body of over 150. All the major specialties and subspecialties are represented with the exception of cardiac surgery and neurosurgery. Visiting consultants provide for thoracic surgery (once a fortnight), plastic surgery, neurology, and super specialist paediatrics. Oncology services are provided by visiting consultants from a specialist Cancer Centre.
The Care of the Elderly Service
We are excited to offer this new permanent full time (1.0 WTE) consultant post within the Directorate of Medicine to improve and enhance Frailty services and the Care of the Elderly department is currently going through an exciting expansion with new services being offered. We are looking for an individual who will help shape the future of the department and can drive innovative ways of working.
The post will be for 10 sessions of which there will be 7 DCCs and 3 SPAs depending on additional duties undertaken, and they are committed to devising an individualised job plan for the successful candidate. Areas that can be incorporated into the final job plan include:
- Frailty
- Community working including pro-active care home medicine
- Falls
- Stroke
- Orthogeriatrics
The Care of the Elderly department provides the following services:
- Frailty
- Acute stroke unit with TIA clinics and a 24/7 thrombolysis service
- Rehabilitation in community hospitals
- Parkinson’s disease and movement disorder clinic
- Multi-disciplinary falls service
- Orthogeriatrics
Current Staff
The current team comprises of 6 consultants with specialist interests in orthogeriatrics, movement disorders, stroke, frailty and falls. Junior staff is comprised of 1 Specialty Doctor (attached to the Stroke Unit), 4 Specialist Registrars, 9 SHO grades (a mix of F2s, CMT, and GPVTS trainees) with the possibility of a third strand F1 during some rotations.
These staff rotate around the department and will make up the junior doctor team supporting the new appointees – at a minimum there will be one SHO attached to each rehabilitation ward, a Staff grade based in the Community Hospital, and one SHO (often more) based on the frailty unit. There is a middle grade to cover the frailty unit and another that provides cover for the rehabilitation unit.
Mentorship
New consultants will be encouraged to take a mentor from colleagues and to attend the local RCP North new consultant events and courses.
Annual and Study Leave
Annual leave is 33 days per year. A maximum of thirty days study leave, over a rolling 3-year period funded study leave is available in line with national terms and conditions. Leave to attend courses will normally be granted up to this limit unless service demands are exceptional.
