An NHS hospital in South East England is looking for a Consultant in Medical Oncology with an interest in Lung Cancer and Melanoma (or another tumour type such as Upper GI, colorectal, or urology). The Oncology department comprises a team of experienced Clinical Oncologists and a Consultant Radiographer, supported by a dedicated team of middle-grade doctors. This Medical Oncology job is open to both international applications and UK-based doctors, with preference given to those with a GMC Medical Oncology Specialist Register, CCT, or CESR.

Full GMC registration with a license to practice and entry on the GMC Medical Oncology Specialist Register (or within six months at the time of interview), CESR, or a GMC-recognised equivalent training are required for this role. WHAT IS THE ONCOLOGY DEPARTMENT & TEAM LIKE?

The Oncology Department operates within the Cancer Centre Directorate, which is a part of the Planned Care Group at the Hospital. As a Cancer Centre, the hospital actively participates in its regional Cancer Alliance. The Consultant team consists of two medical oncologists (soon to be three) and nine clinical oncologists who are primarily stationed at the Cancer Centre in the Hospital. They conduct outpatient sessions and chemotherapy clinics in various nearby locations.

The service participates in multidisciplinary team meetings. The Trust has a dedicated Acute Oncology Service led by one of the Medical Oncologists, with support from senior Clinical Nurse Specialists. In addition, there is a shared Oncology and Haematology ward with 20+ beds. Inpatient care for oncology patients is managed by the appropriate medical or surgical teams, with regular input from the Acute Oncology and specialist oncology teams.

The post holder will share clinical and managerial leadership within the Oncology department with the present consultants. The post holder will be provided with a designated office base with full secretarial and administrative support. The Child and Adolescent Psychiatry Team is a small cohesive community based multi-disciplinary team. They are a team of outstanding doctors working together to provide the best service their patients can have. The team offers assessments and treatments to a wide variety of clinical presentations. They provide Early Intervention Service for the 13 to 18-year-old as well as assessment and treatment of young people presenting with high risk and severe life threatening self-harm. The Adolescent Mental Health Team is composed of a Child & Adolescent Psychiatry (joint consultant team-leads, this post and higher trainee), Systemic Family Therapy, Clinical Psychology, Child & Adolescent Psychotherapy and Mental Health Nursing.

The team model includes joint working between Psychiatry and allocated care-coordinator as well as individual working with young people and their families to provide developmentally informed evidence-based treatments. The Team offers neurodevelopmental assessments including ADHD, learning disability and Autism. Also to offer psycho-education and feedback to patients and their carers; and commence and/or monitor appropriate medication under supervision. 

The post holder will provide assessments and treatment of young people presenting with severe and enduring mental health difficulties as part of a multi-disciplinary team assessment. The Service is divided into four Teams, and there is also a Central County Wide Memory Assessment service (which undertakes much of the MAS work) and also a County Wide Intensive Dementia Support Service IDSS (which acts as a Crisis/Home Treatment Team for those with Dementia). The community mental health teams and memory services provide psychiatric community care to patients over the age of 65. There is also a much smaller group of patients under 65 years of age, with primary dementias who are also cared for by the older adults’ community mental health team.

The multi disciplinary team includes medical, nursing, Psychology, OT with clinical support workers and admin support staff. The overall team provides CMHT, MAS and Specialised Day Hospital services. The two services area covers GP practices in four towns and equates to approximately 11,000 patients over the age of 65 (CCG figures December 2017). The community mental health teams are part of wider integrated teams. Each team is co located with staff from district nursing and the integrated rehabilitation team (community OT and Physiotherapists).

The post holder will hold regular clinics, as well as undertaking domiciliary visits when required. The post holder will provide assessment, diagnosis and treatment of mental health needs of community patients in their community area, and work closely with the community teams to provide effective multidisciplinary collaborative care: this will include availability for consultation, regularly attending team meetings and providing case discussion and advice at the request of team members and / or team managers.

WHAT IS THE DEPARTMENT & TEAM LIKE? 

The Community Older Persons Mental Health Service is divided into four Teams, and there is also a Central County Wide Memory Assessment service (which undertakes much of the MAS work) and also a County Wide Intensive Dementia Support Service IDSS (which acts as a Crisis/Home Treatment Team for those with Dementia). The community mental health teams and memory services provide psychiatric community care to patients over the age of 65. There is also a much smaller group of patients under 65 years of age, with primary dementias who are also cared for by the older adults’ community mental health team.

There is an emphasis on members of the CMHT seeing the majority of initial assessments with a day time duty rota dealing with urgent referrals. This helps to minimise consultant emergency work and keeps their individual community caseload at a manageable size. There are approximately 200 CMHT referrals each year, and approx. 150 Memory Assessment Service referrals in Exmoor and Minehead. With the increase in primary care diagnosis of dementia the number of memory assessment service referrals is expected to reduce. 

The post holder will provide assessment, diagnosis and treatment of mental health needs of community patients in their community area, and work closely with the community teams to provide effective multidisciplinary collaborative care: this will include availability for consultation, regularly attending team meetings and providing case discussion and advice at the request of team members and / or team managers.

WHAT IS THE GENERAL ADULT PSYCHIATRY - INPATIENT DEPARTMENT & TEAM LIKE?

The Ward is a 14 Bed, mixed gender assessment ward based in Worcester. The ward takes all admissions from the community for patients in acute crisis. The ethos of the ward is dynamic assessment, allowing staff to work on identifying, and safely managing patient’s needs. The ward\'s role is to formulate care plans, assess risk and plan each patient’s progression over a 14 day period.

The team works closely together, especially on clinical maters with the inpatient Multi-Discliplinary consisting of 1 Ward Manager, 3 Deputy Ward Managers, 1 part time Psychologist, Nursing Staff, Healthcare Assistants, 1 Occupational Therapist, Support Workers, and Peer Support Workers based in the ward.

WHAT IS THE DEPARTMENT & TEAM LIKE? 
The Team prioritises continuity of care and accepts referrals for patients with both functional and organic presentations. Patients can choose to be seen in a number of settings including their own home, a modern accessible clinic or in their GP surgery. The service offers a comprehensive programme of group and individual therapeutic interventions to cater for different levels of need. There is a separate and specialist care home pathway with HCPs to assess and manage behaviour that challenges; including joint GP/OAP wards rounds & detailed formulations using the Newcastle model. There is additional support from separate and specialist YOD Occupational Therapists for patients with Young Onset Dementia.

The post holder will be responsible for delivering consultant psychiatric input to patients referred (excluding care homes) to and accepted for treatment by the CMHTOP in line with the agreed criteria and commissioned care pathways.

What is the Pathology Group like? 

The group comprises four pathology laboratories providing pathology services for acute hospitals and also local GPs. Some of their laboratories also offer specialist services to the wider NHS and work on research studies and trials. As part of a major initiative in the NHS, Pathology in England is being reorganised into much larger local network models and this sees the formation of 29 pathology units across the country.
