WHAT IS THE GENERAL ADULT PSYCHIATRY DEPARTMENT & TEAM LIKE?
The Old Age / Geriatric Psychiatry Mental Health Service provides a range of mental health services for adults across the age range within the community and in hospital settings. These are integrated health and social care services. Many services across the boroughs are now integrated because of the synergies of the two patches, with members of the senior management often having overarching responsibilities. There is a middle/second on call tier through higher trainees and first on call provision is provided by 3 resident junior doctors.
The post holder will provide senior clinical advice and support to the daily triage, MDT and other regular clinical team meetings, ensure that all referrals are triaged daily and responded to in a timely way, and do assessment of new patients and clinical supervision of assessments carried out by other team members. The post holder review complex needs patients to ensure appropriate care packages are put in place, have Mental Health Act related-work, and do domiciliary visits to provide assessment and treatment services as a member of the CMH.

The Mental Health Crisis Assessment Service (MHCAS) was launched at the end of 2022 and is based in Central London. The service offers a calm and therapeutic mental health setting to treat the majority of emergency mental health presentations, seeing and asses anyone over 18 years old that would otherwise go to an emergency department for mental health reasons and does not have an urgent medical need (for example, an overdose of medication). It offers full emergency mental health assessments and onward care planning in the department with a diverse and highly skilled mental health workforce incorporating nurses, doctors, support workers and access to drug and alcohol workers and psychology. The service can support every aspect of emergency mental health care planning from GP referrals to Mental Health Act Assessments.
The majority of services are provided in the community, which means treating people in their homes or from clinics close to home. WHAT IS THE ONCOLOGY DEPARTMENT LIKE?
The oncology department has the facilities to diagnose and treat many types of cancer in both adults and young people. They provide a variety of inpatient, day-case and outpatient services for cancer treatments through their day-therapy centre which provides a comprehensive range of chemotherapy delivery, alongside procedures including Intrathecal chemotherapy, Bone-marrow biopsies and Scalp cooling for specific types of chemotherapy treatments. Treatment is also provides through their highly specialist ward where patients who require inpatient treatments, such as complex and lengthy chemotherapy, are cared for. Recently, they have also seen a brand-new state of the art radiotherapy centre, run by neighbouring trust which enables their patients to access radiotherapy and specialist services closer to home.
The new consultant will join a well-established, friendly, and supportive team made up of 8 senior medical staff members including consultants and SAS doctors, within the Medical Oncology department.
WHAT CAREER DEVELOPMENT & OPPORTUNITIES ARE AVAILABLE?
The new consultant\'s job plan will include a minimum of 1.5 SPAs for CPD, which will allow them to conduct personal study, attend educational meetings, conduct QI/Audits, complete appraisals, participate in mandatory training, as well as support basic teaching sessions. In addition, there are a variety of roles that attract additional SPA, such as being an appraiser, taking on lead roles, chairing meetings, rota leads, and providing educational supervision. WHAT IS THE DEPARTMENT & TEAM LIKE?
The department holds a weekly staff educational meeting, a Consultant-led Radiology meeting, and regular departmental consultant meetings, with two monthly clinical governance sessions. Consultants do not work in isolation, and are partnered in pairs in each base ward area at any one time, allowing opportunity for collaborative working and mutual support. The department takes part in regular presentations at the Grand Round and has a well-established postgraduate and undergraduate medical training programme.
The department provides undergraduate teaching for third/fourth/fifth year medical students and contributes to medical school examinations. The specialty currently manage patients across a number of inpatient services. These include the review of acute admissions in the Emergency Zone, a newly created Acute Frailty Unit with 32 beds, with post-admission short stay capacity. There are two complex care wards that has 32 beds on each ward, with a further short stay bed base (c.16 beds). Transitional post- acute care beds are located in a nearby ward, which includes 70 beds.
The post holder will be responsible for the clinical management of patients under their care, and, in liaison with consultant colleagues, continuity of care when the post holder is absent. WHAT IS THE DEPARTMENT & TEAM LIKE?
The team is a mix of professionals from different disciplines including doctors in psychiatry and neurology, nurse therapists trained in physical and mental health, psychological therapy, physiotherapy & occupational therapy. They work with patients who have a mixture of different problems including mental illness associated with neurological diseases, epilepsy and non-epileptic attacks, functional neurological symptoms & neurological sleep disorders. They aim to provide high quality specialist assessment and treatment plans that incorporate expertise in both physical and mental health.
Being a part of the neuroscience department, they work closely with our colleagues in neurology and neurosurgery. They also provide neuropsychiatry support for the mental health liaison team for inpatients in other wards at Southmead Hospital. Their outpatient clinics take referrals from across the south west from general practitioners, neurologists, neurosurgeons and psychiatrists. Their inpatient service is situated within the Rosa Burden Centre. We have two inpatient beds for patients with neuropsychiatric conditions, and two inpatient beds for assessment of sleep disorders.
The post-holder will divide the management of the 29 Level 1, and 23 Level 2 in-patients, with the two Consultants in Rehabilitation Medicine, depending on medical/psychiatric need. In addition to the session of NBT Outpatient Clinic work, there is a further session allocated to providing support for the community neurorehabilitation team based at the Head Injury Therapy Unit (HITU).

What is the NHS Trust like?
This Trust was formed through the merger of two previous NHS Trusts which took place in April 2020. It serves a population of around 270,000 across the region and surrounding areas, with a catchment of 900,000 for vascular services, 24 of which are maternity, 16 are paediatric, ten are critical care and 28 are day-case beds. Its core local authority populations are around 160,000 and 260,000.
The hospital has over 2500 members of staff, making it the largest local employer in the area, and a current turnover of approximately £205m. The hospital enjoys strong community support with over 200 volunteers at the hospital and over £6 million being raised by the it\'s independent charity over the past 10 years. The support from the charity has enabled the hospital to build a dedicated cancer centre and it has recently funded a new MRI scanner, operational since early 2018.
The main hospital was erected on the site of a General Infirmary. The Infirmary was originally opened in 1803 providing accommodation for 38 patients. Now, the hospital has approximately 400 beds.
What is the Oncology Department like?
The new consultant will be joining a friendly and forawrd-thinking Oncology Department which includes 8 consultants, covering a variety of specialities. There are currently no Junior Doctors in place, although this may change in the future with expansion of training posts. Currently, support to Consultants is provided by Chemo Nurse Practitioners as well as AOS Nurses.
The cancer centre houses the hospital’s outpatient oncology service for cancer patients and new and follow-up patients are seen in one of the several clinics each week. Chemotherapy, monoclonal antibody therapy and other anti-neoplastic agents are administered here daily by one of their specialist chemotherapy nurses, and supported by a phlebotomist in the unit. The chemotherapy suite offers daily sessions, for oncology patients and shared care for haematology patients.
The cancer centre also has its own phlebotomy service, cancer information and support centre as well as a palliative care team. The unit provides a PICC line service and 24hr telephone support line for chemotherapy patients.

WHAT DOES THE INPATIENT DEPARTMENT LOOK LIKE?
The wards admit a mixture of Informal and detained patients. Detained patients account for 60% of patients on the wards. Delayed transfer of care patients account for 10% of the wards population. The wards admit adults with various psychiatric disorder and majority suffer with severe enduring mental illness. Patients with personality disorder tend to account for 40% of the wards population and they tend to have the below average length of stay. The post holder is expected to support patient flow team in their effort to facilitate discharges from the ward. The current length of stay (LOS) on our acute ward is 50 days but we are working reducing this to 25 days.

What is the Oncology Department like?
This Cancer Centre provides a high degree of specialisation for all the major cancers. It has one dedicated ward of 30 beds (with scope to increase to 36 beds at periods of high demand) for both oncology and haematology patients, together with a chemotherapy and haematology day unit, and an aseptic unit on site. There are also two laminar flow rooms for patients requiring bone marrow/stem cell transplantation and one dedicated room for radioactive isotope therapy.
There is a state of the art telemedicine facility allowing regular multi-professional meetings between oncologists, physicians, surgeons and AHPs in Wales. The system has the capacity to transmit and view both high quality radiological images and histopathological slides.
The staff is highly experienced, consisting of nurses, radiographers, research officers and admin staff. The new consultant will be joining a friendly and forward-thinking consultant team of clinical and medical oncologists who are committed to delivering highest standards of cancer care, alongside a large team of other clinical and non-clinical professionals.

What does the stroke department look like?
The stroke service consists of six acute stroke beds and an assessment bay benefitting from an investment of £1million in 2017. The post holder would be expected to contribute to daily weekday Stroke Consultant rounds, and the Stroke Consultants provide face-to-face Thrombolysis within normal weekday working hours and the Emergency Department team access the tele stroke service out of hours for support. There is a 7-day Advanced Nurse Practitioner / Stroke specialist nurse service attending all stroke patients presenting in ED. The stroke unit has a combined of 32 beds - 6 acute and 25 rehab along with neurosurgical and thrombectomy services available at one of the hospitals. The division provides a wide range of techniques and services, including endocrine investigations; DEXA scanning; GI endoscopy; bronchoscopy; chemotherapy; a cardiac, respiratory and oesophageal function laboratory; and ward-based non-invasive positive pressure ventilation.

The department currently have three full-time urological surgeons, a Locum Consultant along with 5 middle grade registrar level training including 1 SpR from the Mersey Deanery training programme. There are two SHO level staff and one FY1 and FY2. The Department has recently undergone a refurbishment, to provide day-case urology and therapeutic services to include TRUS-Guided prostate biopsy lists, an endoscopy suite, dedicated lithotripsy sessions and specialist out-patient clinics. The department has inpatient beds based on the 3rd floor within the Surgical Department. Paediatric beds are also available on the main hospital site.
The department uses one of the two dedicated purpose built modern integrated laparoscopic surgery theatres for its heavy laparoscopic workload. Currently, major laparoscopic Nephrectomies and prostatectomies are performed in this OR. A 12 bedded Intensive Care/High Dependency Unit is located on the first floor of the hospital. There are excellent pathology facilities, including a Public Health Laboratory. The Radiology Department offers a full range of facilities, including CT, MRI and fully functional intervention radiology service.
The post holder will provide elective and emergency inpatient services, along with other members of the department. The post holder will also participate actively in the Department’s Clinical Audit Programme and Continuing Professional Development, which is fully supported by the Health Board.
