What is the Trust like?
The Trust is one of the largest acute and community providers in the West Midlands having more than 850 beds on one of their 3 sites in the area. The trust has had a new £38 million Emergency Department was opened at the Hospital. Boasting 30 treatment rooms for minor injuries, 18 for major injuries and a resuscitation area for adults, as well as a dedicated paediatric area for children, one of the largest in the region.
The trust has also been rated outstanding in caring in February 2002 by the CQC following an inspection. Additionally in 2018, a new 2.4 million Stroke Unit opened at one of their sites, now all suspected stroke cases from the near by cities are treated in this specialist unit.
What is the Department Like?
Our Stroke service provides Hyper-acute and Acute Care for the combined populations of all the peripheral areas with the hospital which has a combined population of around 600000 people. The department provides a 24/7 service for stroke thrombolysis and thrombectomy pathway consisting of 2 ring-fenced HASU beds for Stroke admissions out of 12 designated HASU beds with a dedicated consultant as a part of rota. The department also consist 24/7 access to CT clinics, 3 CT scanners along with 3 MRI scanners (4th MRI is currently being deployed). In Addition to that, the department have rapid access to outpatient CTAs and have a very efficient pathway with the Vascular service for rapid carotid endartectomy intervention if that is required.

The department holds a weekly staff educational meeting, a Consultant-led Radiology meeting, and regular departmental consultant meetings, with two monthly clinical governance sessions. Consultants do not work in isolation, and are partnered in pairs in each base ward area at any one time, allowing opportunity for collaborative working and mutual support. The department takes part in regular presentations at the Grand Round and has a well-established postgraduate and undergraduate medical training programme.
The department provides undergraduate teaching for third/fourth/fifth year medical students and contributes to medical school examinations. The specialty currently manage patients across a number of inpatient services. These include the review of acute admissions in the Emergency Zone, a newly created Acute Frailty Unit with 32 beds, with post-admission short stay capacity. There are two complex care wards that has 32 beds on each ward, with a further short stay bed base (c.16 beds). Transitional post- acute care beds are located in a nearby ward, which includes 70 beds.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The team is a mix of professionals from different disciplines including doctors in psychiatry and neurology, nurse therapists trained in physical and mental health, psychological therapy, physiotherapy & occupational therapy. They work with patients who have a mixture of different problems including mental illness associated with neurological diseases, epilepsy and non-epileptic attacks, functional neurological symptoms & neurological sleep disorders. They aim to provide high quality specialist assessment and treatment plans that incorporate expertise in both physical and mental health.
Being a part of the neuroscience department, they work closely with our colleagues in neurology and neurosurgery. They also provide neuropsychiatry support for the mental health liaison team for inpatients in other wards at Southmead Hospital. Their outpatient clinics take referrals from across the south west from general practitioners, neurologists, neurosurgeons and psychiatrists. Their inpatient service is situated within the Rosa Burden Centre. We have two inpatient beds for patients with neuropsychiatric conditions, and two inpatient beds for assessment of sleep disorders.
The post-holder will divide the management of the 29 Level 1, and 23 Level 2 in-patients, with the two Consultants in Rehabilitation Medicine, depending on medical/psychiatric need. In addition to the session of NBT Outpatient Clinic work, there is a further session allocated to providing support for the community neurorehabilitation team based at the Head Injury Therapy Unit (HITU).

WHAT IS THE DEPARTMENT & TEAM LIKE?
The Stroke Department\'s Hyper Acute Stroke Unit (HASU) is 6 bedded, and has a twice daily consultant-led ward round with the junior doctors and stroke nurse. The post holder will join another 6 stroke physicians to provide cover to the on a rotational basis. A robust thrombolysis pathway has been in operation since 2008 allowing paramedics to bring FAST positive patients from the county to UHCW ED with a pre-alert. All FAST pre-alert calls go directly to the stroke nurse specialist for initial triage and advice. The HASU has access to 7 day physiotherapy, occupational therapy, vascular ultrasound, CT scanning.
Another unit in the department is the Acute/Rehabilitation Stroke Unit which has 30 beds. The multidisciplinary stroke team consists of physiotherapy, occupational therapy, speech and language therapy, dietetics and neuropsychology. Stroke Association family support workers and the Intermediate Discharge Team are also available on the unit. An early Supported Discharge Service is now well established and members of the team attend comprehensive daily morning board rounds. Currently around 60% of our stroke patients are seen by the ESD service.
There is a daily dedicated TIA clinic held adjacent to the stroke Unit with rapid-access protocols in place from primary care and ED. Clinics are run as a one-stop shop with direct access to carotid Dopplers and Trans cranial Ultrasound and MRI brain scans . The service receives approx. 1000 referrals per annum. The post holder will take part in the daytime acute stroke on-call cover and participate in running the day to day activities, such as ward rounds and MDTs on the stroke ward along with running acute TIA clinics and stroke follow up clinics.

What is the NHS Trust like?
This Trust was formed through the merger of two previous NHS Trusts which took place in April 2020. It serves a population of around 270,000 across the region and surrounding areas, with a catchment of 900,000 for vascular services, 24 of which are maternity, 16 are paediatric, ten are critical care and 28 are day-case beds. Its core local authority populations are around 160,000 and 260,000.
The hospital has over 2500 members of staff, making it the largest local employer in the area, and a current turnover of approximately £205m. The hospital enjoys strong community support with over 200 volunteers at the hospital and over £6 million being raised by the it\'s independent charity over the past 10 years. The support from the charity has enabled the hospital to build a dedicated cancer centre and it has recently funded a new MRI scanner, operational since early 2018.
The main hospital was erected on the site of a General Infirmary. The Infirmary was originally opened in 1803 providing accommodation for 38 patients. Now, the hospital has approximately 400 beds.
What is the Oncology Department like?
The new consultant will be joining a friendly and forawrd-thinking Oncology Department which includes 8 consultants, covering a variety of specialities. There are currently no Junior Doctors in place, although this may change in the future with expansion of training posts. Currently, support to Consultants is provided by Chemo Nurse Practitioners as well as AOS Nurses.
The cancer centre houses the hospital’s outpatient oncology service for cancer patients and new and follow-up patients are seen in one of the several clinics each week. Chemotherapy, monoclonal antibody therapy and other anti-neoplastic agents are administered here daily by one of their specialist chemotherapy nurses, and supported by a phlebotomist in the unit. The chemotherapy suite offers daily sessions, for oncology patients and shared care for haematology patients.
The cancer centre also has its own phlebotomy service, cancer information and support centre as well as a palliative care team. The unit provides a PICC line service and 24hr telephone support line for chemotherapy patients.
The post holder will be responsible to the Medical Director & the Clinical Director, and is required to prepare for and undertake the protective interventions that he/she is responsible for in a manner that is consistent with evidence based practice and maintaining patient, staff and other users safety.
What Career Development and Further Opportunities are available?
The Hospital\'s new Education Centre opened in August 2020. This Centre represents a £1.2M investment to provide a training environment for all Hospital NHS staff. Within the Centre, there is a Lecture Theatre, 2 classrooms, a Clinical Skills Lab, a Resuscitation Training Room and a Simulation Suite. The Centre is also equipped with Video-conferencing facilities. What is NHS Foundation Trust like?
This NHS Trust provides community and hospital services across the entirety of an area covering a thousand square miles. It consists of 4 main hospitals, the new consultant will work at 3 of these. What is Oncology Department like?
One of the largest employers in the area, this trust have approximately 6,000 staff working across 3 sites. The new consultant will be expected to work across at least two of the three sites, which range in sizes from 33 beds to 450, contributing to the acute oncology inpatient service. The Trust has a recognised Cancer Unit, providing systemic therapy for all common tumour sites. It has six Linacs and employs thirteen clinical oncologists and three Medical Oncologists.
There are currently five Medical Oncologists and one Clinical Oncologist employed at the trust, providing acute oncology services, in addition to a site specialised SACT service. There are also three visiting Clinical Oncologists. Inpatients are managed on the acute medical wards and outpatients are seen and treated in dedicated units on all three sites. There is a Medical Oncology SpR on rotation from another hospital attached to the team, three Nurse Practitioners and one AOT CNS to support the acute oncology service, but also chemotherapy clinics on all sites.

WHAT DOES THE INPATIENT DEPARTMENT LOOK LIKE?
The wards admit a mixture of Informal and detained patients. Detained patients account for 60% of patients on the wards. Delayed transfer of care patients account for 10% of the wards population. The wards admit adults with various psychiatric disorder and majority suffer with severe enduring mental illness. Patients with personality disorder tend to account for 40% of the wards population and they tend to have the below average length of stay. The post holder is expected to support patient flow team in their effort to facilitate discharges from the ward. The current length of stay (LOS) on our acute ward is 50 days but we are working reducing this to 25 days.

This is a great opportunity for overseas gastroenterology doctors who want to establish themselves in the NHS, and grow their consultant career. The post is suitable for candidates holding specialisation in the EEA, or for experienced IMGs looking to complete CESR.
WHAT IS THE NHS FOUNDATION TRUST LIKE?
The trust is a dynamic and transforming Trust, committed to delivering the highest quality service to the communities it serves. The trust consists of 5 hospitals as well as a number of community locations across the population. The Trust offers a comprehensive range of acute services, with a bed-base of 500, to around 400,000 people in North Kent and South East London. The Trust is geographically located in the North of Kent with excellent transport links including major motorways and train lines to Kent, London and Europe.
Our Planned Care Centre at one of the hospitals (a multi-provider hospital site) provides a dedicated elective care service for a range of surgical services. WHAT IS THE DEPARTMENT & TEAM LIKE?
The Endoscopy Unit at the Hospital is fully JAG accredited and is a purpose built three roomed Endoscopy Unit with modern equipment for cleaning and sterilisation on site. All the Endoscopes are Olympus Endoscopes (290 series) complete with new stacks including two screens for viewing and an integrated colonoscopy scope guide feature. C02 insufflation is available and ERBE diathermy plus access to APC. Darent Valley Hospital is an accredited BCSP site providing colonoscopy and flexible sigmoidoscopy procedures.
An Endoscopy Unit Extension with 2 addition rooms and fully JAG accredited decontamination facilities is due to be commissioned. Furthermore, the Endoscopy Unit at the other main hospitals provides diagnostic gastroscopy and colonoscopy procedures. It is progressing through the process of JAG accreditation. There is also Screening Room for ERCP and oesophageal stenting or dilatation equipped with a mobile C arm within the Endoscopy Unit in the largest room which was purpose built for screening procedures.
The gastroenterology team consists of 7 consultants and 2 specialists registrars having the responsibilities for both inpatient and outpatient care. The consultants also actively participate on call rota (at 1 in 50) and the Upper GI bleed rota (at 1 in 7)
WHAT OPPORTUNITIES ARE AVAILABLE TO YOU?
Active participation in Education and Research will be encouraged and supported along with the gastroenterology team having weekly lunchtime education meetings. The hospital has an active Research and Development Team supported by Specialist Research Nurses helping doctor to build their interest in teaching in research.
You will also be responsible for all the multi-disciplinary audit is conducted on a monthly basis, led by a designated Consultant responsible for audit. There are also opportunities for leadership development for medical staff with an interest and potential to take greater responsibility in the Trust. Mentoring support for newly appointed Consultants is also available.
Additionally, active participation in Directorate and departmental multidisciplinary clinical review, educational, strategic and governance meetings is expected in order to support service delivery and development. The department also supports for study leave which is 30 days in three years with £800 per annum towards expenses at the discretion of the Medical Director.
WHAT IS LIFE LIKE IN THE SOUTH EAST?
Placed access to London and rural Kent where living and working in the area gives a great choice of places to live, and relax. London, Bromley, Canterbury, Sevenoaks and Tunbridge Wells are all within easy commuting distance. The Trust sits in the Thames Gateway development area; several housing developments have been completed, but others are in the planning stage.
Kent offers an excellent range of primary and secondary schools. Kent continues to operate a grammar school system, with places allocated by an 11+ examination along with excellent independent schools in the area.
London is less than an hour away by rail or road. The choice of things to do and see is vast, with museums, national landmarks, cathedrals and ancient churches, theatres with international stage shows and musicals, shopping in the West End and Knightsbridge, Royal Gardens, Parks and Restaurants. The main road from London to Canterbury, the A2 is no more than five minutes away allowing easy access to London, the Kent Coast and the Continent. Just as close is the M25 motorway providing easy access to all the Gatwick, Stansted and Heathrow airports, the Home Counties and the national motorway network.

The NHS Foundation Trust serves a population of approximately 310,000. It administers services to an area coterminous with that of a primary care trust and a council. The NHS Trust (currently 780 beds) has been developed as the District General Hospital for the area.
The NHS Trust provides patient care in the community at health centres and clinics as well as services such as district nursing and intermediate care. They are a major provider of hospital and community health services in their county, delivering services from their hospital and also providing a wide range of community services from locations across another county. One of their hospital is a major hub for women’s and children’s services and is the second busiest ambulance-receiving site in their area.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The Department is striving to provide best possible care to the large acute emergency medical take and also to the critically ill. Currently, the NHS Trust has 780 beds of which 388 are acute Medical/Elderly. These are composed of the Acute Medical Unit, 8 Acute Wards, a new 10 bedded Coronary Care Unit and a closed ICU/High Dependency Ward. In addition there is a Stroke Unit, Elderly Rehabilitation and Continuing Care beds. The 62 bed Acute Medical Unit and the Ambulatory Care Unit receives all acute referrals from General Practitioners and from the Accident and Emergency Department and provides for early assessment of all patients by the Duty Consultant or Specialist Registrar.
Out of hours cover for acute medicine is provided on a 1 in 18 rota during the week and 1 in 9 at weekends in combination with the rest of the medical consultants/specialists . They also cover the ACU 1600-2000 on a rotating scheme between the GIM physicians. The doctors on call shares care from 1300 each weekday, stay on the unit until 2000 and then supply cover from home. The average number of acute medical admissions is 45-60 per day. On call rota follows a pattern shared between 15 Consultants and is thus a 1 in 15 rota during the week and because two consultants are on call -1 in 7.5 weekends.
The person appointed will work within the Acute Adult Division. The appointee will be expected to join the on call rota for GIM which will be reflected in the Job Plan. The post holder will have office accommodation equipped with computer and internet access. Full secretarial support will be provided.

The area is an industrial, progressively minded town, thirteen miles to the North of Manchester, with an impressive centre attracting a wide range and rich supply of cultural activities. The area is centred at the hub of the North Western Region and there is a convenient motorway network giving fast access to neighbouring towns, the Lake District, the Yorkshire Dales and Fylde Coast. The general environment in the area and the surrounding countryside is a pleasant one, with good residential areas offering a wide choice of housing styles at competitive prices.
