WHAT IS THE DEPARTMENT & TEAM LIKE?
The NHS Trust supports 2 Emergency Departments (1 each at their 2 main hospitals). The Emergency Department at the first site is a designated Trauma Unit A, with a total of over 120,000 new patients attended in 2018/2019. In addition to that, it has a dedicated radiology room supporting PACS. The ED has 18 cubicles (4minors, 2 paediatric, 12 majors 1 ENT/Ophthalmology with a slit lamp, operating theatre with theatre lighting) and a resuscitation area supporting 4 bays.
The Emergency Department at the second site has a co-located radiology room supporting. The ED has 19 cubicles (3 minors, 1 ENP room and 15 major’s spaces) and a resuscitation area supporting 4 adult trolleys and 2 paediatric bays. The resuscitation area for Conquest Hospital has recently been upgraded. Each bay has new monitoring equipment with end tidal CO2, its own ventilator and defibrillator. The resuscitation area at Eastbourne General Hospital has monitoring with end tidal CO2, its own ventilator and defibrillator at each bay.
The specialty doctors, together with the two registrars provide a full 24 hr middle grade rota. The Specialty Doctor will be responsible for the primary management of patients attending the department and for supervising and assisting the more junior grades of doctor. The specialty doctor will also be expected to lead the trauma team and assist the cardiac arrest team if required.

What is the NHS Trust like?
This Trust was formed through the merger of two previous NHS Trusts which took place in April 2020. It serves a population of around 270,000 across the region and surrounding areas, with a catchment of 900,000 for vascular services, 24 of which are maternity, 16 are paediatric, ten are critical care and 28 are day-case beds. Its core local authority populations are around 160,000 and 260,000.
The hospital has over 2500 members of staff, making it the largest local employer in the area, and a current turnover of approximately £205m. The hospital enjoys strong community support with over 200 volunteers at the hospital and over £6 million being raised by the it\'s independent charity over the past 10 years. The support from the charity has enabled the hospital to build a dedicated cancer centre and it has recently funded a new MRI scanner, operational since early 2018.
The main hospital was erected on the site of a General Infirmary. The Infirmary was originally opened in 1803 providing accommodation for 38 patients. Now, the hospital has approximately 400 beds.
What is the Oncology Department like?
The new consultant will be joining a friendly and forawrd-thinking Oncology Department which includes 8 consultants, covering a variety of specialities. There are currently no Junior Doctors in place, although this may change in the future with expansion of training posts. Currently, support to Consultants is provided by Chemo Nurse Practitioners as well as AOS Nurses.
The cancer centre houses the hospital’s outpatient oncology service for cancer patients and new and follow-up patients are seen in one of the several clinics each week. Chemotherapy, monoclonal antibody therapy and other anti-neoplastic agents are administered here daily by one of their specialist chemotherapy nurses, and supported by a phlebotomist in the unit. The chemotherapy suite offers daily sessions, for oncology patients and shared care for haematology patients.
The cancer centre also has its own phlebotomy service, cancer information and support centre as well as a palliative care team. The unit provides a PICC line service and 24hr telephone support line for chemotherapy patients.
The post holder will be responsible to the Medical Director & the Clinical Director, and is required to prepare for and undertake the protective interventions that he/she is responsible for in a manner that is consistent with evidence based practice and maintaining patient, staff and other users safety.

This job can be based at 2 Cancer Treatment Centres, providing radiotherapy treatment and specialist chemotherapy for the population of North Wales, (676,000), excluding paediatric patients and those needing brachytherapy. It includes a 20-bed in-patient ward, a spacious day-case systemic anti-cancer therapy (SACT) unit (Heulwen) with facilities to treat up to 24 patients simultaneously, outpatient department with 10 clinical suites and radiotherapy review facilities
The medical staff includes: 7 substantive clinical oncologists, 1 locum clinical oncologist, 2 substantive medical oncologists, 1 locum medical oncologist and 1 vacant clinical oncology post.
The Radiotherapy Department has 4 Varian Linear Accelerators, a Gulmay Orthovoltage Unit, Phillips CT Planning Scanner, Prosoma, and an Eclipse Treatment Planning System supported by Limbus automated outlining. IMRT techniques are routine for most treatment sites with the addition of 4DCT and the centre is also actively working towards the implementation of SABR for lung.
Parenteral SACT is prepared in validated Pharmacy Aseptic facilities located within the acute hospital sites. Immunotherapy is given across all sites, with a newly-established, nurse-led immunotherapy toxicity service. The SACT service received a successful outcome of its first Peer Review in 2020.

WHAT DOES THE INPATIENT DEPARTMENT LOOK LIKE?
The wards admit a mixture of Informal and detained patients. Detained patients account for 60% of patients on the wards. Delayed transfer of care patients account for 10% of the wards population. The wards admit adults with various psychiatric disorder and majority suffer with severe enduring mental illness. Patients with personality disorder tend to account for 40% of the wards population and they tend to have the below average length of stay. The post holder is expected to support patient flow team in their effort to facilitate discharges from the ward. The current length of stay (LOS) on our acute ward is 50 days but we are working reducing this to 25 days.

The Trust is excited to be developing a Hyper-Acute Stroke Unit (HASU) at the Cumberland Infirmary site which will comprise a 10 bedded HAS unit at CIC, a further 25 acute and rehabilitation beds, and a further 15 acute and rehabilitation beds at WCH. If you successful, you will join the newly forming team and have the opportunity to contribute to this exciting development for our patients in both the Cumberland Infirmary and the West Cumberland Hospital. You would also be instrumental in the establishment of a high quality stroke service across North Cumbria. The planned 7-day service model will allow the delivery of 7-day stroke / TIA clinics which will rapidly speed up diagnosis and therapeutic intervention, and will provide an integrated pathway into community services.
This development was implemented in the last few years, with the new service becoming increasingly embedded into Integrated Care Communities (ICCs) specialist ESSD rehabilitation that is delivered at local level under the joint management of the ICC and the ESSD coordination team.

WHAT IS HEALTH BOARD LIKE?
The Health Board is largest health organisation in Wales, that are developing an integrated health service which provides excellent care delivered in partnership with the public and other statutory and third sector organisations.
The Health Board have a budget of £1.3 billion and a workforce of over 17,000 staff. Providing primary, community, mental health and acute hospital services for the population of around 700,000 across of North Wales. They continuously develop their workforce so that it has the right skills and operates in a research-rich learning culture.
Besides their three main hospital sites, they are also responsible for community hospitals, health centres, clinics, mental health units, community teams, GP practices and other NHS services provided by dentists, opticians and pharmacists. They plan to improve the health of the population, with particular focus upon the most vulnerable in our society.
WHAT IS THE DEPARTMENT & TEAM LIKE?
The Department provides a service to approximately 240,000 people in a predominantly rural area covering 1,600 sq. miles. The service consists of Acute Medical Unit with 23 beds, short stay ward with 6 beds, triage unit with 5 trolleys, ambulatory Emergency Care Unit with 2 trolleys and 2 chairs, procedure unit with 3 trolleys and 4 chairs. Patients are referred by directly by General Practitioner and the Emergency Department. The Acute Medical Service sees just over 1000 patients per month.
The Department has 2 partner services - the Cardiac Services, which has a modern 6 bedded unit and has the facility to remotely monitor 6 beds on the Acute Medical Admissions Unit, 4 on the Acute Medicine ward and a further 2 in the Emergency Department, and the Emergency Department, which is one of the best staffed units in the UK and has got a full complement of (non-locum!) consultants and middle grades. The pre-hospital fellow program attracts candidates from all over the UK.
The post holder will be required to regularly participate in the medical audit and continuing medical education and professional development as part of the organisational clinical governance requirement. The post holder responsibilities include ward rounds on the AMU (8am to 4pm): Review of up to 30 patients in the Acute Medical Unit including ongoing management of patients in the triage area and early review of emergency presentations.

The department currently have three full-time urological surgeons, a Locum Consultant along with 5 middle grade registrar level training including 1 SpR from the Mersey Deanery training programme. There are two SHO level staff and one FY1 and FY2. The Department has recently undergone a refurbishment, to provide day-case urology and therapeutic services to include TRUS-Guided prostate biopsy lists, an endoscopy suite, dedicated lithotripsy sessions and specialist out-patient clinics. The department has inpatient beds based on the 3rd floor within the Surgical Department. Paediatric beds are also available on the main hospital site.
The department uses one of the two dedicated purpose built modern integrated laparoscopic surgery theatres for its heavy laparoscopic workload. Currently, major laparoscopic Nephrectomies and prostatectomies are performed in this OR. A 12 bedded Intensive Care/High Dependency Unit is located on the first floor of the hospital. There are excellent pathology facilities, including a Public Health Laboratory. The Radiology Department offers a full range of facilities, including CT, MRI and fully functional intervention radiology service.
The post holder will provide elective and emergency inpatient services, along with other members of the department. The post holder will also participate actively in the Department’s Clinical Audit Programme and Continuing Professional Development, which is fully supported by the Health Board.
