WHAT IS THE DEPARTMENT & TEAM LIKE?
The Department is sited at the state-of-the-art Pathology and Pharmacy building at the their main hospital. Satellite laboratories are based at another site and their last hospital covers frozen sections for cancer surgery and to support research activities. Cellular pathology services are provided along specialist lines with lead consultants and supporting consultants in each specialist area.
The department provides a range of services across East London and Essex. A wide range of general and specialist histological material is seen including cardiothoracic, dermatopathology, gynaecological, breast, medical renal, lymphoreticular, genitourinary, paediatric and perinatal, endocrine, gastrointestinal, hepatobiliary, oral and head and neck. The department provides histology reporting for four busy Colposcopy Units. Monthly colposcopy MDMs occur for each of these Units, with pathology support whether on-site or via video-microscopy.
The post holders will be expected to support the relevant specialty lead(s) and the Clinical Lead in the management of the cellular pathology service. The configuration of all the responsibilities will be reviewed from time to time and the post holders will be expected to work with consultant colleagues to provide a reliable and expeditious service, with scope for service development.

The department is well-equipped and has a dedicated MDM meeting rooms with video conferencing facilities. The MDMs are divided fairly amongst the consultants with only one pathologist attending each meeting and limited sub-specialisation. The department's laboratory is UKAS accredited, with state of the art equipment, dealing with over 29000 surgical cases and 2000 non-gynae cytology specimens per year.
The department has implemented the Warwick workload management system and this is used to allocate work to consultants. An established double reporting system for cancer cases is in place to enhance quality assurance. The case mix is that normally seen in a district general hospital, with consultants taking an integral role in cancer MDTs.
The post holder will play a full part in the provision of a diagnostic pathology service to the Trust and will be expected to contribute to the provision of a cancer pathology service for one or more anatomical sites, complementing the interests of the consultants already in post.

The department currently have three full-time urological surgeons, a Locum Consultant along with 5 middle grade registrar level training including 1 SpR from the Mersey Deanery training programme. There are two SHO level staff and one FY1 and FY2. The Department has recently undergone a refurbishment, to provide day-case urology and therapeutic services to include TRUS-Guided prostate biopsy lists, an endoscopy suite, dedicated lithotripsy sessions and specialist out-patient clinics. The department has inpatient beds based on the 3rd floor within the Surgical Department. Paediatric beds are also available on the main hospital site.
The department uses one of the two dedicated purpose built modern integrated laparoscopic surgery theatres for its heavy laparoscopic workload. Currently, major laparoscopic Nephrectomies and prostatectomies are performed in this OR. A 12 bedded Intensive Care/High Dependency Unit is located on the first floor of the hospital. There are excellent pathology facilities, including a Public Health Laboratory. The Radiology Department offers a full range of facilities, including CT, MRI and fully functional intervention radiology service.
The post holder will provide elective and emergency inpatient services, along with other members of the department. The post holder will also participate actively in the Department’s Clinical Audit Programme and Continuing Professional Development, which is fully supported by the Health Board.

The department has an annual attendance of approximately 58,000 patients, and sees the full range of presentations from major trauma, to complex medical presentations, critical illness, and paediatric emergencies. The ED has excellent support from and close working relationships with the inpatient medical, paediatric, critical care, radiology, and surgical specialty teams.
The department has a Reception and Triage area - operating on a 24-hour basis, with specially trained reception and nursing staff in attendance, a clinical area for adults & children and ED observation ward. The department runs weekly safety and governance meetings and has a rolling audit programme. Safety and good clinical governance are given extremely high priority in the ED and specialty doctors will be expected to contribute fully to safety initiatives, quality improvement and governance work and to participation in audit cycles within the ED.
The department has an active teaching programme and specialty doctors are encouraged to contribute to this. The ED is keen to support specialty doctors in developing additional areas of special interest within Emergency Medicine, for example such as in sedation, paediatrics, research, education, and interest in IT.

It is a two GP partnered practice based in rural coastal area of South Meirionnydd, Wales with a practice population of 5,500 patients. The team is a forward thinking practice, with a nurse-led triage, practice Pharmacist and Physiotherapist, working with a motivated support team, who provide document management to reduce the GP paperwork.
Education is high on their agenda as a Training practice for medical students and GP trainees. They are keen to provide education/clinical sessions to develop and support the post holder's specialist interests, within their working week. As a family friendly practice they will consider home working sessions via attend anywhere, with support from Consultant Connect.
The post holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients. In accordance with the Practice timetable, as agreed, the post-holder will make him/herself available to undertake a variety of duties including surgery consultations, telephone consultations and queries, triage calls, triage visits, checking and signing of repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.

The hospital provides a full range of general Obstetric & Gynaecology services, primarily on the main hospital site, as well as outpatient services in satellite community hospitals at several areas. The delivery rate has been stable around 2200 per annum over recent years, but the complexity of cases has increased significantly over the last year with the full transition of specialised neonatal intensive care services in North Wales centralised at the hospital's area. This provides cover for over 7000 women a year.
Currently, there are 9 substantive Consultant Obstetricians & Gynaecologists, all provide general obstetrics & Gynaecology services. Three new consultants participate in the out of hours second on call rota. There are six specialty doctors / Senior Clinical Fellows who complement the second on call daytime & out of hours rota. There are eight junior doctors who participate in the first on call out of hours rota. The service provides a local Gynaecology Urgent Suspected Cancer (USC) referral service, and manage some Gynaecology cancer cases locally, networked to the North Wales Gynaecology-oncology surgical centre based at Bangor. Fertility & Urogynaecology services are networked across North Wales.
The post holder will participate in the second on call rota, including out of hours on a 1 in 8 full shift with prospective cover for annual, study & statutory days leave. The post holder will be expected to attend & actively contribute to educational & governance activities within the department.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The Department serves a resident population of approximately 250,000 which is greatly increased at weekends and in holiday periods due to the half million visitor days/year spent in their catchment area, resulting in a high seasonal and weekday/weekend variation in ED attendances. Emergency Department sees a wide and interesting case mix of emergencies in all specialities, with very few “inappropriate attenders” compared to many urban UK EDs.
The department has a five-bedded Emergency Department Observation Unit within ED and there is a dedicated Minor Injury area that runs until midnight where most of our Minor Injury workload is handled by our team of ENPs. The local GP Out-of-Hours service has been co-located adjacent to the ED since 2006. Very close working relationships are enjoyed with colleagues in OOH primary care, with movement of patients from one stream to the other in both directions.
The Emergency Department is currently staffed by: 7 Consultants (including this post), 2 Associate Specialists (on the senior tier), typically 8-12 Clinical Fellows (post-ACCS/ST4-equivalent doctors who do 80% EM and 20% either PHEM or Medical Education), 1 ST4-6 (Wales Deanery), 1 ST3 (Wales Deanery), 8 SHO-tier doctors (3 x FY2, 1 x ACCS, 4 x GPST and a “Health Board SHO”), Typically 3-5 FY3 posts & 1 Academic F2 doctor.
