The TB caseload is 100 notifications per year. The TB team is well staffed by 4 WTE nurses & 0.6 WTE case worker to provide latent TB screening, screening for immunosuppressive therapies, direct-observed therapy, community outreach as well as inpatient support.

Two other consultants have inpatient responsibilities as acute physicians on the acute medical team. Some of the chest physicians are may take up sessions with the acute team.
The Respiratory Medicine Department outpatient services include Asthma, ILD, Lung Function, Outreach Clinics, Outpatient IV Antibiotic Service & Pulmonary Rehabilitation. The department has also has sleep service, supported by a new 15 bedded respiratory high care and NIV unit which opened a year ago. This unit is situated adjacent to the intensive care department and provides care for patients needing acute NIV, acute CPAP, domiciliary NIV, and high flow oxygen.
The post holder is expected to deliver a consultant-led respiratory service at Wrexham Maelor Hospital for the diagnosis and management of both outpatients and inpatients. The post holder is expected to work within a multidisciplinary team of healthcare professionals and managers to further develop local respiratory services, with focus on thoracic oncology and interventional pulmonology, or domiciliary NIV and sleep medicine.

What is the trust like?
The Trust was formed on 1 October 2019 with the merger of two large tertiary trusts partnering with wider areas of the North West of England to become one of the first 14 areas nationally to be confirmed as an Integrated Care System. The trust services to a half a million people. We’re responsible for delivering over 70 services across 15 main locations and we employ over 6500 members of staff.
What is the department like?
The department consists of 5 consultants along with 3 full time nurse specialists, 2 part time sleep nurse specialists, 1 registrar, 2 trainees and 4 FY doctors in the department. The ward is a 23-bedded unit typically ranging 5-8 patients per days. The hospitals is a modern 27 bedded unit for both Respiratory and general medical patients usually shared by 2-3 consultants in the department.
The service has had a significant modernisation in the last few years expanding junior doctors into subspecialties like Lung Cancer focussing more on EBUS with lymph node aspirate and biopsy, a dedicated plural procedures room, improved thoracoscopy business cases, 10 brand new non-invasive ventilation machines, Participation in the regional asthma network / MDT, Participation in the regional Interstitial Lung Disease Network.
CESR
International candidates are welcome and CESR will be supported. Direct input will be provided on a regular basis from the Clinical Director, with opportunity for shadowing, lists and portfolio building built into the job plan.

The department is part of the four divisions the NHS Trust has organised. The respiratory department is a fully integrated department: they provide a comprehensive range of inpatient and outpatient specialist services from their two acute sites. They also have a multi-disciplinary integrated respiratory team (RESPS) providing services across both acute sites, in the community and in the patients homes. The inpatient respiratory care is based on their main ward, which is a 28 bedded medical ward with a high turnover of acute respiratory and medical patients.
There are two consultants covering the ward in patients and referrals at any one time. The department meets daily with their multidisciplinary team attended by doctors in training, nursing staff, social service and physiotherapy teams to ensure efficient, safe, consultant led best practice and patient centred care. They incorporate medical education, audit and quality improvement into their ward rounds with their doctors in training to inspire and encourage learning. The appointee will be expected to help develop and enhance the general and respiratory medicine services for both inpatients and out-patients. The department is committed to developing an integrated and equitable medical service for patient care across the Trust\'s hospitals. This strategy will provide significant career development in a wide range of medical specialties and align to service delivery models across Grampian with increased opportunities for research, education and clinical management for those with an interest in these areas.

They were established as an NHS Trust in April 1991, and became a Foundation Trust in April 2008. WHAT IS THE DEPARTMENT & TEAM LIKE?
The Respiratory Medicine Team is a fully integrated team that works closely with all members of the multidisciplinary team that is involved with the care and support of patients with Respiratory illness. They are involved in both the acute management of patients admitted to Hospital as well as the chronic disease management of respiratory patients. They liaise closely with both primary and secondary care.
The Department has 5 services:
The Service covers all respiratory patients in the acute trust and community. The Team offers support to patients at home following acute exacerbation of their chronic respiratory condition that has required admission in to hospital. They provide support and information for lung cancer patients and palliative care for patients who have non-malignant disease. The team also provides education for all staff.

You will join an existing team of 30 NHS and academic consultants providing general and specialist Respiratory services to the people of the city and region.

DEPARTMENT OVERVIEW:
This is an exciting opportunity to join the respiratory team, one of the largest respiratory departments in the country with a breadth of subspecialty interests; ranging from Cystic Fibrosis, ILD including LAM, through asthma, COPD, lung cancer to infections including TB and a regional fungal service.
This post will help the department develop both the COPD and asthma services, working with colleagues to deliver comprehensive integrated care. As a member of a large and dynamic department there are significant opportunities for professional development, including research. Both the asthma and COPD services benefit from academic leads with internationally recognised research profiles.
The successful applicant will have weekly clinics focusing on both COPD and asthma, and will join both the alternate week Asthma MDT and the regional lung volume reduction surgery (LVRS) MDT, held 6 times a year. There will also be the opportunity for a monthly bronchoscopy list.
REQUIREMENTS FOR THE ROLE:
This is a fantastic opportunity for an ambitious consultant in respiratory medicine to work in a state of the art hospital, set within a wonderful part of the country.
UK based consultants with CCT or experienced overseas histopathologists with completed or part-completed CESR can be considered for these fantastic opportunities working with one of the UK\'s fastest growing NHS pathology services. Those with CCT will have opportunities in leadership should this be desired whilst those pursuing CESR can be supported through the process whilst developing their areas of special interest in line with service needs.
All positions are offered on a 10PA, forty hours per week contract, and consultants with special interest in gynecology, upper and lower GI, dermatopathology, urology, breast and head & neck are of particular interest. Please note however - experienced consultants with different special interests can be accommodated. You will be expected to report on a range of specimens but to develop in your three areas of particular interest. Again, there is also scope for much teaching experience.
WHAT IS THE PATHOLOGY DEPARTMENT & TEAM LIKE?
Comprised of a team of 12 current histopathology consultants, this is a moment of huge growth for the Trust as they look to incorporate another laboratory. The team contains consultants with diverse experience & training from the UK, Spain, France, South Africa, India and the Middle East. Management and clinical staff work hard to engender an empowering and supportive work environment. Using a state of the art time management and reporting system with state of the art equipment across three laboratories - the services are part digitalised. Full digitalisation is expected in within 12-18 months. The laboratory is approved by the IBMS as a training laboratory for pre-registration and specialist training in Cellular Pathology. The department has full UKAS/ISO accreditation. The Department is HTA compliant and HFEA accredited and takes part in benchmarking exercises.

The department:
The department offers modern and well-resourced inpatient facilities on dedicated wards. This includes NIV, High Flow Nasal Oxygen, Step down ventilatory care and pleural ultrasound.
Outpatient services include bronchoscopy lists across all sites with EBUS provision. They offer comprehensive pulmonary function facilitates across the trust including sleep services and bronchial provocation testing. The lung function labs have recently been upgraded to state of the art equipment including a brand new cardiopulmonary exercise machine with excellent exercise physiology research links at Lancaster University.
Specialist clinics are currently offered for lung cancer, interstitial lung disease, asthma, sleep/ventilation and TB.
