WHAT IS THE DEPARTMENT & TEAM LIKE?
The Mental Health Team provides a service to the towns within the area. The post holder will provide clinical leadership within the Community Mental Health Team alongside two Consultant colleagues. It will be expected of the post holder to be actively involved in strengthening relationships with external agencies including the local CCG. Total population served by the Team is 280,000 with the 19-64 population being 162,000.
The Team comprises: 1.0 wte Band 8b Service Manager, 3.0 wte Band 7 Operational Team Leaders, 1.0 wte Band 7 Lead Nurse, 18.27 wte Band 6 Community Mental Health Practitioners, 2.0 wte Band 5 Community Mental Health Practitioners, 1.0 wte Band 7 Occupational Therapists, 1.0 wte Band 5 Occupational Therapist, 1.0 wte Band 3 Occupational Therapist Assistant, and 6.2 wte Band 3 Support Time & Recovery Workers. The team also works in conjunction with 3.0 wte Band 7 Primary Care Mental Health Workers. The medical input is provided by 3 Consultant Psychiatrists and 3 Speciality Doctors.
At present the team receives 200-250 referrals a month. The case load of the Community Mental Health Team is approximately 1000 patients the majority of whom are allocated to healthcare professionals/care coordinators. The post holder will be expected to take an active leadership role in screening of referrals as well as assessment of new referrals jointly with other team members. The post holder will attend Trust meetings including Strategic Development Meetings, Clinical Governance Meetings within the Service Line and other meetings as requested.

The Mental Health Team provides a service to a town, and it’s surrounding villages. The post holder will provide clinical leadership within the Community Mental Health Team alongside one Consultant colleague. It will be expected of the post holder to be actively involved in strengthening relationships with external agencies including the local CCG. Total population served by the Team is 130,00 with the 19-64 population being 75,120.
The Team comprises 1 locality manager, 2 operational team managers, 1 whole time equivalent senior practitioner (Nursing), 6 full time equivalent CPN’s, 2 whole time equivalent occupational therapist, 6 full time STR workers, and 1 Non Medical Prescriber. The current case load varies between 500 and 600 cases with a referral of approx. 5 cases per day. The consultant will work according to the principles highlighted in the New Ways of Working so all CPA cases will be carried by a care coordinator with consultant being consulted in case management.
The post holder will be expected to take an active leadership role in screening of referrals as well as assessment of new referrals jointly with other team members. The post holder will attend Trust meetings including Strategic Development Meetings, Clinical Governance Meetings within the Service Line and other meetings as requested. The post holder will build constructive relationships with the team manager and with other senior professionals.
