The Oncology Department operates within the Cancer Centre Directorate, which is a part of the Planned Care Group at the Hospital. The Consultant team consists of two medical oncologists (soon to be three) and nine clinical oncologists who are primarily stationed at the Cancer Centre in the Hospital. They conduct outpatient sessions and chemotherapy clinics in various nearby locations.

The service participates in multidisciplinary team meetings. The Trust has a dedicated Acute Oncology Service led by one of the Medical Oncologists, with support from senior Clinical Nurse Specialists. In addition, there is a shared Oncology and Haematology ward with 20+ beds. Inpatient care for oncology patients is managed by the appropriate medical or surgical teams, with regular input from the Acute Oncology and specialist oncology teams.

The post holder will share clinical and managerial leadership within the Oncology department with the present consultants. The post holder will be provided with a designated office base with full secretarial and administrative support. There are also two laminar flow rooms for patients requiring bone marrow/stem cell transplantation and one dedicated room for radioactive isotope therapy.

There is a state of the art telemedicine facility allowing regular multi-professional meetings between oncologists, physicians, surgeons and AHPs in Wales. The system has the capacity to transmit and view both high quality radiological images and histopathological slides.  

The staff are highly experienced, consisting of nurses, radiographers, research officers and admin staff. The new consultant will be joining a friendly and forward-thinking consultant team of clinical and medical oncologists who are committed to delivering highest standards of cancer care, alongside a large team of other clinical and non-clinical professionals.

The integrated department of medicine provides inpatient, outpatient and occasional domiciliary consultation services for medical patients of all ages. In-patient and rehabilitation facilities are provided at the main hospital site. There are many acute beds provided for the directorate including a dedicated purpose built 6-bedded coronary care unit and 24 bedded Clinical Decisions Unit. There is also a combined HDU/ITU with a total of 14 beds.  

The department is supported by full pathology and radiology services. In addition, the department of physiological medicine carries out electrocardiography, echocardiography, stress testing, Holter monitoring and lung function testing. The biochemistry department works closely with the diabetes department. Physicians with an interest in Elderly Medicine provide a comprehensive service for the elderly in addition to contributing to the acute medical service for all adult age groups.

• Acute medical take on a 1:10 rota. Patients are cared for by on-call teams and acute physicians until their care is transferred to the wards where ward based teams care for patients.

• The geriatric medicine team delivers a front door and community frailty service.  
• The development of a Same Day Emergency Care unit will include Frailty pathways

The department operates a ward-based policy and have an average of 10-15 outliers on the 3 wards where the inpatient care are shared between the consultant in the department. There are rapid assessment clinics both at 2 sites offering GPs the opportunity for patients who require urgent assessment to be seen within 72 hours of referral. The department are also responsible for rapid access clinics, sub-specialty care in frailty, Orthogeriatrics, falls, and movement disorders.The older people’s physicians enjoy a close working relationship with the general physicians and there are good links with the old age psychiatry departments based at both the sites.

The junior medical team comprise one Specialty Registrar, a Trust Grade registrar, one CMT Senior House Officer, one FY2, three FY1 doctors and a physician associate. The department has a dedicated chest x-ray facilities, and a lung function laboratory. They also have a state-of-the-art endoscopy suite that accommodates full diagnostic bronchoscopy and EBUS facilities.

The ward currently has 27 beds on one of their sites, including four negative pressure side rooms, and provides acute non-invasive ventilation (NIV). There are also 14 beds on another site. The in-patient work also involves reviewing ward referrals. The respiratory firm receives a large number of ward referrals (20-25 per week) from other medical teams. The TB caseload is 100 notifications per year. The TB team is well staffed by 4 WTE nurses & 0.6 WTE case worker to provide latent TB screening, screening for immunosuppressive therapies, direct-observed therapy, community outreach as well as inpatient support.

The post holder is expected to lead the development of an outpatient sleep and ventilation service. The post holder is expected to provide General Internal Medicine and specialist respiratory and TB medicine service; this includes all aspects of treatment and relevant management duties for the proper functioning of the department and the patient flow across the Directorate.

WHAT IS THE DEPARTMENT & TEAM LIKE? 

The Team is a group of excellent & hardworking doctors that provides service in a town with a population of 100,000 with the 16-65 population of 61,000. There are two full time Consultants covering the Community Mental Health Team (this post being one of them). The Community Mental Health team comprises: Operational team managers, Service manager, CPNs, STR Workers, Non-medical prescriber, psychological therapists, and admin staff.

The community mental health team accept referrals from varies agencies including General practitioners, IAPT services, secondary and tertiary agencies. The current caseload is approx. 650 with a referral rate of approximately 4-6 patients per day. Inpatient services are provided in a new purpose-built 40-bedded unit four miles from the team base. A dedicated inpatient consultant psychiatrist and related team provide care for inpatients. The team is also supported by a CRHT service, which deals with all crisis referrals from 9am to 9pm with an on-call service thereafter. 

The post holder will have access to dedicated admin with a ratio of 3:1, with consultants also having full access to a secretarial pool for all preparation of letters and reports etc. The post holder is expected to manage, appraise and give professional supervision to junior medical staff as agreed between consultant colleagues and the medical director and in accordance with the Trust’s personnel policies and procedures. The post holder needs to ensure that junior medical staff working with the post holder operate within the parameters of the New Deal and are Working Time Directive compliant.
