This Cancer Centre provides a high degree of specialisation for all the major cancers. It has one dedicated ward of 30 beds (with scope to increase to 36 beds at periods of high demand) for both oncology and haematology patients, together with a chemotherapy and haematology day unit, and an aseptic unit on site. There are also two laminar flow rooms for patients requiring bone marrow/stem cell transplantation and one dedicated room for radioactive isotope therapy. There is a state of the art telemedicine facility allowing regular multi-professional meetings between oncologists, physicians, surgeons and AHPs in Wales. The system has the capacity to transmit and view both high quality radiological images and histopathological slides. The department is also developing a SABR programme for lung cancer.
The clinical team are supported by highly-experienced support teams of nurses, radiographers, research officers and admin staff. The new consultant will be joining a friendly and forward thinking consultant team of 14 clinical and medical oncologists who are committed to delivering highest standards of cancer care. There are also 9 Specialty Registrars in Oncology (6 Clinical and 3 Medical).

This training takes place at all levels from undergraduate lectures and clinical teaching, through to Foundation and Core Medical trainee clinical and educational supervision, and higher specialist training. The team is a set of innovative & experienced consultants that sees all young people, up to their 18th birthday. The team delivers a full range of services to children and young people including primary mental health input. Additional support come from the Early intervention in Psychosis team which provides intervention for psychosis (covering people between the ages of 14 – 35 with emerging or apparent psychosis).
The Crisis Service, which provides for the assessment of emergencies during normal working hours and at the weekend during the day and by the HOPE team for more intensive outpatient intervention (see below for more details); Extended HOPE provides consultation and assessment to young people in crisis, out of normal working hours. Further, the eating disorders team and the Specialist CAMHS team for adopted and looked after children (established in early 2010) provide input across the county. Access CAMHS provides a single point of access for both CAMHS community team and Primary Mental health team referrals.
The post holder is expected to provide a diagnostic, assessment and treatment service for children and adolescents with a mental health problem. The General Adult Psychiatry department is a team of innovative & hardworking doctors. The Site Hospital is the new purpose built 60 bedded working age adult in-patient Hospital. The Working Age Adult psychiatric services are organised into a community service comprising a Community Mental Health Recovery Services. The Acute Service component comprises a Home Treatment team covering the boroughs. The home treatment team ‘gate keeps’ admissions to the inpatient units.
In patient services aims to provide timely assessments and promote recovery of people with a wide range of mental health needs. The goal is one of therapeutic engagement within a recovery framework. There is a multidisciplinary team who develop individual care plans working with each service user. The inpatient wards interfaces with home treatment teams, psychiatric liaison services, Section 136 places of safety, community mental health recovery services, and the wider criminal justice system.
It is expected that the post holder will provide Consultant Psychiatrist input and Responsible clinician cover for a 18-bedded ward mainly serving the population of aligned to the wards alongside another Consultant. The hospital provides a full range of general Obstetric & Gynaecology services, primarily on the main hospital site, as well as outpatient services in satellite community hospitals at several areas. The delivery rate has been stable around 2200 per annum over recent years, but the complexity of cases has increased significantly over the last year with the full transition of specialised neonatal intensive care services in North Wales centralised at the hospital's area. This provides cover for over 7000 women a year.
Currently, there are 9 substantive Consultant Obstetricians & Gynaecologists, all provide general obstetrics & Gynaecology services. Three new consultants participate in the out of hours second on call rota. There are six specialty doctors / Senior Clinical Fellows who complement the second on call daytime & out of hours rota. There are eight junior doctors who participate in the first on call out of hours rota. The service provides a local Gynaecology Urgent Suspected Cancer (USC) referral service, and manage some Gynaecology cancer cases locally, networked to the North Wales Gynaecology-oncology surgical centre based at Bangor. Fertility & Urogynaecology services are networked across North Wales.
The post holder will participate in the second on call rota, including out of hours on a 1 in 8 full shift with prospective cover for annual, study & statutory days leave. The post holder will be expected to attend & actively contribute to educational & governance activities within the department.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The Mental Health Team provides a service to a city that has for towns, including their surrounding villages. The post holder will provide clinical leadership within the Community Mental Health Team alongside 3.5 Consultant colleagues. The Service is designed in keeping with Locality Leadership groups which liaise closely with the Local clinical commissioning groups. Total population served by the team is around 215,000 with the 19-64 registered population being approximately 133,000.
The CMHT staff have enhanced roles, the Team comprises of (4.5) WTE Consultant psychiatrists (including this post), (1) x band 8 Service Manager, (2) WTE Band 7 Operational Team Lead, (7.6) WTE Band 6 care coordinators and (1) WTE Band 6 OT, (1) WTE Band 5 nurse, (1) WTE Band 5 OT, (1.5) WTE Band 3 HCA, (1) WTE Band 3 STR, (4.5) psychological practitioners with skills in a range of modalities, (1) WTE Non Medical Prescriber & GP trainees on rotation.
It will be expected of the post holder to be actively involved in strengthening relationships with external agencies including the local CCG. The post holder will also provide medical advice and supervision to the Community Mental Health Team looking after patients who have severe and enduring mental illness with more complex needs who are managed at enhanced level of CPA and their care is managed and co- ordinated by the team.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The Mental Health Team provides a service to two towns and their surrounding villages. Total population served by the Team is 106,800 with the 19-64 population being 64,440. The current case load varies between 600-800 cases for the whole of the team with a referral rate varying between 3-7 cases per day. Each Consultant may hold a small caseload of standard care patients who only require medical input prior to transfer to primary care (maximum number of 20).
The CMHT staff have enhanced roles, the Team comprises of (1) locality manager and (1) interim locality manager (Senior practitioner OT), (1) whole time equivalent senior practitioner (Nursing), (5) full time equivalent CPN’s and (1) part time equivalent CPN, (4) whole time equivalent occupational therapist, (8) full time STR workers, and (1) SAS doctor. There are currently no trainee doctors within the team – however this can be developed with the appropriate supervision qualifications.
The post holder will attend Trust meetings including Strategic Development Meetings, Clinical Governance Meetings within the Service Line and other meetings as requested. The post holder will build constructive relationships with the team manager and with other senior professionals.

WHAT IS THE GENERAL ADULT PSYCHIATRY - COMMUNITY DEPARTMENT & TEAM LIKE?
The Psychotherapy Department has been successful in rolling out the IAPT programme and providing brief interventions to the GP practices. This has made a significant reduction in the number of GP referrals to secondary care and has facilitated progress in the New Ways of working. The outpatient clinics are increasingly CTP reviews/ consultation. It Is anticipated that the further introduction of IAPT services will potentially reduce the total number of GP referrals coming to the team as this component of a 'stepped care' approach is implemented.
The introduction of the Mental Health Measure has provided for a Primary Care Mental Health service to be established. In Carmarthenshire there is a system of Link working with GP Surgeries. The link workers are all CMHT members and provide weekly sessions in GP surgeries. The Speciality doctors from the community teams may have sessional time to support the Acute care service during working hours and to broaden their experience of liaison and acute care.
The post holder will not hold an outpatient caseload however outpatient clinics are held In the 4 CMHT's. The post holder will be responsible for the assessment and management of adult community patients or inpatients and is expected to remain closely involved with the care of those admitted to the associated wards. All these clinical activities will be supervised by the Consultant.

The Department of Obstetrics and Gynaecology consists of: -12 substantive Consultant Obstetricians/Gynaecologists and 1 WTE locum consultant. 5 new consultants participate in the out of hours middle grade rota. There are 4 specialty doctors, 1 trainee and 3 senior clinical fellows who complement the middle grade day time and out of hours rota. This post is to complete the full complement of middle grades in order to work a 1 in 9 on call rota.
The birth rate at the Hospital maternity unit has remained steady around 2700 per annum. The Labour Ward, maternity wards, Special Care Baby Unit, Obstetric theatre, Intensive Care, Gynaecology ward and the main operating theatres are located on the first floor of the hospital. There is a dedicated Obstetric theatre within the labour ward complex. There is a dedicated modern ultrasound machine, housed in the antenatal clinic area used for fetal scanning including interventional procedures like amniocentesis and chorionic villous sampling.
The post holder will involve routine clinical and on call commitments with opportunity to complete ATSM in certain areas, as well as research opportunities.
