WHAT IS THE DEPARTMENT & TEAM LIKE?
???The total population of the area is 100,000 with the 16-65 population of 61,000. There are two full time Consultants covering the Community Mental Health Team (this post being one of them). The Community Mental Health team comprises: (1) WTE Operational team managers, (1) WTE Service manager, (6) WTE CPNs, (6) WTE STR Workers, (1) WTE Non-medical prescriber, (4) WTE psychological therapists, and (5) WTE admin staff. There are currently no junior doctors within the team – however this can be developed with the appropriate supervision qualifications - there is a band (7) non medical prescriber.
The community mental health team accept referrals from varies agencies including General practitioners, IAPT services, secondary and tertiary agencies. The current caseload is approx. 650 with a referral rate of approximately 4-6 patients per day. Within the caseload there are currently 3 patients on a CTO and 6 patients who are on a conditionally discharged MoJ Section. Every patient has a lead healthcare professional and the post holder will provide advice/input as requested and attend the majority of CPA meetings. You will see between 15 and 20 patients a week.
The post holder will have access to dedicated admin with a ratio of 3:1, with consultants also having full access to a secretarial pool for all preparation of letters and reports etc.

WHAT IS THE GENERAL ADULT PSYCHIATRY - COMMUNITY DEPARTMENT & TEAM LIKE?
The Psychotherapy Department has been successful in rolling out the IAPT programme and providing brief interventions to the GP practices. This has made a significant reduction in the number of GP referrals to secondary care and has facilitated progress in the New Ways of working. The outpatient clinics are increasingly CTP reviews/ consultation. It Is anticipated that the further introduction of IAPT services will potentially reduce the total number of GP referrals coming to the team as this component of a 'stepped care' approach is implemented.
The introduction of the Mental Health Measure has provided for a Primary Care Mental Health service to be established. In Carmarthenshire there is a system of Link working with GP Surgeries. The link workers are all CMHT members and provide weekly sessions in GP surgeries. The Speciality doctors from the community teams may have sessional time to support the Acute care service during working hours and to broaden their experience of liaison and acute care.
The post holder will not hold an outpatient caseload however outpatient clinics are held In the 4 CMHT's. The post holder will be responsible for the assessment and management of adult community patients or inpatients and is expected to remain closely involved with the care of those admitted to the associated wards. All these clinical activities will be supervised by the Consultant.

WHAT IS THE STROKE MEDICINE DEPARTMENT & TEAM LIKE?
The Stroke Centre provides Stroke & TIA services for the NHS Trust. The geographical area served is wide and the population diverse. We currently treat around 1400 stroke patients per year. There are currently 6 stroke specialist consultants and one associate specialist within the Centre, supported by a team of junior doctors in training grades, Clinical Nurse Specialists, and a well-established multidisciplinary team of stroke specific allied health professionals.
Inpatient services are centred on the Acute Stroke Unit (with dedicated HASU beds). There are currently 59 inpatient beds within the Centre; there are plans to increase this total number contingent upon appropriate staffing across medical and nursing specialties. As an increasingly important regional provider of stroke services, The department is seeking to enhance their service quality and scope through the recruitment of dynamic, forward thinking, flexible and innovative consultant colleagues who share the team\'s commitment to providing the very best in care for the populations they serve.
The post holder will be responsible for the continuing care & treatment of patients in post holder\'s charge & personal performance of clinical duties as agreed in the post holder\'s job plan, allowing for all proper delegation to & training the post holder\'s staff. The post holder is expected to participate in the clinical governance arrangements of the Trust & specifically within the Division including clinical audit, & research.

What is the Pathology Group like?
The group comprises four pathology laboratories providing pathology services for acute hospitals and also local GPs. Some of their laboratories also offer specialist services to the wider NHS and work on research studies and trials. As part of a major initiative in the NHS, Pathology in England is being reorganised into much larger local network models and this sees the formation of 29 pathology units across the country.

The centre sees roughly 120,000 cases across a team of 36 consultants (plus some outsourcing) with a capacity to recruit to 40 full-time consultants
What is the Pathology Department Like?
Formed as a result of the Lord Carter review of Pathology services across England which identified the need for significant reductions in over testing, improved resilience and reduced costs. Serving 1.4 million people and performing 60 million tests each year, the group needed to completely redesign its IT systems to operate effectively and efficiently, allowing them to offer integrated service, adopt new technologies to improve flexibility, and deliver better laboratory results faster.
Highly praised, and considered one of the best centres to work in the UK as a pathologist. The evidence is there to be seen, as latest figures show the BCPS scored 92 per cent compliance on a dashboard provided by NHS Improvement, ranking it one of the best performing services in the country.

The Department of Obstetrics and Gynaecology consists of: -12 substantive Consultant Obstetricians/Gynaecologists and 1 WTE locum consultant. 5 new consultants participate in the out of hours middle grade rota. There are 4 specialty doctors, 1 trainee and 3 senior clinical fellows who complement the middle grade day time and out of hours rota. This post is to complete the full complement of middle grades in order to work a 1 in 9 on call rota.
The birth rate at the Hospital maternity unit has remained steady around 2700 per annum. The Labour Ward, maternity wards, Special Care Baby Unit, Obstetric theatre, Intensive Care, Gynaecology ward and the main operating theatres are located on the first floor of the hospital. There is a dedicated Obstetric theatre within the labour ward complex. There is a dedicated modern ultrasound machine, housed in the antenatal clinic area used for fetal scanning including interventional procedures like amniocentesis and chorionic villous sampling.
The post holder will involve routine clinical and on call commitments with opportunity to complete ATSM in certain areas, as well as research opportunities.

WHAT IS THE NHS FOUNDATION TRUST LIKE?
They were established as an NHS Trust in April 1991, and became a Foundation Trust in April 2008. Since then, the Trust provides a good quality, safe, and effective healthcare to the people of several counties. The Trust manages three different hospitals/centres. The Trust is currently responsible for the provision of a range of community services for people across several areas, working in partnership with local GPs and another NHS Trust.
The NHS Trust employs almost 5,000 members of staff, have 540 hospital beds, with a range of services including accident and emergency, maternity, outpatients, therapies, and children's health. WHAT IS THE DEPARTMENT & TEAM LIKE?
The Respiratory Medicine Team is a fully integrated team that works closely with all members of the multidisciplinary team that is involved with the care and support of patients with Respiratory illness. They are involved in both the acute management of patients admitted to Hospital as well as the chronic disease management of respiratory patients. They liaise closely with both primary and secondary care.
The Department has 5 services:
- Ward 5 - 32 Acute Respiratory beds; 2 Non-Invasive Ventilation (NIV) beds.
- Lung Function Testing (including patient flight assessments)
- Specialist Radiology Services
- Tuberculosis, Lung Cancer, Interstitial Lung Disease (ILD) Clinics
- Pulmonary Oncology Clinics
The Service covers all respiratory patients in the acute trust and community. The Team offers support to patients at home following acute exacerbation of their chronic respiratory condition that has required admission in to hospital. They provide support and information for lung cancer patients and palliative care for patients who have non-malignant disease. The team also provides education for all staff.
