The TB caseload is 100 notifications per year. The TB team is well staffed by 4 WTE nurses & 0.6 WTE case worker to provide latent TB screening, screening for immunosuppressive therapies, direct-observed therapy, community outreach as well as inpatient support.

The Team is a group of excellent & hardworking doctors that provides service in a town with a population of 100,000 with the 16-65 population of 61,000. There are two full time Consultants covering the Community Mental Health Team (this post being one of them). The Community Mental Health team comprises: Operational team managers, Service manager, CPNs, STR Workers, Non-medical prescriber, psychological therapists, and admin staff.
The community mental health team accept referrals from varies agencies including General practitioners, IAPT services, secondary and tertiary agencies. The current caseload is approx. 650 with a referral rate of approximately 4-6 patients per day. Inpatient services are provided in a new purpose-built 40-bedded unit four miles from the team base. A dedicated inpatient consultant psychiatrist and related team provide care for inpatients. The team is also supported by a CRHT service, which deals with all crisis referrals from 9am to 9pm with an on-call service thereafter.
The post holder will have access to dedicated admin with a ratio of 3:1, with consultants also having full access to a secretarial pool for all preparation of letters and reports etc. The post holder is expected to manage, appraise and give professional supervision to junior medical staff as agreed between consultant colleagues and the medical director and in accordance with the Trust’s personnel policies and procedures. The post holder needs to ensure that junior medical staff working with the post holder operate within the parameters of the New Deal and are Working Time Directive compliant.

It is a truly exciting time to be joining the Child & Adolescent Mental Health Service as they are moving towards a county-wide service with clinics at localities. The department provides an assessment and treatment service for young people from the age of 0 – 18 years. The service has an expanded primary mental health staff and has recently been funded to start the Trailblazers project within schools and to support existing clinical services. The average number of sessions for treatment completion and discharge is between 8 and 10
CAMHS is committed to the THRIVE Framework which provides a set of principles for creating coherent and resource-efficient communities of mental health and wellbeing support for children, young people and families. This established CAMHS MDT is a long established specialist CAMHS service with an excellent reputation for innovation as well as established therapies. It follows a treatment model organised in multiple Care Pathways. All new patients are seen within 9 weeks of accepted referral by clinicians in multidisciplinary teams. The CAMHS service also offers embedded clinical sessions to partner agencies in London Borough. Through this we are able to offer consultation and direct work to mainstream schools, Special Schools and Pupil Referral Units. This service also extends to Children’s social care with clinicians embedded in Assessment and Intervention Teams, Children’s Centres, Youth Offending teams and Looked After Children’s Teams. WHAT IS THE GENERAL ADULT PSYCHIATRY DEPARTMENT & TEAM LIKE?
The Department is a modern 17-bedded en-suite unit which comprises doctors, qualified psychiatric nurses, occupational therapist, psychologist and health care assistants, providing assessment and treatment predominately for older people with complex or acute mental health needs, enabling a return to independent living wherever possible. The team accommodates all Older Adult services, including CMHT, Home Treatment Service and Memory Service. On average, there are approximately four new admissions and approximately four discharges per month. The majority of patients are admitted from their usual place of residence with a small percentage coming from the general hospital.

The Child and Adolescent Psychiatry Team is a small cohesive community based multi-disciplinary team. They are a team of outstanding doctors working together to provide the best service their patients can have. The team offers assessments and treatments to a wide variety of clinical presentations. They provide Early Intervention Service for the 13 to 18-year-old as well as assessment and treatment of young people presenting with high risk and severe life threatening self-harm. The Adolescent Mental Health Team is composed of a Child & Adolescent Psychiatry (joint consultant team-leads, this post and higher trainee), Systemic Family Therapy, Clinical Psychology, Child & Adolescent Psychotherapy and Mental Health Nursing.
The team model includes joint working between Psychiatry and allocated care-coordinator as well as individual working with young people and their families to provide developmentally informed evidence-based treatments. The Team offers neurodevelopmental assessments including ADHD, learning disability and Autism. Also to offer psycho-education and feedback to patients and their carers; and commence and/or monitor appropriate medication under supervision.
The post holder will provide assessments and treatment of young people presenting with severe and enduring mental health difficulties as part of a multi-disciplinary team assessment. The successful candidate will provide appropriate medical monitoring, psychiatric and therapeutic follow-up and treatment of a case load of young people under supervision of the Consultant. WHAT IS THE DEPARTMENT & TEAM LIKE?
The Team is a group of collaborative consultants who always makes sure that their patients get the best service. The department is well established with good support from senior clinical and managerial colleagues. They accept referrals from the SPA, Primary Care Liaison Team, Crisis Resolution Team, Inpatient wards, Child and Adolescent Mental Health Team as well as national Early Intervention services for service users with a first episode of psychosis. It normally works with service users for 3 years from age of 14 upwards. It functions between 9am and 5pm between Monday and Friday.
The department works in conjunction with other local mental health teams, and has developed close links with a range of community and youth services. This specialist multidisciplinary, community based team, provides intensive input to young people between the ages of 14 and 35 plus, who have experienced a first episode of psychosis. In the future it is anticipated that the service will extend the hours of operation from 8am to 8pm seven days a week. The team has a Key Performance target to review all new referrals within 2 weeks and meet the level 3 standard for Early Intervention services.
The post holder is expected to be responsible for carrying out detailed psychiatric assessments and risk assessments and to contribute to planning and implementing service users care plans, under CPA procedures and guidelines. The post holder is expected to carry out the above in a variety of settings, including home visits, and other non-stigmatising settings. The successful candidate is expected to work flexibly to ensure that clients and their carers are able to access the service effectively.

The NHS Foundation Trust support children and young people’s mental health through their schools projects across Somerset, and their child and adolescent mental health service (CAMHS). For adults, we offer a range of community-based mental health services. For those with more serious health needs, our community mental health teams offer specialist care to support recovery. Patients who require intensive, daily support and who may otherwise be likely to be admitted to in-patient care are seen in our home treatment service.
WHAT IS THE DEPARTMENT & TEAM LIKE?
The Department is a 14 bedded ward which admits adults of working age. The district covers a largely rural area of 285 square miles with a population of approximately 114,000. There are 275 admissions per 100,000 adult population in Somerset. Over the last three years there was an average of 223 admissions per year with bed occupancy being lower than the national average. The majority of the patients admitted were given a diagnosis of a psychotic illness, followed by a mood disorder and then personality disorders.
The post holder will be part of the team which consists of a ward manager 0.5WTE, a deputy ward manager 1WTE, a trainee doctor 1.WTE, a medical secretary 0.4WTE, a ward administrator 1.0WTE, a ward psychologist 0.5WTE, three occupational therapists, 11 staff nurses, 17 health care assistants, and 6 service assistants.
The post holder will also be responsible for conducting Mental Health Act assessments for adults of working age admitted to the ward. Other Mental Health Act assessments for adults of working age in Mendip will fall to the community adult consultant psychiatrists. The post holder may be invited to contribute.
