What is the department like?
The department consists of 5 consultants along with 3 full time nurse specialists, 2 part time sleep nurse specialists, 1 registrar, 2 trainees and 4 FY doctors in the department. The ward is a 23-bedded unit typically ranging 5-8 patients per days. The hospitals is a modern 27 bedded unit for both Respiratory and general medical patients usually shared by 2-3 consultants in the department.
The service has had a significant modernisation in the last few years expanding junior doctors into subspecialties like Lung Cancer focussing more on EBUS with lymph node aspirate and biopsy, a dedicated plural procedures room, improved thoracoscopy business cases, 10 brand new non-invasive ventilation machines, Participation in the regional asthma network / MDT, Participation in the regional Interstitial Lung Disease Network.
CESR
International candidates are welcome and CESR will be supported. Direct input will be provided on a regular basis from the Clinical Director, with opportunity for shadowing, lists and portfolio building built into the job plan.
The team has recent experience of supporting IMGs with the CESR process, benefiting any newly appointed candidate.
The remainder of the appointee’s professional time will be deployed flexibly on other clinical duties, administration, teaching, research, clinical audit and continuing medical education. The Trust is excited to be developing a Hyper-Acute Stroke Unit (HASU) at the Cumberland Infirmary site which will comprise a 10 bedded HAS unit at CIC, a further 25 acute and rehabilitation beds, and a further 15 acute and rehabilitation beds at WCH. If you successful, you will join the newly forming team and have the opportunity to contribute to this exciting development for our patients in both the Cumberland Infirmary and the West Cumberland Hospital. You would also be instrumental in the establishment of a high quality stroke service across North Cumbria. The planned 7-day service model will allow the delivery of 7-day stroke / TIA clinics which will rapidly speed up diagnosis and therapeutic intervention, and will provide an integrated pathway into community services.
This development was implemented in the last few years, with the new service becoming increasingly embedded into Integrated Care Communities (ICCs) specialist ESSD rehabilitation that is delivered at local level under the joint management of the ICC and the ESSD coordination team.

The department currently have three full-time urological surgeons, a Locum Consultant along with 5 middle grade registrar level training including 1 SpR from the Mersey Deanery training programme. There are two SHO level staff and one FY1 and FY2. The Department has recently undergone a refurbishment, to provide day-case urology and therapeutic services to include TRUS-Guided prostate biopsy lists, an endoscopy suite, dedicated lithotripsy sessions and specialist out-patient clinics. The department has inpatient beds based on the 3rd floor within the Surgical Department. Paediatric beds are also available on the main hospital site.
The department uses one of the two dedicated purpose built modern integrated laparoscopic surgery theatres for its heavy laparoscopic workload. Currently, major laparoscopic Nephrectomies and prostatectomies are performed in this OR. A 12 bedded Intensive Care/High Dependency Unit is located on the first floor of the hospital. There are excellent pathology facilities, including a Public Health Laboratory. The Radiology Department offers a full range of facilities, including CT, MRI and fully functional intervention radiology service.
The post holder will provide elective and emergency inpatient services, along with other members of the department. The post holder will also participate actively in the Department’s Clinical Audit Programme and Continuing Professional Development, which is fully supported by the Health Board.

The team is a forward thinking practice, with a nurse-led triage, practice Pharmacist and Physiotherapist, working with a motivated support team, who provide document management to reduce the GP paperwork.
Education is high on their agenda as a Training practice for medical students and GP trainees. They are keen to provide education/clinical sessions to develop and support the post holder's specialist interests, within their working week. As a family friendly practice they will consider home working sessions via attend anywhere, with support from Consultant Connect.
The post holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients. The hospital provides a full range of general Obstetric & Gynaecology services, primarily on the main hospital site, as well as outpatient services in satellite community hospitals at several areas. The delivery rate has been stable around 2200 per annum over recent years, but the complexity of cases has increased significantly over the last year with the full transition of specialised neonatal intensive care services in North Wales centralised at the hospital's area. This provides cover for over 7000 women a year.
Currently, there are 9 substantive Consultant Obstetricians & Gynaecologists, all provide general obstetrics & Gynaecology services. Three new consultants participate in the out of hours second on call rota. There are six specialty doctors / Senior Clinical Fellows who complement the second on call daytime & out of hours rota. There are eight junior doctors who participate in the first on call out of hours rota. The service provides a local Gynaecology Urgent Suspected Cancer (USC) referral service, and manage some Gynaecology cancer cases locally, networked to the North Wales Gynaecology-oncology surgical centre based at Bangor. Fertility & Urogynaecology services are networked across North Wales.
The post holder will participate in the second on call rota, including out of hours on a 1 in 8 full shift with prospective cover for annual, study & statutory days leave. The post holder will be expected to attend & actively contribute to educational & governance activities within the department.

The Department of Medicine for the Elderly has evolved and developed with many innovations in Care of the Elderly services over the past 20 years. In 1993 the acute unit for Care of the Elderly integrated with Acute General Medicine to form a single department. As part of that process an emergency admission ward was developed to accept and manage all medical emergencies. Patients are subsequently transferred to the ward judged to be most appropriate for their clinical needs.
A fully functioning 13 bedded acute stroke unit with 24/7 thrombolysis facility is operational and a 5 days a week TIA clinic developed. There is a thrombectomy service and protocol linked to a centre which operates 7 days per week 9 till 5pm. The newly approved Stroke Business Plan, which is Health Board wide, will ensure stroke services are fit for the future and ensure compliance with Royal College recommendations and BASP standards of care. It will provide more service resilience and sustainability.
The Care of the Elderly services have had a strong history of strategic thinking initially developed through the health care planning team and now being developed through an Elderly Care Forum to respond to the strategy for old age. This successful candidate will work with the current Stroke consultant to form a team of two consultants. The successful candidate will be supported by a team of junior doctors including a Specialist registrar, F2 and F1 doctors.

The Team is consist of 4 gastroenterologists, 1 Gastroenterology Specialist Registrar, and 4-6 Junior Doctors. The department also has 2 IBD Nursing Specialists, 3 Liver Nurse Specialists, 2 Nutritional Nurse and 2 Gastro Nurse Specialists/Nurse Endoscopists. The Gastroenterology inpatients are predominantly on one Ward which has 27 beds.The Gastroenterologist on the ward sees referrals from other specialties and takes over care as needed. There is shared care of complex inflammatory bowel disease patients with the surgeons.
The Nutrition Team manages both enteral and parenteral nutrition with two Clinical Nurse Specialists, Specialist Dieticians and input from the Consultant Chemical Pathology. They have a number of patients on Home Parenteral Nutrition. There is a Dietician led coeliac clinic. The Endoscopy Department is supported by 4 Gastroenterologists, 2 upper GI Surgeons who provide the Upper GI Cancer Centre service for North Wales, 4 Colorectal Surgeons and 3 Nurse Endoscopists. The team is able to offer a full range of diagnostic and therapeutic endoscopy services along with bowel screening service.
The appointee will be expected to bring innovation and vision to develop the service to ensure it is fit for the future needs of the population we serve whilst ensuring that current service provision continues to be high-value and delivered to the highest standard. The post holder is expected to participate in training for CMT trainees. There is a weekly teaching session to cover the College GIM curriculum, and small group teaching for the Membership is arranged as required.

The Department has a Cardiac Centre that was created in 2014 with the opening of the second catheter laboratory one of the NHS' sites. The Centre was formed to provide tertiary cardiac services for the population of North Wales. The team is consist of 4 Consultant Cardiologists, 1 Associate Specialist, 2 Specialist Registrars, 2 Foundation Year 2 and 2 Foundation Year 1. There is a very well developed Heart Failure Team both in ‘primary’ and ‘secondary’ care supported by 5 Heart Failure Specialist Nurses (including palliate care) and a Specialist Pharmacist.
The Inpatient cardiology patients are located on ACU and CCU. There is a cardiology rota to see referrals from the general medical on–call service and for CCU. The current cardiology consultants are on the general medical on call rota, while all new referrals are seen in the cardiology department in one-stop clinics. Follow ups are seen in main out patients. The service provides echocardiography, device follow-up, treadmill testing and 24 hour and long term monitoring. The echocardiography department undertakes trans-thoracic, trans- oesophageal echocardiography and stress echocardiography.
The post holder will provide a comprehensive inpatient and outpatient cardiology service supported by other members of the department. The post holder is expected to provide professional supervision and teaching of junior medical staff and contributions to local undergraduate and postgraduate activity.
