WHAT IS THE NHS FOUNDATION TRUST LIKE?
The NHS Foundation Trust teaching hospital full of hard working and dedicated staff providing community and hospital services across the area that covers a thousand square miles. The NHS Foundation Trust currently has 6,000 employees, and and provides services for some 350,000. They operate three main hospitals, as well as a number of community healthcare premises. Two of their main hospitals have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The Acute Medical division is comprised of a 39 bedded Acute Medical Unit and an adjoining Ambulatory Care unit that sees and manages up to 30 patients per day. The team consists of the following a Substantive Consultant (Clinical Lead) – Acute and General Internal Medicine Substantive Consultant - Acute Medicine and Geriatrics, Substantive Consultant – Intensive Care and Acute Medicine, Substantive Associate Specialist, Advanced Nurse Practitioner, Foundation Year Doctors (two), Acute Care Common Stem Trainee & Trust Grade Clinicians (two) – Ambulatory Care General Practice Trainee – Ambulatory Care.
The successful appointee will be expected to provide care for acutely unwell patients on the AMU, contribute to service development and quality improvement, provide care for patients seen on the ACU and provide leadership and work as part of and develop the team.

The Health Board have a budget of £1.3 billion and a workforce of over 17,000 staff. Providing primary, community, mental health and acute hospital services for the population of around 700,000 across of North Wales. They plan to improve the health of the population, with particular focus upon the most vulnerable in our society.
WHAT IS THE DEPARTMENT & TEAM LIKE?
The Department provides a service to approximately 240,000 people in a predominantly rural area covering 1,600 sq. miles. The service consists of Acute Medical Unit with 23 beds, short stay ward with 6 beds, triage unit with 5 trolleys, ambulatory Emergency Care Unit with 2 trolleys and 2 chairs, procedure unit with 3 trolleys and 4 chairs. Patients are referred by directly by General Practitioner and the Emergency Department. The Acute Medical Service sees just over 1000 patients per month.
The Department has 2 partner services - the Cardiac Services, which has a modern 6 bedded unit and has the facility to remotely monitor 6 beds on the Acute Medical Admissions Unit, 4 on the Acute Medicine ward and a further 2 in the Emergency Department, and the Emergency Department, which is one of the best staffed units in the UK and has got a full complement of (non-locum!) consultants and middle grades. The pre-hospital fellow program attracts candidates from all over the UK.
The post holder will be required to regularly participate in the medical audit and continuing medical education and professional development as part of the organisational clinical governance requirement. The post holder responsibilities include ward rounds on the AMU (8am to 4pm): Review of up to 30 patients in the Acute Medical Unit including ongoing management of patients in the triage area and early review of emergency presentations.

The Department of Medicine for the Elderly has evolved and developed with many innovations in Care of the Elderly services over the past 20 years. In 1993 the acute unit for Care of the Elderly integrated with Acute General Medicine to form a single department. As part of that process an emergency admission ward was developed to accept and manage all medical emergencies. Patients are subsequently transferred to the ward judged to be most appropriate for their clinical needs.
A fully functioning 13 bedded acute stroke unit with 24/7 thrombolysis facility is operational and a 5 days a week TIA clinic developed. There is a thrombectomy service and protocol linked to a centre which operates 7 days per week 9 till 5pm. The newly approved Stroke Business Plan, which is Health Board wide, will ensure stroke services are fit for the future and ensure compliance with Royal College recommendations and BASP standards of care. It will provide more service resilience and sustainability.
The Care of the Elderly services have had a strong history of strategic thinking initially developed through the health care planning team and now being developed through an Elderly Care Forum to respond to the strategy for old age. This successful candidate will work with the current Stroke consultant to form a team of two consultants. The successful candidate will be supported by a team of junior doctors including a Specialist registrar, F2 and F1 doctors.

WHAT IS THE NHS FOUNDATION TRUST LIKE?
They were established as an NHS Trust in April 1991, and became a Foundation Trust in April 2008. Since then, the Trust provides a good quality, safe, and effective healthcare to the people of several counties. The Trust manages three different hospitals/centres. The Trust is currently responsible for the provision of a range of community services for people across several areas, working in partnership with local GPs and another NHS Trust.
The NHS Trust employs almost 5,000 members of staff, have 540 hospital beds, with a range of services including accident and emergency, maternity, outpatients, therapies, and children's health. The Trust established a campaign that encourages all staff, regardless of their role, to introduce themselves to the patients and visitors whom they come in contact with and they feel strongly that they should support the campaign to ensure this basic human touch is adopted by all.

WHAT IS THE DEPARTMENT & TEAM LIKE?
The Department is a team of people who consistently provide a caring, professional, high quality, efficient clinical service for patients who present with an acute medical condition to secondary care from the community, Emergency Department or following a clinic attendance. They values safety, respect & efficiency - which they believe will empower staff to deliver a high quality service.
The Department currently has three distinct geographical areas that work together to provide Acute Care:
- AMU (Non-Covid) Houses, that currently has 32 beds and is the direct admissions area for ED and GP referrals that are not suitable for the Ambulatory Care Unit. There is a dedicated ward pharmacy team and there is access to Mental Health Liaison, physio and occupational therapy and a complex discharge support team.
- AMU (Covid), that has 22 side rooms for patients who present with symptoms that may be Covid, and 6 side rooms that can support confirmed Covid positive cases. There is a dedicated pharmacy and therapy support.
- ACU, which is located in a dedicated outpatient unit situated halfway between AMU (Covid) & AMU (Non-Covid). It currently has assessment trolleys, and chairs allowing it to house up to 18 patients. The Consultant is supported by an Advanced Nurse Practitioner team and Physician Associates.
The successful candidate will join a team of 4WTE substantive Consultants on a fixed term basis. The post holder is expected to ensure that management plans are clearly displayed in the notes of patients with complex needs, liaise closely with the Divisional and Service Managers, and the Associate Medical Director, to ensure that targets are achieved and other things.

The Trust supports the requirements for continuing education as laid down by the Royal College and is committed to providing time and financial support for these activities. In addition, the post holders will be required to attend mandatory and essential training in line with Trust policy. Their ultimate aim is to improve the quality of health care for the population covered by the local health care services, by PGMDE (which focuses on training of junior staff) and CME (which targets senior staff both Consultant and non-Consultant career grade staff).

The Health Board is largest health organisation in Wales, that are developing an integrated health service which provides excellent care delivered in partnership with the public and other statutory and third sector organisations.

Providing primary, community, mental health and acute hospital services for the population of around 700,000 across of North Wales. Besides their three main hospital sites, they are also responsible for community hospitals, health centres, clinics, mental health units, community teams, GP practices and other NHS services provided by dentists, opticians and pharmacists. WHAT IS THE DEPARTMENT & TEAM LIKE?
Acute medicine in the hospital is changing. The department currently has 3 substantive acute physicians and 2 long-term locums, with subspecialty interests in Respiratory Medicine, Nephrology and Clinical Pharmacology. The acute medicine team is responsible for the medical intake 8am –5pm throughout the week, with support from the GIM rota in the evenings and at the weekends. There is a focus on ambulatory care with an ANP and clinical hub nurse in place to support.
Currently all acute admissions from primary care are received by the clinical hub nurse in hours and directed to either the AMU or ED (if likely to need the resus room). The majority of referrals come straight to AMU for assessment. The team is continuing to improve our care pathways for acute medical patients and further develop our services. The team is passionate about MDT working and work closely with their nursing and therapist colleagues.
Successful candidates will be responsible for providing senior leadership and management working alongside our trainees and other healthcare professionals. They will work between the ambulatory area, the trolley bay, and the short stay ward providing excellent clinical care for our acutely unwell patients. They will provide training and supervision for our staff and play a key role in this exciting service development.

An excellent opportunity for an experienced Acute Medicine Consultant to join an NHS Trust in the South West of England. WHAT IS THIS UNIVERSITY HOSPITALS NHS FOUNDATION TRUST LIKE?

The Trust focuses on delivering excellent general hospital services. They provide service to approximately a population of 750,000 with the help of more than 9,000 staff members.
The hospitals have approximately 1100 beds in total, and provide a broad range of services including General Medicine, General Surgery, Neurology, Rheumatology, Gastroenterology and Dermatology, as well as Gynaecology and Paediatrics.
The Trust achieved foundation status in 2005 and in October 2020, they created a new organisation by merging two of their hospitals with the goal to implement the establishment of emergency (RBH) and planned care sites (PGH).
WHAT IS THE ACUTE MEDICINE DEPARTMENT & TEAM LIKE?
The Team consists of Patient Support Workers, HCAs, Staff Nurses, a team of Deputy Sisters, two experienced Ward Sisters and four Advanced Nurse Practitioners (ANPs) that are enthusiastic, hard working and always working closely together.
The Team has approximately 20 patients per ward round. The current rota is annualised with an eight week rolling pattern. There are two Acute Physicians running the AMU and acute take 0800-2030 with SDEC Consultant cover 0900-1700daily during the week. The weekend frequency is currently 1 in 8. SPA is organised flexibly. The AMU at Poole Hospital is a 30 bedded assessment and short stay unit for general medical and acute medical patients. It receives approximately 8000-9000 patients per year.
The post holder will lead and run the AEC Clinic, alongside the ANPs, during their allocated clinical sessions. The post holder will provide teaching and training to junior doctors, nurse practitioners and allied professions within acute medicine. The post holder will be working at both sites and will have access to a fully-equipped Consultant’s office which includes all the required information technology.

THE GENERAL MEDICINE CONSULTANT POST:
The post offers broad medical experience across the field of medical specialities. The holder of this post will usually work within one speciality team, however may be asked to cover in other areas across general medicine at times in order to maintain a safe effective service.
There is a firm commitment to continuing professional development and the opportunity to develop further in a chosen sub-specialty.
THE HOSPITAL & GENERAL MEDICINE DEPARTMENTS:
Within General Medicine, there are currently 19 Consultants, of which 13 are substantively employed with special interests in Respiratory Medicine, Cardiology, Gastroenterology, Diabetes and Endocrinology, Acute Medicine, Stroke, Rheumatology and Care of the Elderly.
An integrated service in general medicine and care of the elderly has matured within the Hospital. The team of consultants are enthusiastic and forward thinking. We pride ourselves in being a small, friendly hospital, with real drive to improve both patient care and the working environment for our staff.
In addition to an acute in-patient and out-patient medical service, specialist diabetic and non-invasive cardiology services are provided.
Acute medical work is focused on 195 beds in eight wards. ITU/HDU facilities are based on five beds. The Trust is recognised as providing efficient and well-organised Cancer services. This has been supported by both Regional, and National Accreditation processes. As part of the Avon, Somerset and Wiltshire Cancer Network, the Trust provides a range of cancer services. Those services already accredited include, Breast Care and Urology.
THE LOCAL AREA:
North Somerset, the surrounding county, has long coastlines with superb cliff tops and English countryside. You'll find yourself visiting the Mendip Hills, the Quantocks and Cheddar Gorge sooner or later. The large, diverse cities of Bristol and Bath are only a short journey by car or train and our convenient location just off Junction 21 of the M5 mean the rest of Somerset, Devon and Cornwall are virtually on the doorstep.
