HIGHLY RATED OLD AGE DEPARTMENT IN LONDON
The department is part of an NHS Trust, which was awarded the Community-Based Mental Health Services for Older People a “Good” rating. The NHS Trust is one of the most diverse healthcare providers in the UK, delivering a range of mental health, physical healthcare and community services commissioned locally and nationally. The Older Persons Mental Health Service encompasses a specialist service for patients with complex, severe and enduring functional mental illness over age 70, as well as the Cognitive Impairment and Dementia Service which offers an ageless assessment and management services for people with cognitive impairment and primary dementias, including both diagnosis and management of BPSD. Where patients with a severe and enduring mental illness develop a dementia, they will be jointly managed to offer a holistic care and management plan. The service also provides support to patients in residential or nursing care. They work closely with other specialist and intermediate care services to provide as seamless a service as possible for their patient’s health and care needs. The service will have an overall caseload of approximately 110 patients. The referral rate is approximately 3 cases per week and the post holder will be involved with the diagnosis, ongoing management and crisis support of the patients.
The post holder will take a leading role in developing the service, supervising junior colleagues and providing lead medical input to a multi- disciplinary team and be required to enter information on the Trust\'s electronic patient record system according to Trust policies. The post holder is expected to work within the multidisciplinary team promoting respect for all professional groups and focusing on coherent and integrated team work.

The Department provides specialist healthcare to adults who have a diagnosed learning disability. The team is committed to providing evidence-based, safe, and effective care that is centred on each person\'s individual needs and delivers clear outcomes. The service they provide include specialist assessment and support for people with complex and challenging needs, enabling people to access a range of services to keep them healthy, and a range of initiatives to reduce and overcome the health inequalities that can be experienced by people with learning disabilities.
The team works closely with a range of partners, including social care, GPs, voluntary sector, and user-led organisation. They also work with other health professional across the health board to ensure services are accessible for people with a learning disability and that we provide whole person care. The department consists of learning disability nurses, occupational therapist, physiotherapist, speech and language therapist, psychologist, psychiatrist, behaviour practitioners and social workers.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The integrated department of medicine provides inpatient, outpatient and occasional domiciliary consultation services for medical patients of all ages. In-patient and rehabilitation facilities are provided at the main hospital site. There are many acute beds provided for the directorate including a dedicated purpose built 6-bedded coronary care unit and 24 bedded Clinical Decisions Unit. There is also a combined HDU/ITU with a total of 14 beds.
The department is supported by full pathology and radiology services. In addition, the department of physiological medicine carries out electrocardiography, echocardiography, stress testing, Holter monitoring and lung function testing. The biochemistry department works closely with the diabetes department. Physicians with an interest in Elderly Medicine provide a comprehensive service for the elderly in addition to contributing to the acute medical service for all adult age groups.
The post holder’s specific responsibilities would be:
- Acute medical take on a 1:10 rota. Patients are cared for by on-call teams and acute physicians until their care is transferred to the wards where ward based teams care for patients.
- Dementia/frailty ward.
- 20 bedded stroke ward with ASU and rehab beds.
- The geriatric medicine team delivers a front door and community frailty service.
- The development of a Same Day Emergency Care unit will include Frailty pathways
- Very occasionally domiciliary visits are carried out at the request of GPs.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The Team is a group of excellent & hardworking doctors that provides service in a town with a population of 100,000 with the 16-65 population of 61,000. There are two full time Consultants covering the Community Mental Health Team (this post being one of them). The Community Mental Health team comprises: Operational team managers, Service manager, CPNs, STR Workers, Non-medical prescriber, psychological therapists, and admin staff.
The community mental health team accept referrals from varies agencies including General practitioners, IAPT services, secondary and tertiary agencies. The current caseload is approx. 650 with a referral rate of approximately 4-6 patients per day. Inpatient services are provided in a new purpose-built 40-bedded unit four miles from the team base. A dedicated inpatient consultant psychiatrist and related team provide care for inpatients. The team is also supported by a CRHT service, which deals with all crisis referrals from 9am to 9pm with an on-call service thereafter.
The post holder will have access to dedicated admin with a ratio of 3:1, with consultants also having full access to a secretarial pool for all preparation of letters and reports etc. The post holder is expected to manage, appraise and give professional supervision to junior medical staff as agreed between consultant colleagues and the medical director and in accordance with the Trust’s personnel policies and procedures. The post holder needs to ensure that junior medical staff working with the post holder operate within the parameters of the New Deal and are Working Time Directive compliant.

WHAT IS THE DEPARTMENT & TEAM LIKE?
It is a truly exciting time to be joining the Child & Adolescent Mental Health Service as they are moving towards a county-wide service with clinics at localities. The department provides an assessment and treatment service for young people from the age of 0 – 18 years. The service has an expanded primary mental health staff and has recently been funded to start the Trailblazers project within schools and to support existing clinical services. The average number of sessions for treatment completion and discharge is between 8 and 10
CAMHS is committed to the THRIVE Framework which provides a set of principles for creating coherent and resource-efficient communities of mental health and wellbeing support for children, young people and families. The Framework is needs-led. This means that mental health needs are defined by children, young people and families alongside professionals through shared decision making. Needs are not based on severity, diagnosis or health care pathways.
The post holder will be expected to maintain the close working relationships with community paediatricians. Regular liaison meetings occur which facilitate coordinated input to co- morbid and complex cases. WHAT IS THE DEPARTMENT & TEAM LIKE?
The Department is an established high-quality multidisciplinary community & liaison Perinatal Mental Health Service across five Boroughs. The service specification is defined in line with the standards recommended by the RCPsych CR197, the Perinatal Quality Network and the Pan London Perinatal Mental Health Network. In the area served by the NHS Foundation Trust, there are four Maternity Units with a projected delivery rate of more than 6,000/year. In addition, there are approximately 4,000 deliveries in the private sector. The team have the capacity of enabling the delivery of evidence-based perinatal mental health care through a coordinated multidisciplinary specialist approach across maternity, primary and secondary care, tertiary specialist services, social care and voluntary sector.
The service operates at the interface between one of the Trust\'s hospital maternity unit and psychiatry with an outreach arm in the community in the form of home visits, satellite clinics, joint case management with caseload midwifery and a perinatal mental health specialist Health Visitor and NGOs. The service works closely with the liaison psychiatric team with clinical cross cover arrangements for annual and study leave. The Team Consists of: 0.5 WTE team manager, 0.5 WTE consultant psychiatrist in perinatal, 1 WTE specialty doctor (post advertised), 1 WTE Medical secretary, Band 4, 1 WTE Band 8a Psychologist, 0.8 WTE Band 8a Psychotherapist 7, 3 WTE band 7 specialist psychiatric Nurses, 1 WTE Band 6 OT, 0.6 WTE band 7 social worker, 0.6 WTE nursery nurse, and 0.6 WE peer support worker.
The post-holder will work with a medical team consisting of a Consultant Psychiatrist. The post-holder will contribute to the supervision of the trainees/medical students, clinical discussion and support for other non-medical MDT members and is expected to contribute and write perinatal care plans, safeguarding reports for social care and attend pre-birth planning meetings, discharge from maternity or MBU meetings and children and families safeguarding meetings.

This established CAMHS MDT is a long established specialist CAMHS service with an excellent reputation for innovation as well as established therapies. The CAMHS service also offers embedded clinical sessions to partner agencies in London Borough. Through this we are able to offer consultation and direct work to mainstream schools, Special Schools and Pupil Referral Units. This service also extends to Children’s social care with clinicians embedded in Assessment and Intervention Teams, Children’s Centres, Youth Offending teams and Looked After Children’s Teams. The CAMHS service is also complemented by a CAMHS specialist eating disorder ‘hub and spoke’ service - the Community Eating Disorders Service for Children & Young People (CEDS-CYP) and an extended crisis service (11 am – 11 pm) across three London Boroughs.

WHAT IS THE GENERAL ADULT PSYCHIATRY DEPARTMENT & TEAM LIKE?
The Department is a modern 17-bedded en-suite unit which comprises doctors, qualified psychiatric nurses, occupational therapist, psychologist and health care assistants, providing assessment and treatment predominately for older people with complex or acute mental health needs, enabling a return to independent living wherever possible. The team accommodates all Older Adult services, including CMHT, Home Treatment Service and Memory Service. On average, there are approximately four new admissions and approximately four discharges per month. The majority of patients are admitted from their usual place of residence with a small percentage coming from the general hospital.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The Old Age Psychiatry Team (Memory Assessment and Management Service) provides assessment and initial diagnosis to patients of all ages where there is a suspected dementia. It operates across Newcastle and North-Tyneside. The team works closely with the older adults Community Treatment Teams (CTT), which covers Newcastle North and East; Newcastle West; and North Tyneside. The department includes a specialist Dementia. The team receives approximately 100 referrals per month for patients who are low-complex, and where there is a high likelihood of a dementia diagnosis. Most new referrals are seen by nurses, discussed at the daily MDT meeting, and then offered a diagnostic appointment with a doctor, senior nurse or psychologist.
The Young Onset Dementia Team provides specialist follow up for people living with dementia aged under 65. It is a well-established multi-disciplinary team within Older Adults Community Services, and the sessions allocated provide opportunities for specialist assessment and support for this group of patients, and the chance to develop greater integration with the Old Age Psychiatry Team. Most appointments will be conducted in the out-patient setting, but home visits are also offered for patients based on their clinical need. Referrals are reviewed at a daily triage meeting and new assessments are allocated to the most appropriate member of staff.
The post holder will be expected to see more complex new patients (approximately 1 per week), conduct diagnostic appointments (approximately 10 per week) and to lead the MDT meeting (2 per week). The post-holder is expected to be committed to delivering excellence in clinical care, provide medical leadership to and work effectively with the multidisciplinary MAMS service and Young Onset Dementia teams, promote the safety and well-being of the patients we serve, and to respect the rights and the dignity of patients.

The Child and Adolescent Psychiatry Team is a small cohesive community based multi-disciplinary team. They are a team of outstanding doctors working together to provide the best service their patients can have. The team offers assessments and treatments to a wide variety of clinical presentations. They provide Early Intervention Service for the 13 to 18-year-old as well as assessment and treatment of young people presenting with high risk and severe life threatening self-harm. The Adolescent Mental Health Team is composed of a Child & Adolescent Psychiatry (joint consultant team-leads, this post and higher trainee), Systemic Family Therapy, Clinical Psychology, Child & Adolescent Psychotherapy and Mental Health Nursing.
The team model includes joint working between Psychiatry and allocated care-coordinator as well as individual working with young people and their families to provide developmentally informed evidence-based treatments. The Team offers neurodevelopmental assessments including ADHD, learning disability and Autism. Also to offer psycho-education and feedback to patients and their carers; and commence and/or monitor appropriate medication under supervision.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The Team is a group of collaborative consultants who always makes sure that their patients get the best service. The department is well established with good support from senior clinical and managerial colleagues. They accept referrals from the SPA, Primary Care Liaison Team, Crisis Resolution Team, Inpatient wards, Child and Adolescent Mental Health Team as well as national Early Intervention services for service users with a first episode of psychosis. It normally works with service users for 3 years from age of 14 upwards. It functions between 9am and 5pm between Monday and Friday.
The department works in conjunction with other local mental health teams, and has developed close links with a range of community and youth services. This specialist multidisciplinary, community based team, provides intensive input to young people between the ages of 14 and 35 plus, who have experienced a first episode of psychosis. In the future it is anticipated that the service will extend the hours of operation from 8am to 8pm seven days a week. The team has a Key Performance target to review all new referrals within 2 weeks and meet the level 3 standard for Early Intervention services.
The post holder is expected to be responsible for carrying out detailed psychiatric assessments and risk assessments and to contribute to planning and implementing service users care plans, under CPA procedures and guidelines. The post holder is expected to carry out the above in a variety of settings, including home visits, and other non-stigmatising settings. The successful candidate is expected to work flexibly to ensure that clients and their carers are able to access the service effectively.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The service (generally for people aged 16 to 64) has been running since 2008, according to a model whereby their team of consultants are deployed as follows: three consultants work exclusively in the community and outpatient areas, one consultant works with the Early Intervention Service, two consultants cover the inpatient area and one works with our Home Treatment and A&E Liaison Team. Within this arrangement, the consultant appointed to this post will work with other sector consultants (East, West and South sector community outpatients).
