The Acute Medicine Department and Team

The Acute Medical Unit (AMU) serves as the primary gateway for all medical admissions, with exceptions for those requiring direct admission to specialized units such as ITU, HDU, CCU, or Stroke Unit. Recently reorganized, AMU comprises an Assessment area, an ambulatory area, and short-stay beds with a maximum duration of 48 hours. The unit is staffed by three consultants, who provide flexible coverage throughout the day, gradually reducing to two consultants in the afternoon and one in the evening. A goal is to ensure all patients, barring overnight admissions, are seen by a consultant within 4 hours of arrival, with a further consultant review within 12 hours of admission.

The Same Day Emergency Care (SDEC) unit, inaugurated in July 2022, aligns with national guidelines, facilitating the swift evaluation and management of patients within a single day. The unit, jointly serving surgical and medical specialties, promotes collaborative care. Plans within the initial year involve optimizing patient throughput. Consultants allocate one day per week to the unit, which also offers rapid access Acute Medicine clinics to support early discharge from the Acute Medicine department.

In terms of consultant medical on-call responsibilities, the medical team admits an average of 50 to 75 patients over a 24-hour period. Consultants conduct reviews of medical admissions in the afternoon of their on-call day, commencing with a post-take ward round around 1:30 pm. The following morning, acute physicians review admissions to AMU from the previous evening and overnight, with the on-call consultant attending the 9 am handover and completing reviews of any remaining admissions on outlying wards.

The Acute Medical Unit (AMU) is equipped with 25 beds, organized into four bays and one sideroom. Consultants in Acute Medicine conduct thorough ward rounds starting at 08:00, followed by a secondary review at 14:00. They collaborate closely with junior staff to ensure timely patient assessments between rounds. The unit works in conjunction with the Rapid Assessment and Frailty Team (RAFT) to facilitate prompt discharges to home or community support when suitable. With approximately 10,000 patient assessments annually, translating to roughly 35-50 patients daily, the AMU handles a broad spectrum of acute medical conditions. All referrals to the AMU are streamlined through the Medical Streaming bleep, managed by a designated senior nurse serving as the central point of contact. The CAMHS Community Eating Disorders Service is a newly expanded, multi-disciplinary team which provides specialist CAMHS input to children and young people with eating disorders. It provides home-based treatment, out-patient treatment and gate-keeps referrals to in-patient services. The team has been successful in attracting Government funding to allow service expansion.

The post holder is Providing consultation, advice and appropriate support to professionals in the wider network of the young person and/or family. The inpatient service consists of one acute assessment and treatment medium secure male 4 4 ward, four treatment and intervention medium secure male wards, one female medium secure ward, one medium secure and one low secure male learning disability ward, one low secure male FICU ward and 4 low secure male wards. There are currently 25 Consultant Forensic Psychiatrists in post, working with 5 senior psychiatric trainees who rotate with posts at other forensic psychiatric services in the region. The service currently employs 10 specialty doctors, working in the prisons and in the rehabilitation parts of the service.

The Dermatology service sees 16,500 appointments yearly for diagnosing and treating various skin conditions, including skin cancers, lesions, eczema, psoriasis, urticaria, and Vulval conditions. Additionally, a limited number of clinics and treatments are available at the newly established outpatient facility located in one of their sites.

The Dermatology service currently has over 5000 2ww Outpatient attendances per annum for the diagnosis and treatment of potential skin cancer.
