What is the Respiratory Medicine Department & Team like?

NUH is one the largest respiratory departments in the country with a wealth of subspecialty services, ranging from asthma, COPD, cystic fibrosis, bronchiectasis, ILD including LAM, through lung cancer to infections including TB. In particular they are a regional centre for complex ventilation and sleep. The Respiratory service runs on both campuses; therefore cross site working is a requirement of the post. Respiratory inpatient beds are currently predominantly on the City campus. NUH has a dedicated respiratory assessment unit (RAU), taking direct admissions via 999 ambulance service, as well as transfers from the Emergency Department based at Queen’s Medical Centre campus. RAU has 20 beds, including 4 side rooms, and is led by a strong team of nurses, including Advanced Clinical Practitioners (ACPs) and a team of doctors, including a dedicated Trust grade post. On the QMC campus we have the 12 bedded Advanced Respiratory Care Unit (ARCU). The Respiratory team also provides in-reach cover to the Emergency department and acute medical admissions area based on the QMC campus. The successful candidate will join a team of 24 NHS Respiratory consultants and 8 academics, who also have a wide range of sub-specialty interests. The consultant team is supported by 8 middle grade doctors and a team of nurse specialists across COPD, lung cancer, oxygen, ILD, Asthma and TB. This multidisciplinary team manages a range of complex cases, including patients with a BMI below 15, those with complex medical or psychiatric comorbidities, pregnant patients, and diabetic patients. Treatments provided encompass individual therapy based on psychodynamic, cognitive-behavioural, and systemic approaches. The service also offers specific pathways for patients transitioning from CAMHS, pregnant patients, and SEED patients, alongside a dedicated Clinic for Eating Disorders and Diabetes, with plans for an ARFID clinic.

Facilities include both an outpatient service and a 15-bed regional inpatient unit, all housed in a self-contained unit on a general hospital site. Annually, the service is expected to handle a caseload of approximately 230 patients, with around 360 new referrals. The team works closely with the Community Mental Health and Wellbeing Teams, the Crisis and Home Treatment Team, and the Gastroenterology Departments at the NHS Trust. The Acute Medical Unit (AMU) is equipped with 25 beds, organized into four bays and one sideroom. Consultants in Acute Medicine conduct thorough ward rounds starting at 08:00, followed by a secondary review at 14:00. They collaborate closely with junior staff to ensure timely patient assessments between rounds. The unit works in conjunction with the Rapid Assessment and Frailty Team (RAFT) to facilitate prompt discharges to home or community support when suitable. With approximately 10,000 patient assessments annually, translating to roughly 35-50 patients daily, the AMU handles a broad spectrum of acute medical conditions. All referrals to the AMU are streamlined through the Medical Streaming bleep, managed by a designated senior nurse serving as the central point of contact. The CAMHS Community Eating Disorders Service is a newly expanded, multi-disciplinary team which provides specialist CAMHS input to children and young people with eating disorders. The team has been successful in attracting Government funding to allow service expansion.

We are currently looking for an experienced Clinical Oncologist, who works with both chemotherapy and radiotherapy treatments for a large acute hospital in the Midlands, England.

You\'d be joining a team of 8 clinical and 2 medical oncology consultants at the modern Oncology Centre (opened in mid 2010\'s), to help support and develop site-specific services in outpatient and chemotherapy as part of the development of the centre. 

There are 3 Elekta LINACS with VMAT and IGRT, using Mosaiq and Raystation for RT planning on site, along with 35 chemo day chairs used across 3 hospitals under this NHS trust.

WHAT IS THE DEPARTMENT & TEAM LIKE? 

The service is a team of excellent and forward thinking doctors consisting of: psychiatrists, psychologists, nurses, creative therapists and family therapists. There is currently funding for 4.8 WTE Community Consultant Child & Adolescent Psychiatrists. The service aims to keep children and adolescents at home with increased support if possible. The Trust provides a comprehensive Specialist Child & Adolescent Mental Health Service for young people that are 19 years and under. 

The department has a Crisis Team that operates 24 hours a day, 7 days a week. While the Home Treatment (Intensive Intervention Team) aims to work from 8AM to 6PM, 7 days a week. The services is provided at various locations across the town in order to be able to offer families an accessible and convenient service within a less stigmatising and less threatening environment. There is also a team that works with people aged between 14 and 65, who are experiencing their first episode of psychosis or might be at risk of developing psychosis.

The post holder expected to make assessment of referrals/admissions. The post holder will liaise and collaboratively work with other services/agencies. The successful candidate is expected to have a care plan and treatment formulation, guidance on evidence-based treatment and effectiveness.

Currently, three purpose-built units provide in-patient services for acute mental health. The in-patient units are supported by an Intensive Home Treatment Team, which aims to facilitate early discharge and treat people at home. Community mental health teams cover the areas mentioned above with Consultant input, and one Consultant based at the Department of Psychological Medicine undertakes liaison work.

Community Mental Health Services has a practice population of approximately 122,500, including 17,500 people over the age of 65 and 4,300 people aged 80 and above. However, there are already several services within the Trust that reduce the dependence of the catchment population on the CMHT. Both inpatient units have Intensive Home Care Teams that provide home-based treatments as an alternative to hospital admission, with the consultant responsible for the medical management of patients seen by each team already known to the CMHT in their locality.

The post holder is expected to provide high-level clinical leadership and practice to the multi-disciplinary team while providing a comprehensive mental health service to older people and their families. The appointee is also expected to provide medical leadership, support, and supervision to the Multi-Disciplinary Team, to participate in continuing professional development, and to comply with all Trust policies and procedures. The duties of the post are subject to review, and any alterations to the described duties will be mutually agreed between the appointee and the Trust.

WHAT IS THE DEPARTMENT & TEAM LIKE? 

The Rehabilitation Service is an innovative inpatient rehabilitation program with eleven wards. The service adopts a recovery-based holistic approach, led by enthusiastic multidisciplinary teams, to provide therapy and treatment promoting well-being, independence, and meaningful activity for patients with complex psychosis and related impairments. The service line is unique within the Trust, serving patients from eight Northwest London boroughs, and up to a third of its core business generates income. 

The service\'s 5.5 WTE Consultant Psychiatrists, along with a full multidisciplinary team and additional support services like substance misuse and employment support, ensure comprehensive care for patients across all their rehabilitation sites. Admissions are based on suitability assessments and approval by relevant Intensive Care Boards (ICBs), and the service collaborates closely with community mental health teams within the trust\'s Boroughs and neighboring mental health trusts who place patients in the service. Their Rehabilitation Service aims to enable patients\' recovery and help them achieve their optimum level of independence during their rehabilitation journey.

The post holder will have various duties, including providing clinical leadership to the team, acting as the Responsible Clinician for detained patients, and participating in care quality and business meetings. They will also be responsible for attending Mental Health Review Tribunals and Ministry of Justice hearings, assessing referrals promptly, and collaborating with care coordinators and placement panels for community placements. Additionally, the post holder will be involved in teaching and training junior doctors, medical students, and other clinical staff from different disciplines and offering support and advice to colleagues as needed.
