The Older Adults Community Mental Health Team (OACMHT) manages a caseload of about 300 patients, including new referrals triaged by the Crisis and Assessment team, providing both short-term treatment and long-term care coordination. The role involves providing medical support to the OACMHT alongside two other consultants and a staff grade doctor.
The post holder will join an established team, comprising 2 WTE older adult consultants, 1.0 WTE Associate Specialist (Memory Assessment Service), 2.0 WTE staff grades (inpatient and community), and 1.0 WTE F1 doctor (inpatients and community). Dedicated administrative support includes a 0.5 WTE medical secretary and ward and team secretarial support.

General Adult Psychiatry Department & Team
The inpatient service features 17 adult acute beds, including an extra-care area, for individuals aged 18 to 65 with severe mental illness requiring admission. The goal is to promote recovery and well-being, helping service users achieve their potential. The service caters to a diverse population with complex needs. The multidisciplinary inpatient team includes doctors (two part-time consultants, staff grade/clinical fellow, and F2), a clinical psychologist, nurses, a social worker, occupational therapists, an art therapist, healthcare assistants, and administrative staff.
The Home Treatment Team functions as a step-up, step-down service to prevent unnecessary hospital admissions and facilitate early discharge. They attend ward reviews, support patients on leave, and assist with discharge planning, also gatekeeping inpatient beds. The team includes two part-time consultants, a 0.5 FTE team manager, two FTE senior practitioners, 5.5 FTE nurse practitioners, one FTE social worker, three FTE senior HCAs, and one FTE occupational therapist. Covering all adult ages, there is an arrangement with an Older Adult Consultant for additional support if needed.

Currently, CAMHS manages 1,627 open cases. In 2022, 926 new referrals were received, with 752 new referrals to date in 2023. The current locum postholder\'s caseload is approximately 90, with most cases having an allocated care coordinator. While most new assessments are conducted by other members of the multi-disciplinary team, some will require an initial medical assessment. Access to Neurophysiology services is excellent, with an experienced and fully staffed department supervised by a consultant Clinical Neurophysiologist. The department is expanding its range of services to include intraoperative spinal monitoring, quantitative small fibre sensory testing, and 24-hour home video EEG recording.

What is the department & team like?
The Department of Acute Medicine, nestled within the Division of Medicine, operates as a vital component of their healthcare ecosystem. The team of seven dedicated consultants collaborates within the recently refurbished Emergency Assessment Unit (EAU), equipped with state-of-the-art facilities. With a primary focus on providing efficient and compassionate care for their local community, the department remains committed to adapting and expanding their acute services in response to evolving emergency care strategies.
As a Consultant in Acute Medicine, the successful candidate will play an integral role in their interdisciplinary approach, working closely with Specialty Medicine, Surgery, and Diagnostic Imaging departments to ensure comprehensive assessment processes for all medical emergencies. Effective communication and seamless collaboration with allied health professionals and consultant colleagues within EAU are essential components of their mission to deliver excellence in emergency care.
Career Development & Opportunities Available
The Trust supports the consultants with opportunities to support the multi-professional teaching and development along with active participation in teaching programmes for Foundation, IMT, GP, Higher Specialty Training. Consultants on the specialist register will be given autonomy and will freedom to develop their special interests, with access to additional leadership, research and teaching duties available for those that want it.
All consultants will receive a full induction process and will be involved in clinical audit and governance.

The Acute Medicine Department and Team
The Acute Medical Unit (AMU) serves as the primary gateway for all medical admissions, with exceptions for those requiring direct admission to specialized units such as ITU, HDU, CCU, or Stroke Unit. Recently reorganized, AMU comprises an Assessment area, an ambulatory area, and short-stay beds with a maximum duration of 48 hours. The unit is staffed by three consultants, who provide flexible coverage throughout the day, gradually reducing to two consultants in the afternoon and one in the evening. A goal is to ensure all patients, barring overnight admissions, are seen by a consultant within 4 hours of arrival, with a further consultant review within 12 hours of admission.
The Same Day Emergency Care (SDEC) unit, inaugurated in July 2022, aligns with national guidelines, facilitating the swift evaluation and management of patients within a single day. The unit, jointly serving surgical and medical specialties, promotes collaborative care. Plans within the initial year involve optimizing patient throughput. Consultants allocate one day per week to the unit, which also offers rapid access Acute Medicine clinics to support early discharge from the Acute Medicine department.
In terms of consultant medical on-call responsibilities, the medical team admits an average of 50 to 75 patients over a 24-hour period. Consultants conduct reviews of medical admissions in the afternoon of their on-call day, commencing with a post-take ward round around 1:30 pm. The following morning, acute physicians review admissions to AMU from the previous evening and overnight, with the on-call consultant attending the 9 am handover and completing reviews of any remaining admissions on outlying wards.
