An opportunity for a Consultant Old Age Psychiatrist to join an established community older adults team, as part of the Memory and Later Life Services. This is a replacement post due to the retirement of a full-time Consultant. CESR, Section 12 and AC training and development are supported in this NHS post.
Based in one of the trust main hospitals, this full time old age psychiatry job would be part of the community services which are organised into two teams. Each of these teams delivers: Memory clinic function (all ages); Hospital liaison; and CMHT functions for patients with dementia (all ages) and people who have functional disorders with age related needs and/or frailty issues.

WHAT IS THE THE OLD AGE PSYCHIATRY TEAM LIKE?
As a consultant in the Community Mental Health Team for Older Adult Psychiatry, you would be joining an enthusiastic supportive team of Consultant Old Age Psychiatrists covering the 3 clusters that provide specialist assessment and care for Older Adults with moderate to severe mental illness. The total team case load at present is around 700 (none on CTOs). Consultants do not regularly carry their own case load as each patient has a care co-ordinator in the CMHT.
You will work with a large number of partners and stakeholders both in mental health services, universal services and specialist mental health practitioners and health visitors.
For community clinic work adequate time, at least one hour, is allocated for the assessment of a new patient, or typically half an hour for a follow-up appointment. Current medical staff will tend to see 3 or 4 patients in a typical clinic (1PA) as a mixture of new and followup.
In the past few years the older adult CMHTs have started to work through ‘virtual clinics’. This model is to manage the increasing demand of new referrals with suspected dementia. These referrals are seen initially by a nurse who conducts an enhanced assessment including a cognitive evaluation. Neuroimaging is then typically carried out prior to the ‘virtual clinic’. In these clinics a psychiatrist will meet with the nurses and discuss the assessments carried out in order to try to reach a diagnosis and management plan, which is followed through by the nurse. This way of working has improved doctor time efficiency by 300% and allowed nearly 75% of new dementia referrals to be diagnosed this way.
The Trust is responsible for community and mental health services. Having specialists such as neurologists, neurophysiologists, speech and language therapists, physicians and many others within the same Trust has made for a much more collaborative way of working, breaking down barriers that sometimes exist when these services are within differing NHS Trusts.

WHAT IS THE CAMHS DEPARTMENT & TEAM LIKE?
The clinics accept referrals from a number of sources including General Practitioners, Social Workers, Health Visitors and Consultant Paediatricians. Referrals are processed through SPA (the Single Point of Access). The teams are multi-disciplinary including Child and Adolescent Psychiatrists, Psychologists, Mental Health Nurses, Child Psychotherapists, Systemic Therapists and Social Workers. These teams work on an outpatient model and currently receive approximately 65 referrals per month.

The team contains consultants with diverse experience & training from the UK, Spain, France, South Africa, India and the Middle East. Using a state of the art time management and reporting system with state of the art equipment across three laboratories - the services are part digitalised. Full digitalisation is expected in within 12-18 months. The laboratory is approved by the IBMS as a training laboratory for pre-registration and specialist training in Cellular Pathology. The department has full UKAS/ISO accreditation. The Department is HTA compliant and HFEA accredited and takes part in benchmarking exercises.

WHAT IS THE INPATIENT WARD & TEAM LIKE?
The ward is a mixed gender acute psychiatric ward with 18 beds. The ward provides assessment and treatment for patients who cannot be safely managed in the community by the Crisis Resolution & Home Treatment Teams. This includes a significant number of patients detained on Sections 2 and 3 of the Mental Health Act.
The ward serves patients residing in this London Borough, although may also receive some referrals from the other two boroughs within the Trust.

The post offers broad medical experience across the field of medical specialities. The holder of this post will usually work within one speciality team, however may be asked to cover in other areas across general medicine at times in order to maintain a safe effective service.

THE HOSPITAL & GENERAL MEDICINE DEPARTMENTS:
Within General Medicine, there are currently 19 Consultants, of which 13 are substantively employed with special interests in Respiratory Medicine, Cardiology, Gastroenterology, Diabetes and Endocrinology, Acute Medicine, Stroke, Rheumatology and Care of the Elderly.
An integrated service in general medicine and care of the elderly has matured within the Hospital. The team of consultants are enthusiastic and forward thinking. In addition to an acute in-patient and out-patient medical service, specialist diabetic and non-invasive cardiology services are provided.

This has been supported by both Regional, and National Accreditation processes. As part of the Avon, Somerset and Wiltshire Cancer Network, the Trust provides a range of cancer services. General Radiology duties will be based in the main department at one site, while sessions in breast imaging are located in a seperate site within the larger complex.
Performance:
42,225 women were invited for screening in 2017/18 with an uptake of 78.9% and the very successful High Risk MR breast screening service was implemented in 2014.
Symptomatic breast imaging services are provided by the breast imaging team independently and the symptomatic service receives approximately 4500 referrals per year. All these referrals are seen in a one stop clinic with surgical and imaging assessment in the same visit.
