Each consultant will be offered a budget and 30 days of study and professional leave over each 3 year period to pursue their career development activities. Furthermore there is an active Postgraduate Medical Centre which acts as a focus for contract between doctors at all levels an in all disciplines.

The Acute Geriatric services are provided on all 3 main Trust sites. The department provides a multidisciplinary, multi-agency falls prevention service across acute sites. The service has been operating since 2002 and has grown considerably. The Geriatricians are responsible for providing emergency care of the older person from ED/ emergency floor through to discharge. The wards are supported by daily board rounds led by a Consultant Geriatrician and strong multidisciplinary team.
The sites operate an age-related take at admission, but are implementing frailty identification early in the emergency pathway to enable prompt CGA initiation, working closely with an established Integrated Discharge Team and their community partners. The frail patients with acute medical illnesses are admitted directly to a Frailty ward from A&E. The service provides a comprehensive osteoporosis service, and has a Dementia Team, comprising of a Matron and 2 Band 6 nurses supported by Clinicians.
The post holder is expected to communicate effectively with all other medical and service departments within the hospital, and with GPs and other hospitals, to ensure continuous care for each patient. The post holder is expected to undertake non-emergency work during premium time as required. The post holder will lead on Parkinson disease and movement disorders.

The integrated department of medicine provides inpatient, outpatient and occasional domiciliary consultation services for medical patients of all ages. In-patient and rehabilitation facilities are provided at the main hospital site. There are many acute beds provided for the directorate including a dedicated purpose built 6-bedded coronary care unit and 24 bedded Clinical Decisions Unit. There is also a combined HDU/ITU with a total of 14 beds.
The department is supported by full pathology and radiology services. In addition, the department of physiological medicine carries out electrocardiography, echocardiography, stress testing, Holter monitoring and lung function testing. The biochemistry department works closely with the diabetes department. The appointee will be joining a group of 6 other consultants in elderly medicine who provide a comprehensive service for residents of Northeast Essex. The department consists of 3 main clinical delivery groups - General Community – comprising Older Peoples inpatient and outpatient services, Specialist Community Services, and Integrated Therapies. The department operates a ward-based policy and have an average of 10-15 outliers on the 3 wards where the inpatient care are shared between the consultant in the department. There are rapid assessment clinics both at 2 sites offering GPs the opportunity for patients who require urgent assessment to be seen within 72 hours of referral. The department are also responsible for rapid access clinics, sub-specialty care in frailty, Orthogeriatrics, falls, and movement disorders.The older people’s physicians enjoy a close working relationship with the general physicians and there are good links with the old age psychiatry departments based at both the sites.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The department holds a weekly staff educational meeting, a Consultant-led Radiology meeting, and regular departmental consultant meetings, with two monthly clinical governance sessions. Consultants do not work in isolation, and are partnered in pairs in each base ward area at any one time, allowing opportunity for collaborative working and mutual support. The department takes part in regular presentations at the Grand Round and has a well-established postgraduate and undergraduate medical training programme.
The department provides undergraduate teaching for third/fourth/fifth year medical students and contributes to medical school examinations. The specialty currently manage patients across a number of inpatient services. These include the review of acute admissions in the Emergency Zone, a newly created Acute Frailty Unit with 32 beds, with post-admission short stay capacity. There are two complex care wards that has 32 beds on each ward, with a further short stay bed base (c.16 beds). Transitional post- acute care beds are located in a nearby ward, which includes 70 beds.
The post holder will be responsible for the clinical management of patients under their care, and, in liaison with consultant colleagues, continuity of care when the post holder is absent.
